OverviewWhile antibiotics cannot treat infections caused by viruses, there are still a number of things you or your child can do to relieve some symptoms and feel better while a viral illness runs its course. Over-the-counter medicines may also help relieve some symptoms.

General AdviceFor upper respiratory infections, such as sore throats, ear infections, sinus infections, colds, and bronchitis, try the following:

• Get plenty of rest• Drink plenty of fluids• Use a clean humidifier or cool mist vaporizer• Avoid smoking, secondhand smoke, and other

pollutants (airborne chemicals or irritants)• Take acetaminophen, ibuprofen or naproxen to relieve

pain or fever (read about what is safe to give your child)

• Use saline nasal spray or drops

Sore ThroatTry the following tips if you or your child has a sore throat:

• Soothe a sore throat with ice chips, sore throat spray, popsicles, or lozenges (do not give lozenges to young children)

• Use a clean humidifier or cool mist vaporizer• Gargle with salt water• Drink warm beverages• Take acetaminophen, ibuprofen or naproxen to relieve

pain or fever (read about what is safe to give your child)

Ear PainThe following tips can be used to help ease the pain from earaches:

• Put a warm moist cloth over the ear that hurts

• Take acetaminophen, ibuprofen or naproxen to relieve pain or fever (read about what's safe for your child)

Runny NoseStop a runny nose in its tracks by trying the following tips:

• Get plenty of rest• Increase fluid intake• Use a decongestant or saline nasal spray to help

relieve nasal symptoms (read about what's safe for your child)

• Sinus Pain/Pressure

Try the following tips to help with sinus pain and pressure:

• Put a warm compress over the nose and forehead to help relieve sinus pressure

• Use a decongestant or saline nasal spray• Breathe in steam from a bowl of hot water or shower• Take acetaminophen, ibuprofen or naproxen to relieve

The following tips can be used to help with coughing:

Over-the-Counter Medicines

For children and adults, over-the-counter (OTC) pain relievers, decongestants and saline nasal sprays may help relieve some symptoms, such as runny nose, congestion, fever, and aches, but they do not shorten the length of time you or your child is sick. Remember to always use OTC products as directed. Not all products are recommended for chil-dren of certain ages.

Pain Relievers for Children

For babies 6 months of age or younger, parents should only give acet-aminophen for pain relief. For a child 6 months of age or older, either acetaminophen or ibuprofen can be given for pain relief. Be sure to ask your child’s healthcare professional for the right dosage for your child’s age and size. Do not give aspirin to your child because of Reye’s syn-drome, a rare but very serious illness that harms the liver and brain.

Cough and Cold Medicines for Children Younger than 4 Years of Age

Do not use cough and cold products in children younger than 4 years of age unless specifically told to do so by a healthcare pro-fessional. Overuse and misuse of OTC cough and cold medicines in young children can result in serious and potentially life-threat-ening side effects. Instead, parents can clear nasal congestion (snot) in infants with a rubber suction bulb. A stuffy nose can also be relieved with saline nose drops or a clean humidifier or cool-mist vaporiz-er.

Cough and Cold Medicines for Chil-dren Older than 4 Years of Age

OTC cough and cold medicines may give your child some temporary relief of symptoms even though they will not cure your child’s illness. Parents should talk with their child’s healthcare pro-fessional if they have any concerns or questions about giving their child an OTC medication. Parents should always tell their child’s healthcare professional about all prescription and OTC medicines they are giving their child.

Federal regulators have approved a game-changing immunotherapy cancer drug to treat patients with certain types of lymphoma that uses a patient’s own cells to boost the immune system to fight the disease.

The U.S. Food and Drug Administration’s approval of Yescarta (axicabtagene ciloleucel), is the second gene therapy approved by the agency and the first for certain types of non-Hodgkin lymphoma (NHL).

Yescarta – known as CAR-T cell therapy – has been shown to be effective in treating adult patients with certain types of large B-cell lymphoma who have not responded to or who have relapsed after at least two other kinds of treatment,

“Today marks another milestone in the development of a whole new scientific paradigm for the treatment of serious diseases. In just several decades, gene therapy has gone from being a promising concept to a practical solution to deadly and largely untreatable forms of cancer,” said FDA Commissioner Dr. Scott Gottlieb in announcing the agency’s action.

“This approval demonstrates the continued momentum of this promising new area of medicine and we’re committed to supporting and helping expedite the development of these products. We will soon release a comprehensive policy to address how we plan to support the development of cell-based regenerative medicine. That policy will also clarify how we will apply our expedited programs to breakthrough products that use CAR-T cells and other gene therapies.”

Diffuse large B-cell lymphoma (DLBCL) is the most common type of NHL cancers that begin in certain cells of the immune system.

