We have relocated our Alma location to the other side of town and now have dental and medical services offered as an addition to the social services the community has come to count on. Now we are welcoming change here in Wamego with the new program called "No Place Like Home" a program that as- sists Seniors maintain a healthy lifestyle that will help them stay at home. We hope that these changes will help people and meet the needs for the community and empower them to achieve whole person wellness. Wamego, Kansas 2.27.2017 Community Health Ministry launches new program to promote ‘aging in place’. The CHM - No Place Like Home program is designed to help the aging population remain in their own, more affordable, homes longer. “The average monthly cost of an assisted living apartment is $3,500 and the average cost of a nursing home is $6,235, so even post- poning a move to a nursing facility by just a few months can have a major financial impact” according to Rick Hernandez, CEO of Community Health Ministry. The CHM-No Place Like Home Program is intended to help elderly members of our community, who require some form of assistance to maintain a safe and independent life style while remaining in their own more affordable homes. This free service provides a monthly (or as needed) community liaison visit. The trained community liaison will evaluate the paent’s needs and make the appropriate arrangements to meet those needs. The community liaison will assess the house for safety and/or ease of access (what adjustments can be made for ease of cooking, bathroom safety, etc.), provide a physical evaluaon (weight/blood pressure), check the mental well-being and even look aſter the cat. When a need is idenfied, the community liaison will work with the appropriate service provider (including but not limited to CHM services) to address this need. The community liaison can also assist with or find assistance for Medicare and/or address insurance quesons or concerns. Although the CHM-No Place Like Home Program does not directly provide cleaning services, transportaon, financial assistance, house repairs, etc., the service will find resources for these services (including but not limited to CHM services). This program is not intended to replace any exisng church outreach or home health service but rather serve as a companion to these services. It is meant to be an advocate for the paent. Many, many needs can be addressed through this service. It will also give piece of mind for distant relaves or caregivers. CHM is hoping that the support this program will provide by having someone checking in to make sure the medicine is being refilled, the light bill is paid, there is adequate and appropriate food in the house, loved ones are geng to appointments, have a social/church life, etc. is enough to extend the paent’s ability to stay in their own home longer. Members become eligible for Thrivent Choice based on insurance pre- miums, contract values and Thrivent volunteer leadership. Members are designated Choice Dollars that they can direct to enrolled charies. Both CHM Wamego and CHM - Alma Outreach are registered Thrivent Choice Charies. Mary’s adult daughter brought her in one afternoon. Mary’s husband had knocked her front teeth out earlier that morning. Unfortunately, our dentist was not in but our receptionist knew she needed more than dental assistance. Mary met with the nurse who found out she had just come from the emergency room and her physical wounds had been taken care. Mary was in great pain and had prescriptions for pain and nerve medication, but was afraid she did not have the money to pay for them. CHM does NOT pay for these types of medications, no matter the circumstances. The nurse connected her with the social department to see what they could help her with. Her case manager put her in touch with The Crisis Center, Inc. of Manhattan, a service for victims of domestic abuse and sexual assault. In the meantime, the case manager called the pharmacy to see what the two prescriptions would cost. They came to $22. Her daughter said she could pay for that. After talking with the Crisis Center and making a safe plan for her for the weekend, Mary met with CHM social worker. Mary and her younger children had a safe weekend and then met with the Crisis Center representative at CHM first thing Monday morning. Mary returned to see our dentist on Wednesday. We have helped her with groceries, LIEAP application and more so she can put her money towards the mortgage payment. She stills receives support services from the Crisis Center, CHM social, dental, medical, behavioral services and Ruby Slippers. Changes

The Kansas Mission of Mercy annual dental clinic was in Manhattan, KS on January 27-28, 2017. Volunteer dentists and hygienists provided 1,041 patients with $1,102,222 in care.

Thanks CHM staff for volunteering at KMOM, a free dental clinic, held in Manhattan this year. This is an annual event and CHM is proud to be a part of it! Thanks staff!

The Dental Clinic will be offering hygiene and assessment services for the uninsured or under insured by appointment only. Upcoming Clinic dates are February 23rd, March 23rd, April 13th, May 23rd. Clinic hours are 9:00am to 12:00. A copay of $30/$40/$50 is determined on a sliding fee scale. Proof of income for the household is required. Without proof of income, it will be the $50 copay. Copay due at time of service.** We can bill Medicaid (KanCare) and private insurance if un-der insured. The best time to make an appointment is Monday mornings at 10:00am, call the Wamego office at 785-456-7872. The Medical Clinic will provide doctor services for the uninsured or underinsured. The doctor will be in, on the 4th Thursday afternoon of the month beginning February 23rd.* Clinic hours will be 1:00 to 4:00. A copay of $10/$20/$30 will be determined on a sliding fee scale. Without proof of income, it will be the $30 copay.** Call the Wamego office for an appointment 785-456-7872. *Hours will be expanded as needed. ** Co-pay is due at time of service. If you need to see the doctor or hygienist but do not have the co-pay, please request payment assistance information.
