That a permit requires a finding of Suitability by the Director and has submitted with this application an Art Gallery - Complimentary Beer/Wine Permit (Part 2). Permits are granted for a one (1) year period for the dates authorized and that any proposed changed in dates must be submitted to the Director in writing fifteen (15) days prior to any event. All employees serving alcoholic beverages must have Alcohol Awareness Cards and any required Work Cards. In addition, one employee on site must have a valid health care. The applicant will maintain any required health permits. A key employee must be assigned (if not the owner) and be found suitable by the Director to supervise alcohol service (Responsible Party Acknowledgement Notice - Form # PL233) Complimentary service is limited to four (4) hours per event and a maximum of 250 guests per event. The applicant has read the requirements of LVMC Title 6.50 and is aware of the rules and regulations for alcohol service in the State of Nevada and the City of Las Vegas and agrees to follow all such rules and regulation. The applicant acknowledges that this permit does not allow the sale of any alcohol. The applicant agrees that no charge for entry, no tickets and no fee in anyway can be accepted from guests for an event under this permit. The service of beer/wine must be complimentary.
