Immune Thrombocytopenic Purpura

• Characterized by mucocutaneous bleeding and a low, often very low platelet count with otherwise normal peripheral blood cells and smears.

Immune Thrombocytopenic Purpura

Clinical Features• Usually present:

– Ecchymoses and petechia.– Thrombocytopenia

• May be present:– Mucocutaneous bleeding

(Oral/Nasal mucosa, gastrointestinal, or heavy menstrual bleeding)

• Rarely present:– CNS hemorrhages may occur.

Patient• Petechial rashes in the lower

extremities (chief complaint)• Platelet Count of 29 x 109/L

(N.V. = 150-400 x 109/L)• 1 episode of epistaxis which

resolved with local pressure.• Normal menstrual flow (4-5

days, 3-5 pads)• None present

• Hgb: 120-160 g/L• Hct: 0.36-0.48• WBC: 4.0 – 11.0 x 109/L• Seg: 2.5-7.5 x 109/L• Lympho: 1.5-3.5 x 109/L• Mono: 0.2-0.8 x 109/L• Platelet: 150-400 x x 109/L

Patient’s Values

• Hgb: 120 g/L• Hct: 0.39• WBC: 12.0 x 109/L• Seg: 0.60 (7.2 x 109/L)• Lympho: 0.32 (3.8 x 109/L)• Mono: 0.08 (0.7 x 109/L)• Platelet: 29 x 109/L

• Characterized by low, often very low platelet count with otherwise normal peripheral blood cells and smears.
