TO QUALIFY In order to qualify for the Caroline Families First program you must have a child who: -Is a resident of Halton region -Is between 2-21 years of age -Presents with challenging behaviours, mental health concerns, and be actively involved with a physician for these needs -If in residential care, must be within Halton and with a plan to return home -Is involved with multiple systems including community services, doctors, a school, mental health agency, the youth justice system, etc. -Has legal guardian and/or family willingness and the ability to participate in the program The following is a list of family doctors and paediatricians that have referring privilege to the Caroline Families First program. WHAT ROLES DO CFF STAFF FILL?

Family support providers are parents with lived experience raising children with mental health struggles. They provide support, guidance, and

navigation for families.

Care coordinators oversee the care plan, implement the wraparound process, communicate with each care team and connect with

community resources.

A multi-disciplinary steering committee meets monthly to ensure that the program is working well and properly addressing client needs.

WHAT IS WRAPAROUND CARE?

Wraparound care is a model of care where a team of individuals who are relevant to the well-being of the child or youth collaboratively develop an individualized plan of care, implement this plan, and

evaluate its successes over time. The wraparound plan includes formal services and interventions, community services and interpersonal

support and assistance provided by friends, family and other people drawn from the family’s network.