About 72,000 new cases of NHL are diagnosed in the U.S. each year.

Yescarta is a customized treatment developed to work with a patient’s own immune system to help fight the lymphoma. The patient’s T-cells, a type of white blood cell, are collected and genetically modified to include a new gene that targets and kills the lymphoma cells. Once the cells are modified, they are infused back into the patient.

“The approval of Yescarta brings this innovative class of CAR-T cell therapies to an additional group of cancer patients with few other options – those adults with certain types of lymphoma that have not responded to previous treatments,” said Dr. Peter Marks, director of the FDA’s Center for Biologics Evaluation and Research (CBER).

The FDA approval came after a multi-center clinical trial of more than 100 adults with refractory or relapsed large B-cell lymphoma were found to benefit from it. About 51 percent of the patients treated with the new drug went into complete remission.

FDA officials noted that Yescarta can cause severe, and even life-threatening side effects. As are a result, Yescarta is being approved with a risk evaluation and mitigation strategy requiring that hospitals and their associated clinics that dispense the drug to be specially certified.

Also, patients must be informed of the potential serious side effects and of the importance of promptly returning to the treatment site if side effects develop.

In addition, the FDA is also requiring the manufacturer – Kite Pharma Inc. – to conduct a post-marketing observational study involving patients treated with Yescarta.

The FDA granted Yescarta Priority Review and Breakthrough Therapy designations, which fast-tracked its review and approval.

About 1 in 8 children (8.7 million) aged 17 or younger lived in households with at least one parent who had a past year substance use disorder (SUD), according to a new report by the Substance Abuse and Mental Health Services Administration (SAMHSA). SUDs are characterized by recurrent use of alcohol or other drugs (or both) that results in significant impairment.

In addition, the report shows that about 1 in 10 children (7.5 million) lived in households with at least one parent who had a past year alcohol use disorder and about 1 in 35 children (2.1 million) lived in households with at least one parent who had an illicit drug use disorder in the past year.

“Substance use disorders can have a profound influence on the lives of people and their families, particularly their children,” said Dr. Kim Johnson, director for SAMHSA’s Center for Substance Abuse Treatment. “The data in this report highlights the potential breadth of substance use prevention and treatment needs for the whole family.”

What’s New for the2017-2018 Flu Season?

Getting an annual flu vaccine is the first and best available way to protect yourself and your family from flu. Flu vaccination can reduce flu illnesses, doctors’ visits, and missed work and school due to flu, as well as prevent flu-related hospitalizations. The more people who get vaccinated, the more people will be protected from flu, including older people, young children, pregnant women and people with certain long-term health problems who are more vulnerable to serious flu complications.

For the 2017-2018 flu season:• Only injectable flu shots are recommended for use

again this season.

• The H1N1 component of 2017-2018 flu vaccines was updated.

• There is a quadrivalent recombinant influenza vaccine that is new for the 2017-2018 flu season. (Last season, only trivalent recombinant influenza vaccine was available.)

• For the first time, a cell-grown H3N2 vaccine reference virus was used to produce the H3N2 component of the cell-based vaccine, Flucelvax. (The remaining Flucelvax vaccine components were manufactured using egg-grown reference viruses.)

What flu vaccines are recommended this season?This season, only injectable flu vaccines (flu shots) should be used.

Options this season include:

• Standard dose flu shots. Most are given into the muscle (usually with a needle, but two can be given to some people with a jet injector). One is given into the skin.

• A high-dose shot for people 65 and older.

• A shot made with adjuvant for people 65 and older.

• A shot made with virus grown in cell culture.

• A shot made using a vaccine production technology (recombinant vaccine) that does not require the use of flu virus or eggs.

Live attenuated influenza vaccine (LAIV) – or the nasal spray vaccine – is not recommended for use during the 2017-2018 season because of concerns about its effectiveness.

Are there new flu vaccines this season?A new quadrivalent vaccine (“Afluria Quadrivalent” IIV) was licensed last season after the annual recommendations were published.

A quadrivalent recombinant influenza vaccine (“Flublok Qudrivalent” RIV) is newly available this season.

Why get a flu vaccine?

Yearly flu vaccination is the best tool currently available to protect against influenza (flu), a serious disease which sickens millions of people each year.

The Centers for Disease Control and Prevention (CDC) recommends a yearly flu vaccination as the first and most important step in protecting against flu and its potentially serious complications. Millions of people have safely received flu vaccines for decades. Flu vaccination can reduce flu illnesses, doctors’ visits, and missed work and school due to flu, as well as prevent flu-related hospitalizations.

Reasons to get a flu vaccine:

• Flu vaccination can keep you from getting sick from flu.

