On behalf of Northwell Health Cardiology, we would like to extend to you an invitation to the 5th Annual Long Island Cardiovascular Conference, which will be taking place at Atlantis Banquets + Events on Friday, September 20, 2019. Please join us at this unique venue which accents all the East End has to offer as we explore guideline-directed medical therapy in the management of cardiovascular disease.

This year’s theme looks at the American perspective of cardiovascular care and how guideline directed therapy influences management. Dr. Franklin Zimmerman will kick-off the program with an intriguing keynote address on heart disease and the American presidents. This keynote is designed to provide participants with a review of state-of-the-art knowledge using a unique approach, drawn from national experts. Dr. Maria Carney will be providing a special innovation address on the role of caregivers in cardiovascular disease management. By engaging family and loved ones in disease management, the trajectory of care is dramatically improved.

The East End is home to some of Long Island’s most beautiful beaches, historic Montauk, exquisite vineyards, hiking trails and outlet shopping – making it a great getaway. The wonderful feedback we received after last year’s conference inspired us to host our 5th year at this destination setting. Along with a compelling agenda led by our exceptional faculty, we will be concluding the day with a wine tasting and reception at Paumanok Vineyards to follow the conference.

Statement of need: Cardiovascular disease is the leading cause of death among Americans and a major source of significant morbidity. New innovations in cardiac care can save lives as well as improve the quality of life. Structural heart disease such as failure and valvular issues, remain areas of tremendous growth for complex interventions including percutaneous approaches. But guideline-directed therapy remains the foundation of care. With Northwell having received Heart Transplant designation from the New York State Department of Health, this new program for Long Island requires education across the cardiovascular continuum so referring providers understand both when to refer and the management concerns. Additionally, endovascular approaches for the management of valvular disease, stroke therapy and coronary artery disease require new management strategies and guidelines education for referring practitioners. Novel pacemaker device therapies are also now available and education on these options is required for primary care practitioners and other referring providers. This program will focus on the opportunities to treat common cardiovascular diseases in America with guideline-directed and innovative treatments for the hospital and ambulatory settings. Through case-based, lectures, panel discussion and Q&A, participants will learn about updated guidelines and state-of-the-art methods for diagnosing and treating cardiovascular and thoracic conditions. This CME activity will provide a foundation for clinicians to work collaboratively in the effective management of complex patient care to achieve optimal outcomes. Participants will expand their fund of knowledge in guideline-directed therapy and advances in cardiovascular care.Program objectives: All Northwell Health CME activities areintended to improve patient care, safety and outcomes. Atthe conclusion of this conference, participants should beable to:

– Summarize available options for patients on Long Island with advanced heart failure including local access to mechanical support and heart transplant. – Illustrate new techniques for the treatment of arrhythmia including devices that reduce risk and improve survival and outcomes. – Apply strategies for the management of atherosclerotic disease including coronary artery disease and stroke care using a team based multidisciplinary approach. – Incorporate guideline directed therapies including hypertension and dyslipidemia for preventing cardiac disease and reducing cardiac risk. – Employ evidence and empathy in educating patients and their caregivers about goals of care to improve outcomes.

Target audience: MDs, PAs, NPs, RNs in the areas of Cardiology, Endocrinology, Ob/Gyn, Internal and Family Medicine, along with other medical professionals who care for patients that will benefit from attending.

CME accreditation: Northwell Health is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.

Credit designation: Northwell Health designates this live educational activity for 8 AMA PRA Category 1 Credits™. Physicians should only claim credit commensurate with the extent of their participation in the activity.

MOC Statement: Successful completion of this CME activity enables the participant to earn up to 8 MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity.
