Placement F2 – Acute Paediatrics

The department The department of Paediatrics comprises 2 inpatient wards and an acute admissions unit.

On average we have 10-20 inpatients at any one time on the inpatient wards with additional 20-30 patients (per 24 hours) on the assessment unit.

There is a large body of Consultants representing all specialties. The department operates with a Consultant of the week system who is responsible for the care of all patients seen on the ward for that week. The department serves Bournemouth, Poole and Swanage.

The type of work to expect and learning opportunities

F2s are on the SHO rota. They split their time between the inpatient wards and the acute assessment unit.

There is lunchtime teaching most days of the week.

Main duties of the placement

The F2 doctor is responsible with other staff for the ward care of patients.

If working on the assessment unit the F2 carries the on call bleep and takes referrals from A&E/GP/midwives.

The F2 then clerks the patients and initiates appropriate management.

Occasionally you are called down to A&E resus (with the paeds registrar) to assist with an emergency admission. There are lots of

opportunities to get involved with practical procedures such as cannulating, venesection and LPs.

Placement F2 - Cardiology

The department Cardiology Department in Poole Hospital. This department consists of two cardiology teams. One is comprised of two consultants, a registrar, a ST grade and two F1s. The other consists of two consultants, one registrar, an ST, an FY2 and one FY1.


Work involves:

Taking referrals ‘on-take’ and working with Acute Physicians on Medical Admissions Unit when on-call

Assessing in-patients and devising a management plan -ensuring one asks for help/advice if needed

History taking and examination of both elective and, when on call, acute admissions

Interpretation of investigations such as blood tests and ECGs Writing referrals to a wide range of specialties Telephone communication skills Liaising with the multidisciplinary team to enable effective patient

care Practical procedures within the FY2s competencies.

Learning opportunities:

A weekly journal club meeting on Monday at 1pm where a topic specific to cardiology is presented and discussed.

The ECHO meeting on Wednesday at 1pm provides an ideal opportunity to learn the basics in the interpretation of ECHOs by listening to the discussion of interesting cases and their outcomes. Additionally, on Wednesday afternoons one of the cardiology consultants (Dr Blakemore) carries out teaching for the junior members of the team which is very useful in learning about the fundamentals of cardiology.

The radiology meeting on Fridays at 12.00h provides exposure to

various imaging modalities used in cardiology and helps to appreciate the application of imaging in cardiology.


Predominantly on the coronary care unit (accommodates 8 patients) and Cranborne (a 30 bed ward) but also entails working on other wards throughout the hospital depending on where patients are located.

Involvement in the ward rounds of the cardiology inpatients and supervising the FY1 doctor in carrying out the jobs that are generated from this. Activities also include clerking in elective admissions and seeing day-case patients on the medical investigation unit.

Working in cardiology also means that the FY2 is placed on the on call rota which involves helping your senior colleagues to clerk in the acute medical admissions. You are also required to come in for the post-take ward round the following morning and present any patients you clerked in on the preceding day.

Placement F2 – ENT (Otolaryngology)

The department The ENT department comprises of 5 Consultants. The ENT patients are cared for within the dedicated ENT/OMF 23 bedded ward. 5 of these beds are allocated for short stay surgery which is closed on Friday evening for the weekend. There are 24 day case trolleys and 13 reclining chairs for use by all surgical and some medical specialties in an adjacent Day Case Unit. There are 3 dedicated Day Case Theatres with a recovery area. The Day Case Unit including theatres is the management responsibility of the Surgical Clinical Care Group. There is also a Paediatric Unit with access to ENT beds.


Ward based during a normal working day. Seeing inpatients on the ward round every day. Seeing and assessing all acute ENT problems in the treatment room on ward.

The department The department consists of 10 very supportive consultants who provide a comprehensive service for older people, covering all the main subspecialties as well as general acute inpatient and outpatient work. They share particular interests in Stroke medicine, Movement Disorders, Intermediate Care, Orthogeriatrics and Falls as well as Acute Geriatric Medicine. Strong ethos of team working for the delivery of high quality care for older people.


Inpatient wards work, on call duties. Close clinical supervision. Ample opportunities for Supervised Learning Events, departmental presentations and audit projects.


Poole Hospital – Philip Arnold Unit and RACE unit with outlier wards to cover.

Placement F2 - Haematology

The department Oncology and Haematology


Daily ward rounds of haematology patients, discuss any problems with consultant over phone, on call duties clerking oncological and haematological emergencies, covering haematology ward patients, covering general medical nights.

