How to create and record a “client consent” form

PURPOSE - Recording ‘consent’ given by a client is relevant to the client’s interaction with the clinic as a whole and with practitioners. This series shows how to use the template in VO to create a consent form for a particular purpose and how to attach it to client documents in the client file.

PREREQUISITE - Part 5 of the Visual Outcomes Training Manual explains the operation of the Document Manager

To Create a Consent form

1. Open the Consent Creation Wizard

2. Select Create a new Consent

3. Click on Next

4. Type the Consent Base Name 5. Select Web Enabled for client to be able to give consent on VO-iSite

6. Set up an expiry date for the consent given (the nominated term runs from when the consent is actually attached to the client file).

7. Type the consent text

8. Select Next and Finish

To attach the consent to the client’s file

1. Push the client to the Client Director Clinical, switch to Clinical mode by clicking on the switch icon

2. Select the Clinical tab, click on the Document Manager

3. Click on the New Document button in the Document Manager window

4. Select the Consents tab in the New Document window

5. Select the Consent you’d like to attach and click on Save

6. The Consent is visible in the Client Documents window. To open for the client to read - click on the document link

7. To require the client’s consent, click on the selection icon under the Action header

ABOUT THE PASSWORDThe default password is found in the Client Wizard. It should be disclosed to a client when they first join the practice. The Client should change the default password as soon as possible to the password of choice. The client can change the password using the personal login to VO-iSite. The client can be re-issued with the default password if the personalised password is forgotten

10. Select the Client Logon tab and find the Default Web Password

NB Remember, the password is case sensitive

8. Now the Client password window appears

9. Open the Client Wizard by clicking on star icon in the Client Director

13. If the consent is current, a green tick appearsunder the Latest header.

An expired consent shows as a red cross

12. The Consent given is visible in the Document Manager under the Consented header

11. Observing Privacy protocols, the Client enters the password and clicks the Consent button
