New York Relay ServiceThe New York Relay Service relays calls between a person using a TDD and any other telephone user within the state. Chazy & Westport Telephone Corporation was incorporated in the State of New York in 1903. Connecting the communities we serve with pro-gressive telecommunications, while remaining small, flexible and charitable. Those communities include Chazy, West Chazy, and Westport, as well as portions of Altona, Champlain, Elizabethtown, Essex, Lewis, Scotia, and Wadhams.

Westelcom, our wholly owned subsidiary, delivers comprehensive voice and Internet solutions on a fiber based network. Boasting stellar customer service, expert engineering and innovative concepts, we design and deliver some of the most sophisticated services in telecom today. In additional to deploying fiber technologies at Gigabit levels, Westelcom has also widely deployed Metro Ethernet Networks, Disaster Recovery concepts and has constructed a Carrier Neutral Data Center. Westelcom serves the business community from Lake Champlain to Lake Ontario.

HERE ARE A FEW NOTABLE DATES THAT WE ARE PROUD OF:

1941: First dial office in the North Country was installed in Chazy and West Chazy.

1952: Gerald R. Forcier purchased Westport, Essex and Lewis Telephone Co.

1958: Westport installs one of the first digital offices in Essex County.

1959: Westport Telephone Company purchases Chazy Telephone Company, alas the name Chazy & Westport Telephone Corporation came to be.

1968: The Chazy dial offices were replaced.

1972: We were one of the first in the North Country to start offering touch-tone service and direct distance dialing without operator assistance.

1981: The first Digital Central Office in the North Country was installed in Chazy and West Chazy. Westelcom, a full service interconnect, was formed to provide customers in our franchised area and in the surrounding North Country with the communication needs of our growing environment to include phone systems and voice mail systems.

1984: Westport became the first digital office to make Chazy & Westport Telephone Corp. the first Telco with more than one fully digital central office in New York State.

1993: The Chazy and Westport Central Office were updated to accept modern and future technologies such as SS7 and CLASS features like Caller ID. This change positioned the company to assure that we provide our customers with the most up-to-date, state of the art technology.

1994: First digital, high-speed video conferencing system in the North Country was installed in the Chazy and Westport offices.

1994: C&W ran first fiber in Chazy and Westport to meet Verizon and connect to the outside world.

1996: Westelcom offers direct Internet Access to the North Country.

1997: Westelcom purchases Mr. Computer, a retail computer business, and renames it Westelcom Computer.

2000: Chazy & Westport Telephone offers Long Distance Service and High Speed Internet Access, two major innovations implemented to keep the community level with the growing Telecommunications industry.

2001: Westelcom Family of Companies acquires Gisco and NorthNet, two competing Internet Service Providers in the North Country to expand their reach across the North Country. This acquisition makes us the North Country’s largest local Internet Provider, with over 30,000 customers. Westelcom’s local facilities voice and data launch in Watertown marks a first for the area, a LOCAL carrier for voice services that is able to react and respond to the local community’s needs.

2002: Chazy Westport High Speed Internet (DSL) is available to 100% of their territory. This feat is more than uncommon in the industry.

2007: C&W started deploying FTTH (Fiber To The Home) in May of 2007

2008: Westelcom starts deploying Fiber Optic Technology to businesses in Watertown and Plattsburgh.

2011: Westelcom brings Fiber optics to Elizabethtown businesses and lands the deal to construct and deliver fiber internet for the Telemedicine Network.

2012: Westelcom begins Fiber construction to Malone businesses as well as Ticonderoga in conjunction with the Fiber Optic Telemedicine Project. The company also completed the construction of their Non-Specific Carrier Neutral Colocation facility located in Plattsburgh.

2013: Chazy Westport starts deploying Fiber Optic Technology to the homes to Chazy, West Chazy and Westport. Westelcom builds to Clayton, NY.

2014: Westelcom launches Hosted Phone Systems In an effort to offer advanced telecommunications to businesses in the region.

2017: The NYS Broadband Grant to bolster fiber deployment was awarded to CWT. The company built 117 miles of fiber to bring higher internet speed to 530 homes.

2020: Westelcom acquires Teo, a west coast Unified Communications provider.

Recording Telephone ConversationsVoice recording equipment can be provided to customers and connected to their telecommunications equipment. When the connection is made, the voice recording equipment must be arranged so the user can activate and deactivate it at will.

Under both federal and state law, at least one party to a telephone conversation is required to give their consent prior to the recording of the conversation. Wiretapping or any interception of a telephone call without this consent, other than by a properly authorized law enforcement agent, is a crime. Punishment may include imprisonment and/or a fine.

