IntroductionIntroductionChesapeake Medical Staffing (CMS) offers a very comprehensive benefitspackage designed to meet the various needs of our Clinical Staff.
This presentation will provide you with an overview of the many company benefitsoffered to you during this Annual Enrollment and Election Period.
During this Annual Enrollment and Election Period you will be able to enroll in benefit plans that you may have previously waived out of, or make changes to your current enrollment status without a ‘Qualifying Event’ (QE).
Changes outside of your initial election or annual enrollment periods may be accommodated based on a Qualifying Event (QE). Please see Katelin Sneckenberger in Human Resources for more details.

Benefits Eligibility Benefits Eligibility Full-time employees working 30 hours or more per week, on average, areeligible to participate in the applicable benefit plans on the 1st day of the month following their full-time active employment date with CMS.
Full-time employees are eligible to participate in:

► Voluntary Term Life ► 401(K) Retirement Savings Plan*
* The 401(K) Retirement Savings Plan requires a 1 year of active full-time employment qualification period.

Medical Benefits OverviewMedical Benefits Overview• CMS will offer three (3) CareFirst medical benefit plan options again this
election period. • As a result of the federal Health Care Reform (ACA) legislation, there
have been some plan modifications for the 2014 benefit offerings.• Coverage includes physician services, hospitalization, surgery,
diagnostic services, prescription benefits, behavioral health services, and more.
• Preventative Benefits are covered at 100% - No Deductible or Co-pays– Expanded in August 2012 to include Contraception, HPV testing, etc.
• If the Qualified High Deductible Health Plan (QHDHP) options is elected, the employee will be given the opportunity to enroll in a Health Savings Account (HSA) offered through the HSA Bank. (Enrollment link to follow)

Health Savings Account (HSAs)Health Savings Account (HSAs)• Health Savings Accounts (HSAs) are offered to those employees
enrolled in the CareFirst Blue Cross Qualified High Deductible Health Plan (QHDHP) medical benefit option – Blue Choice QHDHP/HSA Option #3.
• HSAs use tax free dollars to cover expenses such as:– Deductibles; Co-pays; Dental expenses; Vision expenses; Over-the-
Counter medications – All of the IRC Section 213(d) eligible expenses• Maximum annual contributions for HSAs for 2014 are $3,300 Individual
and $6,550 for non-Individual coverage. • Access funds through either the debit card and/or checking account
through HSA Bank. Enroll here: https://secure.hsabank.com/enrollment/
• HSA funds are never at risk of forfeiture or loss.• You may also contact your personal bank and inquire about creating a
HSA account through them.

Voluntary Term Life and AD&D Voluntary Term Life and AD&D Benefits OverviewBenefits Overview
• CMS will offer Voluntary Life and AD&D insurance through Mutual of Omaha. Premiums are collected through payroll deductions and are based on the age of the employee or spouse and the amount of coverage selected. Coverage is available to employee, spouse, and child dependents. Employees pay 100% of cost.
• Employee Voluntary Term Life Insurance:– Minimum Coverage: $10,000– Maximum Coverage: $300,000– Guaranteed Issue Amount: $100,000 (EOI required if increasing or electing)
• Spouse Voluntary Term Life Insurance: – Minimum Coverage: $5,000– Maximum Coverage: 100% of employee’s benefit up to $50,000 maximum.– Guaranteed Issue Amount: $25,000 (EOI required if increasing or electing)
• Child(ren) Voluntary Term Life Insurance:– Minimum Coverage: $2,000– Maximum Coverage: $10,000– Guaranteed Issue Amount: $10,000 (EOI required if increasing or electing)
• Evidence of Insurability (EOI) is required for amounts over the guaranteed issue amount

Benefit Election (Waiver) ProcessBenefit Election (Waiver) Process• You will receive a Kelly Associates Insurance Group (KAIG) customized
enrollment/election/waiver form at the end of the Annual Enrollment benefits communication meetings.
• Due to several plan benefit changes and the ACA legislation, we are conducting a mandatory election or ‘waiver’ process this year. Simply put,
all full-time and benefit eligible employees will need to complete the KAIG enrollment/election form with either a health insurance benefit election or a ‘waiver’ of health insurance election. If waiving coverage, it is required to complete Section #5 – Other Insurance Information as requested on the KAIG enrollment/election form. • Please complete the customized KAIG Enrollment form and submit them to Katelin
Sneckenberger by Monday, November 23rd.
