Dental Charts

Some dental assistants may be required to chart conditions of the teeth on dental charts. Dental charts are legal records and must be complete, neat, and correct.

The Sections of a Dental Chart❖ Personal Patient

Information ➢ Name, birthdate,

address etc. ❖ Medical History

➢ Allergies, past medical conditions etc.

❖ Charting Area➢ Anatomical or

Geometric Diagrams of the teeth

Universal Numbering System

● A notation system for associating information to a specific tooth

● Primary teeth/Baby teeth, are labeled with letters

● Permanent teeth are labeled with numbers

● Occlusal: Numbers 1 and 32● Incisal: Numbers 8 and 25● Buccal: Numbers 3 and 30● Labial: Numbers 9 and 24● Lingual: Numbers 5, 10, 23, and 28● Mesial: Numbers 6 and 27● Distal: Numbers 12 and 21