PHONES NOT WORKING? FOLLOWING THESE STEPS COULD SAVE YOU MONEY!Springtime is notorious for phone outages from lightning storms. We advise you to test your lines prior to contacting our Telephone Office, to be certain a damaged phone isn’t causing the problem. It could save you a service charge.

Step 1: Unplug every telephone, cordless phone and answering machine and leave them unplugged.

Step 2: Take one telephone (not a cordless) and plug it into a phone jack.

Step 3: Pick up the phone and check to see if you have a dial tone (if you don’t have a dial tone, plug the same phone into a separate jack and continue until you have checked all your jacks). If this doesn’t work, and you have another phone, do the same.

Step 4: If you get a dial tone, then you have a damaged phone. One bad phone can disable the rest of the phones. By unplugging the bad one, your telephone service will be restored.

Step 5: If you do not get a dial tone and would like to test further, test the NID (Network Interface Device) or call our technical support.

After you read this, you should keep it in mind for future reference.These rights and responsibilities result from New York State Public Service Commission (PSC) Rules Governing Provision of Telephone Service to Residential Customers.

QUESTIONS OR PROBLEMSContact us as soon as possible if you have any questions or complaints about your telephone service. We will try to give you a prompt answer. Our representatives are available to help you between 9:00AM and 4:00PM during business days.To get your question answered fully and as quickly as possible, we suggest you call our Business Office at the telephone number shown on the front of your service bill. If you prefer, you can write to Chazy Westport Communications at the address shown on the front of your service bill. Please include your telephone number. Give us as much information about the problem as you can.If you are still not satisfied you can call or write to the Public Service Commission (PSC) at One Penn Plaza, New York, NY 10119. They have a staff available to give you assistance if you request it. The telephone number is 1-800-342-3377 (toll free) between 8:30AM and 4:45PM on business days.While the Public Service Commission is considering your complaint, we will not disconnect your service for failure to pay the amount in question. However, all undisputed bills must be paid when due to ensure continued service.

BILLINGWe have a responsibility to provide reliable telephone service to our customers, and you have a responsibility to pay bills for telephone service promptly. Billing Information: Telephone bills are rendered once a month and are due upon receipt. Payment within ten days is requested. Please return remittance coupon with payment. Local Services Charges are billed for one month in advance. Charges for toll or long distance calls will appear on the same bill covering a period of one month ending about fifteen days prior to the date of the bill. Chazy Westport Com-munications will not accept post dated/or third party checks. A fee is required for all check returns. A detailed listing of the items and charges that are included in your service appear on your bill each month. Charges for installing or changing services usually appear on the first bill after your order is completed.Where to Pay: The most convenient way to pay your bill is online. Please visit our website chazywestport.com and click on Bill Pay. You can also pay by calling in, mail or drop off at one of our business offices.Basic Local Service: Your Basic Local Service is the group of services that are necessary to provide you with dial tone, and all taxes and surcharges that ap-ply to these services. Charges for unpublished numbers and construction are included in this group.Non-Basic Local Service: This group includes the optional services that you choose to enhance your telephone service, such as Custom Calling Features and Inside Wire Maintenance.

RIGHTS AND RESPONSIBILITIESIntraLATA Toll Service: Per minute charges for calls placed to points that are within your (518) area code but out of your local calling area, are separated from all other types of long distance charges. They are detailed on the long distance section of your bill.Other Services: Our telephone company bills for some other companies that provide long distance service, operator services, recorded information services or group conversa-tion programs.

PAYMENTSYour payments should reach us by the due date shown on the cover page of your bill. This will give us time to process and credit your payment. If we do not receive your payment by the time your next months bill is prepared, unpaid charges will ap-pear on your next month’s bill and they will be added to your next month’s charges.Late Payment Charge: Customer bills for telephone service are due when they are rendered. A customer is in default unless payment is made before the due date specified on the bill. If payment is not received before the due date, a late payment charge equal to one and one-half percent (1 ½ %) per monthly billing period will be applied to all amounts in arrears. Late payment charges will also be applied to the unpaid late payment charges excluding one month’s local service. Payment Plans: If you are having problems that make it difficult to pay your bill on time, we will work with you to come up with a payment plan. We will take into account your financial situation and any income limits you have when working out a down payment and payment schedule. When you arrange for a deferred payment agreement (DPA), you agree to make monthly payments toward past-due bills and to pay any new charges in full each month. Currently, a deferred payment plan is usually for up to $150 in past-due bills for basic local service. It may be for more in certain special cases. Payment agreements are for a period of no less than 5 months unless you request a shorter period. You may participate in only one payment plan at a time.Quarterly Payment Plans are available. If you are 62 years of age or older, and your yearly telephone service costs are $150 or less, you may arrange to pay your bills every three months. Call your service representative to apply.Partial Payments: If your payment is less than the total amount due, the tele-phone company will apply your payment to basic local service before we credit the other groups of service. In this way, you will not experience an interruption in your basic service. Your ability to use the other services, such as access to your long distance carrier, will be blocked until you have paid the full amounts owed for these services. (This policy does not apply to business customers.)