Consultants are happy for you to attend clinics, but this is a busy post and this only happens occasionally

There is a weekly teaching session undertaken by either a consultant, or a junior.

Occasionally consultants will do a quick round of their patients on the ward, but there is no fixed time as to when this happens. If you have concerns about a patient, there is a registrar who will review them.

At weekend you cover oncology, haematology and palliative care. There is only one doctor on for all three specialties. There is a consultant providing cover via the phone.

There is an occasional requirement to work nights on the general medical rota (20:30 – 09:00).

On call requirements: Generally one-two on call duties per week. Work about 1 in 4 weekends.





Placement F2 – Medical Microbiology & Virology

The department The Microbiology Department at Poole Hospital consists of 2 consultant microbiologists, the F2, laboratory staff and 3 infection control nurses.


Work entails daily ward rounds of inpatients referred to the microbiologists by clinicians as well as regular visits to ITU and the medical assessment unit. There are multiple learning opportunities including tutorials by the consultants on a range of microbiological topics. The F2 also attends some clinics (e.g. cystic fibrosis, GUM and tuberculosis outpatient clinics) and spends time in the laboratory learning about the various microbiological techniques.


The majority of the attachment is in the Microbiology Department at Poole Hospital although there are 2.5 days spent at Bournemouth Hospital at the GUM clinic.

The F2 attends the daily ward round(s) with the consultant and is expected to ensure an accurate list of the microbiology results of ITU patients is prepared. He/she takes referrals from clinicians and liaises with the microbiology consultants to provide appropriate advice. The F2 is required to follow up blood culture results and complete audit forms for all C. difficile, E-coli and MSSA patients. In addition the F2 undertakes both an audit and a number of literature reviews with formal presentations during their attachment.

The department Neurology Ward. There are 4 neurology consultants and a neurology registrar, one neuro-rehabilitation registrar and a part time rehabilitation registrar. On this ward there are a wide range of disciplines working very closely together, coming together for weekly MDT meetings and neuroradiology meetings.

Placement F2 – O&G

The department St Mary’s & B2 Gynaecology Ward, Poole Hospital NHS Foundation Trust

We have 8 SHOs, three of which are career trainees, one is FY2 and the others are GP trainees. There are three SHOs from Bournemouth hospital who help cover the obstetric on-call shifts.


- Team based i.e. each SHO works for one obstetric and one gynaecology consultant. Each team has one full time, or two part time, SpR’s

- Mixture of obstetrics and gynaecology

- Ample opportunity to spend time in clinics, delivery suite and theatre including assisting in elective Caesarian sections

- Many opportunities to attend department teaching (SHO teaching, Morbidity and mortality meeting, clinical governance and CTG teaching)

- There is also time to attend USS clinics if you are interested

- Very useful if you are interested in O&G, surgery (as there is opportunity practice practical skills and add to your log-book) or GP


The delivery suite, post & antenatal wards and antenatal clinics are based at St Mary's hospital which is across the road from Poole Hospital.

The gynaecology ward, B2, is in the main hospital. The gynaecology on calls is based in the main hospital.

- Ward round of your consultant's gynaecology patients, including post-take as rota'd. Usually this is done before starting the mornings activity

- Attend clinics as rota'd, this includes gynaecology and antenatal clinics. You will be able to see patients on your own but there is plenty of support.

- Assist in theatre. Usually each SHO assists their team each week for the same list but there is also need to cross-cover other SHOs

- Occasional pre-clerking clinics. This is usually limited to a maximum of once a week

- Assess women who are day one post C-section and complete any ward jobs on post-natal ward

- Gynaecology on calls which include clerking new admissions, assessing women on early pregnancy unit and assisting with CEPOD as needed

Placement F2 – Old Age Psychiatry

The department Alderney Community Hospital.

The Department of Old Age Psychiatry consists of 10 Consultant Psychiatrists based within Community Mental Health Teams, Memory Assessment Service and Inpatient services providing mental health services to people aged 65 and older in Bournemouth, Poole and East Dorset.


This post is one of 2 F2 ward based posts at Alderney Hospital, Poole. The F2 is expected to deliver daily medical care and management of patients with cognitive impairment/dementia and associated psychiatric and physical comorbidities, presenting with challenging behaviour.

