    DSM In 1952, the American Psychiatric Association's

    Committee on Nomenclature and Statistics

    published the first edition of DSM (DSM-I).

    Four editions have been published since then:

    DSM-II (1968);

    DSM-III (1980);

    A revised DSM-III, DSM-III-R (1987);

    DSM-IV (1994).

    Multiaxial Evaluation in DSM-IV Axis I : consists of all mental disorders except those listed

    under Axis II, and other conditions that may be a focus ofclinical attention (Table 3-2 )

    Axis II: consists of personality disorders and mental

    retardation (Table 3-3 ) Axis III: lists any physical disorder or general medical

    condition that is present in addition to the mental disorder. Axis IV: is used to code psychosocial and environmental

    problems that contribute significantly to the developmentor the exacerbation of the current disorder

    Axis V: is the Global Assessment of Functioning (GAF)scale with which the clinician judges the patient's overall

    level of functioning during a particular time period