DEPOSIT POLICYNew Customers: If you are a new customer who has not established a payment record with our company, it may be necessary to ask you to pay a security deposit.Customers 62 Years of Age or Older: If you are 62 years of age or older, we will not ask you for a deposit unless your service was suspended and/or terminated for nonpayment within the past six months.Customers Who Are Delinquent: If you are delinquent in your bill payments, you may be required to pay a deposit or an additional deposit. Delinquent in payment means that a customer has received two bills without making payment of one-half of the total of the two bills.Customers Whose Service Is Suspended and/or Terminated: If your service has been suspended and/or terminated for nonpayment of bills in the last six months you may be required to pay a deposit.

RIGHTS AND RESPONSIBILITIESSOME FACTS ABOUT DEPOSITSAmount of a Deposit: If you do require a deposit, the amount is based on two times the average monthly bill.Payment of Deposits: If you are unable to pay the required deposit amount and request an agreement, you can pay the deposit in installments over a period not to exceed 12 months.Deposit Interest and Refunds: We pay interest on your deposit at a rate specified by the Public Service Commission. We hold a deposit for one year. If you have not been delinquent (as previously described) in the past 12 months, the deposit and interest earned will be applied to your bill, or refunded to you if you request. Oth-erwise we will credit your account with the earned interest, but hold your deposit. The account will be reviewed again once a year to see if it qualifies for a refund.

FINAL TERMINATION NOTICEIf you fail to pay overdue bills or a deposit, we can terminate your service. However, before this is done we would forward a Termination Notice so that you would have an opportunity to arrange payment and avoid interruption of your service.If you are eligible under the Rules of the Public Service Commission, we will offer you a deferred payment agreement before we terminate your service. If you need assistance in understanding the deferred payment agreement you may obtain the assistance of the Public Service Commission. If you sign a deferred payment agreement, we will not disconnect your service as long as you make the agreed upon payments.If there is no question about the bill and you fail to pay the amount shown on your termination notice or refuse to sign a deferred payment agreement, your service will be terminated. Service will not be terminated if you qualify for the special protection described in the section on Special Protections in this book.If payment is not received before the due date, a late payment charge equal to 1½% per monthly billing period will be applied to all amounts in arrears. Late pay-ment charges will also be applied to the unpaid late payment charges excluding one month’s local service.If you fail to pay this debt in a timely manner, we will submit your account to our collections department and you may be responsible for all costs of collection; we will pursue any and all legal remedies we may have against you, and you may also be responsible for all reasonable attorney’s fees expended to collect on your account. Your credit report may also be in jeopardy.

WHEN SERVICE CAN BE TERMINATEDWe are allowed to terminate residential telephone service for nonpayment only between 8:00AM and 7:30PM, Mondays through Thursdays and between 8:00AM and 3:00PM on Fridays . We cannot terminate service on a public holiday, the day before a holiday, any day our business office is closed, or on December 23, 26, 30 and January 2.

RECONNECTION OF SERVICEIf we terminate your service, we will reconnect it within 24 hours after you have paid the amount due or sign a deferred payment agreement and make the down payment required.If service is terminated there is a charge to reconnect the service.

SPECIAL PROTECTIONSMedical Emergency: If you or a member of your household is ill or has a serious medical condition, we can help you if you can’t pay your telephone bill. Send us a medical certificate from your doctor or the local Board of Health describing your medical emergency and we will continue your service for 30 days. The certificate can be renewed, but to do this your doctor or official of the local Board of Health must explain the medical emergency and why service is needed. You must also explain why you are unable to pay your telephone bill by filling out a form telling us about your income and expenses. We will not turn your service off during the medical emergency, but you still must pay part of your bill. You may delay pay-ment of up to $30 for each bill you receive during the medical emergency. You must pay all charges over $30. Once the medical emergency ends, we will offer you a payment plan. Elderly, Blind or Disabled: If you advise us that you are blind, disabled or 62 years of age or older, and all other occupants of your household are under 18 years of age, 62 years of age or older, blind or disabled, and additional 20 days will be allowed before your service is terminated. If you qualify under this protection you should notify us to complete a form for us to keep on file. We can also work with you to set up a payment plan if necessary.Third Party Notification: All residential customers may choose a third party to receive copies of all credit notices. A third party notice may keep your service from being turned off by mistake. You may choose a relative, a friend, a member of the clergy of a community agency to be a “third party” for you if they agree to act on your behalf. A third party will receive copies of any credit notices we sent to you because of overdue bills. The third party is NOT responsible for paying your bills. Notify us if you want to complete a third party form.