During the attachment the F2 will be involved in the clerking of patients being admitted to the unit and their ongoing care

within the unit. There is opportunity for community experience with an adult community mental health team, seeing patients in outpatient clinics and in their home environment. The F2 is expected to attend the weekly teaching or journal club, on a Tuesday at St Ann’s hospital. The F2 will meet once weekly for 1 hour for weekly clinical supervision. There will also be opportunity to become involved in clinical audits or experiencing other psychiatric specialty as agreed with the clinic supervisor.

The overall educational objectives are to provide the trainees with the knowledge, skills and attitudes to deal with older patients presenting with psychological or behavioral problems associated with cognitive impairment. They would be expected to:

Take a collateral history from relatives and from a patient if appropriate, perform a mental state examination and to examine a patient

Communicate effectively with patients, relatives and colleagues including GPs.

The F2 will be expected to complete discharge summaries within five days of patients’ discharge. One day a week the F2 will be involved with Bournemouth East (BE) general adult CMHT, seeing patients in outpatient and in their own homes. The F2 is also expected to attend the weekly structured teaching programme offered at St Ann’s Hospital and to teach other members of the multidisciplinary team. The two F2 doctors are expected to provide cross cover for each other during period of leave.

The department Psychiatry : Acute Assessment Unit (AAU) and CRISIS Team

Rotation: Poole Hospital Foundation Programme

The AAU is the 17 bedded general adult admission ward in St Anns Hospital Poole, with an average length of stay of between

2-4 weeks.

The post holder will provide medical backup along with the CT1-3 trainees, ST4-6 doctor and the Consultants in the teams.


The overall educational objectives are to provide the trainee with the knowledge, skills and attitudes to deal with patients presenting with psychological or behavioral problems associated with severe mental

disorders. They would be expected to:

Assess new admissions by taking direct/collateral history, carry out mental state examinations and clerk in new

admissions,

Undertake physical examination of the inpatients and request appropriate investigations (Inc. arranging scans, doing ECGs, liaising with GPs, managing straightforward physical

conditions, arranging ECT etc.).

Prescribe safely, particularly psychotropic drugs Communicate effectively with the patients, relatives and

colleagues within the multidisciplinary team and the

Community Mental Health Teams.

The department Psychiatry : Dudsbury Ward, St Anns Hospital

Dudsbury Ward is an open general adult female ward in

St Ann’s Hospital Poole, with an average length of stay of between

4 to 10 weeks.

The FY2 Doctor will be based primarily on the ward and will provide

medical backup along with the two Consultants and CT1-3 trainee

doctor in the teams. The trainee will have two sessions of working

with a Community Mental Team.


The overall educational objectives are to provide the trainee with the knowledge, skills and attitudes to deal with patients presenting with psychological or behavioral problems associated with severe mental

disorders. They would be expected to:

Placement F2 – Rheumatology & Dermatology

The department Dermatology: comprises 4 consultants, 1 associate specialist, a number of GPSI’s, 1 SpR and a multitude of specialist nurses. Mainly outpatient led with services including simple to complex skin surgery in 3 designated theatres, UVB light therapy, photodynamic therapy, leg ulcer team, and other services. Inpatient dermatology normally between 1-5 patients with complex skin management issues, plus referrals and shared care with other teams.

Rheumatology: 4 consultants and 1 SpR with RFU nurses and extended scope physiotherapists. Mainly outpatient based. Non-cauda-equina back pain on the wards under care of Rheumatology. Otherwise, referrals seen on the ward, with 1-2 inpatients at any time.


The main priority of this job is towards your inpatients. When the SpR’s are unavailable you are expected to see referrals. Clinic attendance is not mandatory but is expected at some point. You are also encouraged to learn certain minor surgical procedures in dermatology including punch biopsy. You are expected to consent for spinal injection lists under rheumatology.

Look after inpatients. Co-ordinate/Filter//see referrals. Consent for rheumatology lists. Admit elective/emergency patient admissions. Advise other teams.


Mon: See inpatients (Derm). Consent injection lists. Rheumatology ward round. See referrals

Tues: Dermatology consultant ward round. See Rheum inpatients. See referrals

Wed: See inpatients both teams. Referrals. Afternoon surgery

Thurs: See inpatients. Grand round. See referrals.

Fri: Dermatology Consultant ward round. Rheumatology consultant ward round. See referrals.

Sun:

+Opportunity for clinic / further skin surgery as inpatient/referral load allows.

On call requirements: 1 in 4 weekends for medicine/DME alternate.
