Class Climate

Log in

Name your questionnaire

You will be asked to create an abbreviation (5 to 10characters) and title for your questionnaire. This isvery important because as you create futurequestionnaires it will allow you to distinguish betweenthem. Your abbreviation and title will appear at the topof the page once you click Apply. You can edit themlater under Form Properties.

Toolbar

The toolbar at the top let's you (from left to right) zoomin, zoom out, reset your preview window, preview as apdf, or preview as an online survey. You can go to adifferent page using the tool on the right.

Place Holders

You can click on any of the following placeholders andinsert text: subunit, organization, author, and survey.

Editor Control -Add Group

1. Opens the dialog to create a new question group2. Opens the dialog to create a new question (must createquestion group first)3. Adds the pole labels defined under form properties (e.g.Strongly Disagree, Disagree, etc.)4. Opens the dialog to add vertical scale labels 5. Adds an empty line 6. Adds a horizontal separator7. Opens the dialog to create a text box8. Opens the dialog to add a picture from the library9. Adds a page break

New Question Group

When you create a new question group, you will haveto determine how you want to measure your data usingthe indicator drop down menu.

Question Types

When you create a new question, you can choose fromthe types listed to the right.

Scaled Question

If you choose a scaled question, you have to determineif a participant can abstain from the question. If sochoose "activated" next to Abstention and you cantype in the pole label. Then pick the number ofcheckboxes including abstention. Check the box nextto "Individualize pole labels" and then click the pencilto edit the left and right poles. If you like you canchoose the type of diagram to display your data.

Pole Labels

As a default, if you create a scaled question it willautomatically create horizontal pole labels. To createvertical pole labels, you have to add them first, byclicking "Extended Pole Labels" and then add yourquestion with the appropriate boxes WITHOUTindividualizing your poles.

Open question

With an open question, you can determine how muchspace will be provided for a response and themaximum number of characters it can contain. If youare doing a paper survey, you can check the box forhandwriting recognition.

Single choice question

With a single choice question, you can enter eachoption by typing in the space provided and clicking the "Add option" button. If you need to edit an option clickthe pencil. You can also choose if you want them todisplay as a drop down list (you can not see this on thepdf preview) or checkboxes.

Additional editing

You can also use the Clipboard and Move groups to make anyadditional edits. First, you must hover over an item and ensure that the "Editing Focus" is on. This indicates that you can edit this item. Youcan edit the form's layout etc. under the "Form Properties" group.

(From top to bottom, left to right) Template (Y/N), Maximumpage count, Current page number, Write protection (Y/N),Paper format, Numbering (Y/N), Color highlighting (Y/N),Focus Information, Total number of questions, Maximumnumber of questions, PDF version, and Editor-version of theform

Save form and exit the editor

If you click the "open door" icon you will save your form and itwill exit the editor. Be careful, DELETE is below this option!!!

To create a new survey

1. Click the Folders icon in the Surveys box in the topleft-hand corner of the screen.2. You can create a new folder or choose an existingfolder to create your new survey.3. Then click "Create a new survey".4. Choose the questionnaire.5. Give it a name.6. Choose the type: paper or online. If it is online youcan create several passwords or one password foreveryone.7. Click Finish.

To send out your survey

8. Click the name of the survey you created. 9. Click "Send password via email to participants".

Emailing your distribution list

10. You will see a blank field to add recipient emailaddresses in a vertical list. 11. After clicking submit, you can edit the outgoingmessage to your recipients.12. Check your email addresses (Click Check emails)then click Send email and your survey will be sent!