ACCESS TO TELEPHONE REGULATIONS (TARIFFS)You’re entitled to see all tariffs filed by Chazy Westport Communications. They are also available at the New York State Pubic Service Commission Offices. Telephone numbers for our Business Office and the Public Service Commission are listed in the informational pages of the telephone directory.

BILLING DISPUTEYour telephone service will not be disconnected for failure to pay charges in question (disputed amount). However, all undisputed amounts must be paid when due to ensure continued service. If you are sending in a disputed amount provide as much information as you can. Please include your telephone number with all correspondence and mail to the address that is on the inside front cover.

ADVERTISING TELEPHONE NUMBERSAdvertising in any form, a residence telephone for business purposes or the regular use of a residence telephone for conducting of a business is not permitted. If your telephone service is used for business it must be classified as a business. Establishing a business classification provides the business customer with added benefits.

MONEY SAVING HINTSYou are subject to service charges if your customer provided equipment is putting your line out of order. Check your phone in a working jack in your home or at the Network Interface Device located on the outside of your home. If you are unsure of what to do you may stop by our office and someone will show you what you should do. You may also use the Chazy Westport Communications test jacks located at our business office. You are responsible for all calls made from your home. Be careful about letting others use your phone unless you’re sure they won’t abuse the privilege. If you have concerns and would like suggestions as how to safe guard your service, contact our business office and a representative will be glad to assist you.

SPECIAL ASSISTANCE SERVICES VISUAL OR PHYSICAL IMPAIRMENTIf you: • Are legally blind with visual acuity of 20/200 or less in your better eye

with correcting lenses, or• Have a visual impairment which prevents you

from reading normal printed material, such as a telephone directory, or

• Have a physical disability (such as loss of hands, loss of use or control of hands, constant severe tremor, spasticity or paralysis, uncorrectable double vision, incapacitating confinement as in

an iron lung, or severely debilitating conditions such as found in advanced Parkinson’s Disease, cancer and the aftermath of stroke)

You may receive:• Free directory assistance as allowed by your long distance company

and/or• Free operator assistance for placing calls (excluding special calls such

as person-to-person, collect, and requests for time)• Reduced purchase prices and rental rates for telephone equipment

CALLING A 900 NUMBER IS NOT FREEA charge will show up on your phone bill. It may be much higher than the cost of other long-distance calls.NOTE: There is also a charge for calls to 700, 976, 540, (and to some 800 numbers after a minimum of free time is used). 700 numbers are designed for conference calls – business consultations, some teen “talk lines,” etc.

ASK AROUNDHave friends heard of the service? Does it have a good reputation?

KNOW THE COSTStudy ads and commercials for 900 numbers carefully. Listen closely to any price information given at the start of a 900 number message. Be sure you understand:• How the fee is set-by the minute or per call?• How long the message lasts, if you’re charged by the minute.

CONSIDER “CALL BLOCKING”If 900 calls create a problem in your family, consider call blocking. Call blocking prevents 900 calls from being direct-dialed from your home* and is provided by Chazy Westport Communications. *NOTE: your local telephone company cannot block 900 calls charged to a calling card.

KEEP TRACK OF 900 CALLSConsider Keeping a Log Book:• Especially if more than one person makes 900 calls.

Use it to record:• Who made the call, and when• The 900 number called• The name and address of the service provider• How much the call cost per minute (or the flat rate)• Any products or services to be received in return for the call.

CHECK YOUR PHONE BILL Compare the 900 charges with the information in your logbook.

IF YOU SUSPECT AN ERROR Or have another problem with the 900 service, contact your local telephone company. The telephone company usually will not disrupt basic telephone service for disputed 900 charges. Depending on your situation, the representative may resolve the problem for you or refer you to the responsible Long Distance Company.

DIRECT DISTANCE DIALEDThese are calls where the caller dials the number directly and no operator assistance is required. Toll rates are lowest for Direct Distance Dialed calls.

