Things to Consider: 1. You have the option of the COM paid GatorCare health insurance through Florida Blue OR one of the State of Florida plans which will be a cost to you. 2. It is imperative that you enter a beneficiary for your life insurance cov-erage. This can be done through myUFL. 3. You have 60 days from your date of hire to enroll in your benefits. 4. College of Medicine and UFSelect plans are enrolled in using myUFL. State of Florida plans are enrolled in using the People First website. 5. Your 403b deductions will start on your first paycheck, however in order to select the funds that you want your money invested in, you must enroll with one of the vendors for each plan. 6. In order to enroll your dependents, you will need their DOB and SSN. If your dependent doesn’t have a SSN, please notify the COM Benefits Manager.

Health Insurance

The College of Medicine provides Clinical Faculty GatorCare coverage at no cost to you. GatorCare is designed to promote improved health care access, quality of care and employee health. GatorCare is a consolidated group health insurance plan available to eligible employee groups associated with the University of Florida and its affiliates. Ga-torCare is a self-insured health plan. Both Florida Blue and Magellan Pharmacy Solutions have partnered with UF to manage the plan’s administration. You may choose between two plan options: Prime Plus or Premium. As an additional option, you are also eligible for health insurance plans through the State of Florida. The cost of these plans are shared between you and the State. You are only eligible to be covered under ONE health insurance plan through the University. If you decide to go with the State plan, you must go in and decline GatorCare coverage.

Life InsuranceAs a clinical faculty member of the College of Medicine, you are provided with a group term life insurance policy at no cost to you. The coverage consists of a $500,000 Life Insurance Benefit and a $150,000 Accidental Death & Dismemberment Benefit. This benefit does reduct to 65% at age 70 and then 50% at age 75. It is also portable, in which new rates and billing fees will apply. *Limitations and Exclusions apply as stated in the booklet Certificate and Master Contract. * For more detailed information and to view the policy document, go to https://financeadmin.med.ufl.edu/com-hr/benefits/certificate-booklets/

You also have the option to purchase additional life insurance for you and your covered dependents, through both the UFSe-lect Plan, as well as through the State of Florida. Rates are age banded.

A free basic group term life insurance benefit of $25,000 is available to all full-time employees through the State of Florida.All employees enrolled in basic term life insurance may elect and pay for dependent spouse coverage.

Disability InsuranceAs a clinical faculty member of the College of Medicine, you are provided with a long term disability insurance policy at no cost to you. The coverage provides a monthly income benefit of 60% of your basic monthly earnings (tax-free) up to a maxi-mum of $15,000 per month. There is a 180 day elimination period. This is a specialty own occupation policy.

You also have the option to purchase additional disability insurance for you through the UFSelect plans. There are 30 day and 90 day plans to choose from.

UFSelect Preferred Legal Plan- low-cost access to comprehensive legal assistance, advice, and representation on different types of legal services.

Baby Gator Child Development Center at Newell Drive- was established as a partnership with the Colleges of Medicine and Public Health and Health Professions. Faculty and Housestaff Members whose children are between the ages of 6 weeks and 5 years are eligible for enrollment at reduced tuition rates. Questions or comments about Baby Gator should be directed to [email protected] Phone: 352-273-8000, Fax: (352) 273-8747. Baby Ga-tor maintains a waiting list for all age groups. Please apply to the waiting list by visiting the website atwww.babygator.ufl.edu. There is a $100.00 application fee that must be paid by either check or money order, after you have completed the application.

Gator Care Now- Same-Day care for all UF and Shands Employees. Located at the Internal Medicine at Medical Plaza. If you are feeling under the weather, you can seek same day care on campus. Please call (352) 265-1669 for an appointment, Monday through Friday 8am-noon.

State University System Optional Retirement Program (ORP)The ORP is a defined contribution plan qualified under section 403(b) of the Internal Revenue Code ad-ministered by the Florida Department of Management Services. The department contracts with five In-vestment Provider Companies to offer a variety of mutual funds and annuities to university personnel. Time Away From WorkVacation (annual) Leave- provided for 12 month faculty. This leave should be taken when it is necessary for a faculty member to be absent from assigned duties and responsibilities for reasons other than for illness. Full-time faculty members on 12 month appointments accrue annual leave at the rate of 6.769 hours biweekly (roughly 22 days/year). A maximum of 352 hours of annual leave may be accumulated. All requests for vacation should be sub-mitted to the chair or supervisor as far in advance as possible. Additional information may be obtained from your department’s administrator. Upon resignation or retirement, an employee may have a lifetime cash out up to 200 hours of annual leave.

Sick Leave- Full-time faculty members earn 4 hours biweekly (roughly 13 days/year) of sick leave. There is no limit on total hours accrued. Sick leave must be earned before being taken. Additional information may be obtained from your department’s administrator. There is no cash-out option for sick leave.

December Personal Leave (PDL)- Full-time faculty members earn four additional days of leave which are credit-ed to their account in December of each year. These days are typically used between December 26th- 31st. Howev-er, due to department staffing needs, these days will be available for use until the end of the current fiscal year (June 30th.).