• Flu vaccination can reduce the risk of flu-associated hospitalization, including among children and older adults.

• A 2014 study showed that flu vaccine reduced children’s risk of flu-related pediatric intensive care unit (PICU) admission by 74% during flu seasons from 2010-2012

• Another study published in the summer of 2016 showed that people 50 years and older who got a flu vaccine reduced their risk of getting hospitalized from flu by 57%.

• Flu vaccination is an important preventive tool for people with chronic health conditions.

• Vaccination was associated with lower rates of some cardiac events among people with heart disease, especially among those who had a cardiac event in the past year.

• Flu vaccination also has been shown to be associated with reduced hospitalizations among people with diabetes (79%) and chronic lung disease (52%).

• Vaccination helps protect women during and after pregnancy. Getting vaccinated also protects the baby several months after birth.

• A study that looked at flu vaccine effectiveness in pregnant women found that vaccination reduced the risk of flu-associated acute respiratory infection by about one half.

• Another study found that babies of women who got a flu vaccine during their pregnancy were about one-third less likely to get sick with flu than babies in unvaccinated women. This protective benefit was observed for four months after birth.

• Flu vaccination also may make your illness milder if you do get sick.

• Getting vaccinated yourself also protects people around you, including those who are more vulnerable to serious flu illness, like babies and young children, older people, and people with certain chronic health conditions.

Flu Vaccination:Who Should Do It, Who Should Not

Everyone 6 months and older should get an annual flu vaccine, with rare exceptions. Talk to your doctor or nurse if you have any questions about which flu vaccines are best for you and your family.

Vaccination to prevent flu is particularly important for people who are at high risk for serious complications from flu. For a complete list of people who are at higher risk for flu complications, visit: https://www.cdc.gov/flu/about/disease/high_risk.htm.

Some people should not be vaccinated, or should talk with their healthcare provider before receiving the vaccine.

For the 2017-2018 season, CDC recommends use of injectable flu vaccines--inactivated influenza vaccine (or IIV) or the recombinant influenza vaccine (RIV).

People who can’t get a flu shot:

People who should talk to their doctor before getting a flu shot:

• Children younger than 6 months are too young to get a flu shot.

• People with severe, life-threatening allergies to flu vaccine or any ingredient in the vaccine. This might include gelatin, antibiotics, or other ingredients. See Special Considerations Regarding Egg Allergy for more information about egg allergies and flu vaccine.

• If you have an allergy to eggs, talk to your doctor about your allergy. See CDC’s Special Considerations Regarding Egg Allergy for more information about egg allergies and flu vaccine.

• If you ever had Guillain-Barré Syndrome (a severe paralyzing illness, also called GBS). Some people with a history of GBS should not get this vaccine. Talk to your doctor about your GBS history.

• If you are not feeling well, talk to your doctor about your symptoms.

Note: There are certain flu shots that have different age indications. For example, people younger than 65 years of age should not get the high-dose flu shot or the flu shot with adjuvant and people who are younger than 18 years old or older than 64 years old should not get the intradermal flu shot.

Treatment

In the early 1980s when the AIDS epidemic began, people living with HIV were not likely to live more than a few years. However, since 1996, the use of antiretroviral therapy (ART) has

dramatically improved the quality of life for people with HIV.

ART prevents the HIV virus from multiplying inside a person, helps the

body's immune cells live longer, lowers a person's risk of developing a non-HIV-related illness, and reduces the chances

of transmitting HIV to others.

Diabetes Type 3—which is regarded as “brain” specific diabetes—is a dangerous diabetes hybrid that was first discovered in 2005. A study, which was conducted at Brown University Medical School, suggests the brain produces insulin in a way that’s similar to the pancreas. A problem with insulin production in the brain is thought to result in the formation of protein “plaque”—not unlike that which is found among suffers of Type 1 (insulin-dependant) and Type 2 diabetes (insulin-resistant). But in the case of diabetes Type 3, plaque appears in the brain and leads to memory loss and problems forming memories.

What Happens

When it comes to the body, insulin is responsible for helping to convert food to energy. The brain uses insulin, too, but it’s thought insulin’s primary purpose in the brain is to form memories at synapses (the spaces where cells in the brain communicate), notes Time.com. Neurons save space for insulin receptors; insulin makes way for memories to form. In order for the brain to keep making more brain cells, it needs insulin. When insulin receptors flee—as is the case with sufferers of diabetes Type 3—the brain does not receive the energy it needs to form memories.