PERSON-TO-PERSON CALLSCall person-to-person when you wish to talk to a particular person or exten-sion number. Tell the “OPERATOR” you wish to make a “person-to-person” call. Rates are higher than station-to-station and also apply when you talk to an alternate person or extension. Charging starts when conversation begins.

OPERATOR-ASSISTEDStation-to-station calls requiring operator assistance are charged at a higher rate than those that are completed without an operator’s help. Higher rates are necessary to compensate for the cost of providing these specialized services. Included are calls such as coin phone, collect, code billing, time and charge (including hotel guest calls), sequence calls and calls placed by the Operator at customer request.

COLLECT CALLSYou can call collect; either station-to-station or person-to-person, if the person or business you are calling agrees to pay the charge. Charging starts when conversation begins. Collect Calls can only be made with operator assistance.

TIME AND CHARGESCustomers, when placing calls, may request from the “Operator” the amount of time and the charge for the call. This service requires “OPERATOR” assistance. The call may be either station-to-station or person-to-person.

LONG DISTANCE RATESRates for all types of calls are based on distance, the time that you make the call, the length of your conversation and your service providers calling plan. There is an additional charge if you make an Operator Assisted, Calling Card, or Public (coin) phone call.The initial period for all calls is one minute. Charges are based on the rates in effect at the calling point. If you start a call during one rate period, and continue through to another rate period, you will be billed the rate that applies for each period.

“Slamming” occurs when your long distance and/or your regional calling service is changed without your knowledge and approval. Federal and state laws prohibit “slamming”. You must contact the FCC or PSC within 30 days if you feel that you’ve been slammed.

If you are a victim of “slamming”, first call the long distance or regional calling provider associated with the charge. The company’s name and logo appear on your bill. You are entitled to be switched back to the company of your choice at no charge and to have any disputed charges billed at your original company’s rate. You should write a detailed letter to the Federal Communications Commission (FCC) for help in resolving the dispute.

Protection against “Slamming”As a protection against slamming you may call our business office and request that no changes be made to your long distance or regional

service providers. This is known as a “PIC Freeze”.

“Cramming” is the term used when unexpected and unauthorized charges are unknowingly “crammed” onto your telephone bill. Usually these charges are for phone related services, but charges may also be for added on telephone services or surcharges. These charges generally appear on a separate page of your bill with the billing company’s name. If you suspect a charge has been “crammed” on your bill, contact our business office to have the erroneous charge removed.

CALL SPOOFINGCall "spoofing" occurs when a fraudulent caller deliberately falsifies the information transmitted to your caller ID display in an attempt to trick you into giving away your valuable personal identifiable information. The FCC and U.S. law prohibit most types of spoofing, but with the widespread use of caller id and Voice over IP (VOIP) technology it is becoming easier and more commonplace for call spoofing and robocallers to disguise their identity. The common practice is to manipulate the caller ID displayed in hopes you will fooled into thinking you are dealing with a legitimate representative of your bank, credit card or insurance company, or even a government agency. Is the caller claiming to be a representative from one of the aforementioned agencies? Are they pressuring you for personal information almost immediately? If so, hang up and call the phone number on your account statement, from the phone book, or the company's or government agency's website, to verify the authenticity of the request. Be aware that during the normal course of business none of the above entities will request personal identifiable information over the phone. It is imperative to be very careful about responding to any request for personal identifying information over the phone where you have not initiated the action. Never give out personal identifiable information such as account numbers, social secu-rity numbers, mother's maiden names, passwords or other personal identifying information in response to an unexpected call or if you are at all suspicious.

ELECTRONIC SPAMMINGElectronic spamming is the use of electronic messaging systems like e-mails and other digital delivery systems to send unwanted bulk messages indis-criminately. The term spamming can also be applied to other media like instant messaging, mobile text messaging, and social networking spam. The most recognized form of spam is e-mail spam, but recently it has begun to encompass mobile phone messaging also. Please be aware that e-mail scammers may send you an e-mail that appears to come from CWT/Westelcom. The e-mail may provide you with a hyperlink that redirects you to a fraudulent site and asks you to provide your confidential personal account or security information. While CWT/Westelcom may send you e-mail communications from time to time, we will never request you to send your account number, password or other personal identifiable information via e-mail. If you need to verify if an e-mail is from us please contact our offices (do we want a phone number). CWT/Westelcom is vigilant in protecting our customer’s personal identifiable information and will educate and inform you on the latest trends via our Facebook page. Never give out personal identifiable information such as account numbers, Social Security numbers, mother's maiden names, passwords or other identifying information in response to a suspicious e-mail or message.
