Pressure ulcers (bedsores)—costly to treat but totally preventable! Centers for Medicare to stop reimbursing hospitals in ’08 for these costs. Pressure ulcers that are NOT “Present On Admission” will not be allowable as a secondary diagnosis. So how do you prevent pressure ulcers from developing? The best approach is for nurses and physicians to work together to prevent the problem. Nurses will be focusing their efforts on prevention (daily skin and Braden risk assessments, turning, special beds, etc). Physicians need to collaborate with the nurse when developing the patient’s care plan taking into account any skin care needs. Physicians also need to: • Perform and document a comprehensive skin assessment during your history and physical, with special attention to Hi Risk patients- those admitted from Nursing Homes, paralyzed, bedridden and wheelchair bound patients. If a patient comes to us with a pressure ulcer we need it to be clearly documented in the physician and nurse initial assessments. • Document ongoing care and treatment to any existing and/or acquired ulcers care in progress notes in the assessment and plan sections. • Determine and order appropriate pressure ulcer treatments. See below information the new preprinted physician order form to help you order appropriate pressure ulcer treatment. Introducing “Pressure/Decubitus Ulcer Preprinted Physician Order Form” ( Form # 135960).

This Order form easily allows you to document whether the ulcers was Present On Admission (POA) or not. The Pre-printed Pressure Ulcer Physician Order form is now available on the nursing units. When completing the form: 1. Indicate “Yes” or “No” for POA 2. Document specific pressure ulcer orders (collaborate with nursing staff and or Skin Pressure Ulcer Education and Consultation team to determine treatment options.) 3. Sign/date form This collaboration between nurse and physician can only result in decreasing the occurrence of pressure ulcers and treating them more effectively. For more information, contact Luann Capone, RN,MSN Director Nursing Quality (44995 / 20088) or Molly Sammon, RN, Certified Wound Nurse and Nurse Manager for Pressure Ulcer Education and Consultation team 42778/ 23922.