Alzheimer’s Connection

According to a research team at Northwestern University, insulin may prevent or slow memory loss among those with Alzheimer’s disease by protecting the synapses that form memory. Those with the disease tend to have lower insulin levels and are insulin-resistant. The team found that the reason memory fails when insulin shortage occurs is because amyloid beta-derived diffusible ligands (ADDLs) destroy the receptors in the brain that typically are reserved for insulin, thus making the receptors insulin-resistant. Without the space for insulin, receptors cannot connect, and memory loss occurs.

Possible Cholesterol Connection

According to the Journal of Neurology, Neurosurgery and Psychiatry, there may be a link between cholesterol and cognitive decline among Alzheimer's patients taking lipid-lowering drugs. French researchers note taking certain cholesterol-lowering drugs may result in a slower progression of Alzheimer's disease. It’s thought statins could play a role in keeping Alzheimer's disease from progressing.

Type 1 and Type 2 Diabetes Connection

The American Diabetes Association notes that there isn’t a “standard definition” for diabetes Type 3, though it’s sometimes called “double diabetes” because it has been used to describe those with Type 1 diabetes who additionally shows signs of insulin resistance (the major symptom of Type 2 diabetes). Type 3 diabetes also has been used to describe those with Type 1 diabetes who become insulin-resistant over time. In some cases, taking insulin-sensitizing drugs may be beneficial.

Symptoms of Insulin Function Problems

When insulin function goes awry, a number of symptoms may occur. Some of the most common ones include cravings for carbohydrates and sugars—such as cookies, chips and crackers—and feeling “out of it” or fatigued in the late afternoon. Weight gain in the middle of the body also is common, according to Kathleen Jade, ND.

What Comes Next

Diabetes Type 3 is not completely understood. Diagnosis and treatments remain in the early stages, and mores studies are required in order to fully understand how to help those with diabetes Type 3—as well as its connection to Alzheimer's and dementia.

Early signs suggest that the United States will see a severe flu season, so it's especially important for Americans to get their shots, health experts say. Australia had its worst flu season on record, and what happens in the southern hemisphere typically predicts what happens in the northern hemisphere, said Kevin Harrod. He is a professor in the University of Alabama at Birmingham's department of anesthesiology and perioperative medicine.

"These data tell us that we should see a worse-than-average flu season," Harrod said.

This year's vaccines are combating the H3N2 strain and B strains of influenza, according to Harrod. H3N2 viruses cause more severe disease in the elderly and young children, and they are associated with a high hospitalization rate, he noted. The best protection against the flu is vaccination, Harrod said in a university news release.

"While getting the flu shot may not keep you from getting the flu, it will limit the severity and duration of the illness, and provide you with some protection against future infections in subsequent seasons," Harrod said.

"Even in years when the flu vaccine is a 'bad match,' there is partial protection because one's immune system can make antibodies that still recognize and bind to the influenza virus even when new strains emerge unexpectedly," he added.

Dr. Leah Leisch, an assistant professor in the division of general internal medicine at UAB, noted that the U.S. Centers for Disease Control and Prevention recommends everyone 6 months and older -- including pregnant women -- get a flu shot.

"It is especially important for people at high risk for flu-related complications. This includes, but is not limited to, pregnant women, children younger than 5, adults older than 65 and people with certain medical conditions," Leisch said.

With Severe Flu Season Lurking, Shots a Must

Harrod said the CDC's Advisory Committee on Immunization Practices does not recommend the use of the nasal spray flu vaccine in the 2017-2018 influenza season.

Leisch had some tips on what to do if you think you're coming down with the flu.

"During flu season, if you experience flu-like symptoms, it is wise to let your doctor know within 24 to 48 hours of when the symptoms began," Leisch said. "However, your doctor may not prescribe any medications, as most -- otherwise healthy -- adults under age 65 do not require prescription medication for flu."

For most people, the best therapy is to stay home and get plenty of rest and fluids, she said. Young children, adults older than 65, and adults with certain medical conditions may require anti-viral medications.

"These medications are good at preventing complications of flu and shortening the duration of flu by one or two days," Leisch said. "However, they will not make the symptoms go away immediately."

Potential complications of flu include bacterial pneumonia, ear infections, sinus infections, dehydration and worsening of chronic medical conditions, such as heart failure, asthma or diabetes.

In 2014, about 970,000 Americans were hospitalized due to the flu and more than 40 million were affected by flu-related illnesses, according to the CDC.

While most flu-related hospitalizations and deaths occur among people 65 and older, healthy children and younger adults can also develop severe problems and even die. Each year, nearly 100 flu-related deaths among children are reported to the CDC.

The Statin Use in Persons with Diabetes measure remains a display measure for 2018, but will be included in the 2019 ratings.

Secondary to the importance of the pharmacist’s role in quality measures, many payers review quality measure scores to select pharmacies for preferred network status.
