S1.2 Transfer of Fuel Oil and Hazardous Liquids / Waste Management Plan

Comments: Quick dry disconnect fittings seen in Bosun store for use in hazardous liquid transfers on board. Appropriate fittings were available for hazardous liquid transfers over water. Detachable calibrated totalizer meter, for use in the fuel system, was sighted in Bosun store. Electronic fuel monitoring system was not fitted in place. Paints and Chemicals kept in paint store were seen adequately labelled. Hoses found were without floatation collars as per NWE offshore supply Guidelines, but floatation collars were seen in Bosun store. MSDS for last bunker fuel of 50m3 supplied by Bourbon Explorer 506 at Afren Ebok Oil field on 26/04/2015, as required by SOLAS Annex VI, was not sighted. Wastes were properly segregated in waste bins on board and safely disposed. Record of 0.020m3 of category B disposed to sea on 06/05/2015 was sighted.

Comments: Vessel in operating area did not meet statutory requirements for size/type/class as shown in Section S1.1.1 above. Master and Chief Officer have knowledge of the content of the CNL Marine Standard, Guidance and Marine Notices Master and Chief Officer’s signatures, acknowledging that they have read and understood CNL Marine Standard, Guidance and Marine Notices, were sighted. Drinking water on board was supplied in portable plastic bottles. Toilet facilities found clean and functional with no leakages. Living accommodation was found in good hygienic condition with sufficient lighting and effective air-conditioning The galley and dining utensils were found tidy and clean, free of infestation and with pest killer in place. Materials were seen orderly arranged on stable shelves with clear access. Not all crew members have received Chevron Safety Orientation/OE familiarization. Behaviour Based Safety Observation cards were available on the bridge. Record of 10/05/2015 indicated that a welder was not wearing proper PPE while cutting steel plate. Evidence of knowledge of Stop Work Authority was noticed on board. Notice was posted in mess room and crew demonstrated awareness of the ethos during discussion. Stop Work Authority was last used on 10/05/2015 when a welder was stopped for not wearing proper PPE while cutting steel plate. All rotating parts of machinery on board were fitted with adequate guards. Proper and adequate Personal Protective Equipment was seen provided for all crew members, with enough spares for replacement and visitors.

S1.4 Hazard Identification / Culture

Comments: JSA records were found on board. Record of JSAs held prior to hot work on port side fender pad eyes between 18-20/05/2015 was sighted 8 crew members signed the JSA documents. No record of new JSAs for change of weather, work tasks, or people was seen on board. Tasks steps were written out to perform the job as well as address highlighted hazards No record of contractor participation in JSAs found on board. No record of critical behaviour identification, observation, and feedback was sighted. Noise monitoring certificate of engine room not available onboard. Ear muffs were supplied to all crew members working in noisy areas with extras seen at the entrance of the engine room. Gas bottles for welding was seen secured upright. Welding hoses and cables were found in serviceable condition. Flashback preventers for both acetylene and oxygen bottles were seen in place. Portable electric welding machine, sighted in bow thruster room, was well secured and grounded.

S1.5 Mooring and tow/ anchor handling system design review & Safety re-enforcement

Comments: Condition of mooring ropes, wires and stoppers and greasing of deck machinery found satisfactory. Tow spread maintenance programme and towing log was found in place and utilized. Record of comprehensive JSAs held on 01/05/2015 prior to mooring and towing of MT Africa Beauty to Afren Oil Field Ebok, and on 17/05/2015 for towing of Barge DLB 332 from Afren Ebok to Lagos fareway buoy, was sighted. Effective supervision is provided during mooring, towing and anchor handling, particularly for inexperienced seamen by Chief Mate and Bo-sun in conjunction with the Captain. Hazardous zones and trip hazards were clearly marked. Snapback zones were seen clearly marked in Yellow. Effective means of communication with the bridge was with 3 handheld VHF radios, 2 UHF, 2 fixed VHF radios, 1 Public Address & talkback system, and Powered phone talk back system. No visible mechanical defects were observed on the windlasses and capstans.

Marine Inspector shall undertake a review of the mooring arrangements & procedures in place in each vessel: • Check maintenance of equipment. Condition of mooring ropes, stoppers, greasing of deck machinery & fairleads. Provision of PPE, • Tow spread maintenance program and towing log in place and utilized.. • Ensure effective supervision is provided during mooring, towing and anchor handling, particularly for inexperienced seamen. • Ensure comprehensive JSAs are implemented prior to any mooring activity. Ensure Stop Work Authority is reinforced. • Ensure Terminal Export Tug JSAs contain vessel stability condition, reference water tight integrity and the need to ensure openings

which are required to be sealed during towing operations. • Are hazardous zones, trip hazards clearly marked? Check and ensure snapback zones are clearly marked in YELLOW. • Check effective means of communication with bridge • Assess design of capstans or windlasses for potential weakness e.g. slips, trip hazards, hand injuries, bights, line of fire. Suggest

S1.8 Vessel Access / Work Practices

Comments: Safe loading/landing zone was identified on board for personnel transfers. Crew members have not received training in the use of FROG transfer system, and no training DVD available on board. The gangway was adequately secured on one end on main deck of adjacent vessel while the other end rested on the main deck. Life ring, with self- activating light and buoyant safety line attached, was available adjacent to the gangway.. PFDs were available for use while crossing on the gangway. Crew change procedure was found to be in place. Crew rotation is two months On and one month Off. Crew change was arranged by Operation Manager who informed the Master accordingly. No record of crew change during high risk or critical marine operations was seen on board. Walkways on board were provided with adequate lighting, and found free of tripping hazards. Appropriate trash cans available on the bridge, galley and dining room. Designated smoking area was outside of accommodation, at bow winch space, on the main deck. Record of hot work, indicating electronic communication devices were in use within hot work guidelines, was sighted. Caution reflective tapes and watch-man stationed in place to prevent unauthorized passages were used to control entry/exit at work site. Walk areas on board were found to be clear of hazardous openings.

Comments: A ratchet type chain binder for securing of cargo was sighted in Bosun store. Record of JSA including use of tag line prior to crane operation to lift fending tyres on 20/05/2015 was sighted. Fending tyres loaded on board were provided with adequate access lanes for crew members to pass at the time of this inspection. Slings were kept in Bosun store located at the bow deck when not in use. Pedestal crane with telescopic boom certificate of inspection issued by J and G SIME Nigeria Limited on 30/01/2015 and valid till 01/07/15 was sighted. Crane operator on board, Mr Akpoghene G. Samuel, had Crane Certificate of Competency No: AGIL/GOG/PHC/007 issued by Amony Global Inspections Limited on 10/07/2014 and valid till 08/07/2016. Movement warning device (horn) and revolving light installed on the crane were sighted. Davit was not fitted. SWL of 2.00Tons was clearly identifiable on the crane’s telescopic boom. Crane operations were logged in general deck log book on the bridge. Lock out and Tag out system found in place and kits available on board. No defective or damaged tool sighted on board. Appropriate tools were available for use. All power tools were found properly grounded. Guards seen in place for energized moving parts. No damaged electric cords sighted. Electrical systems were found with explosion-proof fixtures. Sufficient overhead power line clearances were provided for electrical systems. All power panels were adequately closed.
