“It’s more than what we do – it’s who we are.”PIH Health has been dedicated to serving the community for nearly 100 years. Today, amidst a dynamic healthcare landscape, our mission spans across two hospitals, 20 medical offices, countless community programs in partnership with those who share our vision to improve health and wellness, and our generous donors. Collaboration runs deep within our organization, and we are proud to be an integral part of so many thriving partnerships in our local communities.

When it comes to caring for our communities, PIH Health offers prevention-focused programs and nurtures meaningful community relationships that promote health and wellness. Three new grants have strengthened PIH Health’s ability to improve health outcomes and extend our collaboration. Support from the UniHealth Foundation will fund a two-year project at PIH Health Hospital - Downey to reduce low-acuity emergency department visits through a case management model. PIH Health will connect patients who lack primary care doctors to a medical home through Wesley Health Center-Downey, a nearby federally-qualified health clinic. A grant from the California Community Foundation will help PIH Health bring together community-based organizations in the region and foster coordinated approaches to address identified community health needs and reduce health disparities. Finally, PIH Health is proud to be one of five U.S. organizations selected to participate in a two-year international project supported by the Robert Wood Johnson Foundation Global Ideas Fund at CAF America. Selected organizations will exchange information and learn from approaches used in other countries as a basis for enhancing local community participation and decision-making about health initiatives.

Community benefit programs and services are designed to provide treatment and promote health as a response to identified community needs, with specific objectives to:

• Improve access to healthcare services

• Enhance the health and wellness of the community

• Advance medical or healthcare knowledge that provides public benefit

• Relieve or reduce the burden of government or other community efforts

Our Community Benefit Investment

BY THE NUMBERS…

• More than $4.9 million in free or discounted care (charity care) was provided to patients lacking financial resources to pay for needed healthcare services.

• 23,499 community members were reached through evidence-based health education programs and free community-based clinical services.

• 2,551 community members secured health insurance coverage via PIH Health-contracted agencies specializing in Medi-Cal and other publicly funded health insurance programs.

• 1,460 student nurses, physician residents and other individuals studying to become healthcare professionals were mentored and trained for their future careers by dedicated PIH Health employees.

• 10 cancer clinical trials were conducted with approximately 62 individuals participating.

• $96,070 in cash donations was provided to 33 organizations to address significant health needs in our service area communities.

• 40 nonprofit organizations were convened for a coalition development series aimed at improving health. Four workgroups with an average of six community based organizations participating in each were formed around the topics of education, food insecurity, chronic disease and mental health/substance use.

PROFILE OF OUR COMMUNITIES

PIH Health conducts a Community Health Needs Assessment (CHNA) every three years as part of our ongoing efforts to address our communities most significant health needs. The CHNA features a wide variety of demographic and health-related statistics for a geography that represents more than 1.6 million residents in 26 communities across Los Angeles and Orange Counties, including the San Gabriel Valley region.

Investing in Health, Strengthening Communities

Defined as free/discounted health services for individuals who cannot afford to pay and who meet financial assistance eligibility criteria. PIH Health offers a generous program, providing charity care to those with family income(s) at or below 400% of Federal Poverty Level* guidelines.

*For 2015, the Federal Poverty Level for one person was $11,770 and for a family of four $24,250.

The Healthy Los Nietos collaborative was highlighted by the Health Research & Educational Trust in a guide

on building effective partnerships, 2016.

Defined as activities that extend beyond patient care and are typically subsidized by the healthcare organization, although the activities may involve a nominal fee (but no patient care bill is generated).

Health & Wellness Center at Whittier First DayPIH Health Hospital - Whittier, PIH Health PhysiciansA successful collaboration with Whittier First Day, an emergency transitional shelter, the Center meets the healthcare needs of First Day residents and those who are chronically homeless or at-risk.

Light Rail Transit Coalition PIH Health Hospital - WhittierParticipation in this coalition is focused on bringing light rail public transit to the area.

Support GroupsPIH Health Hospital - Downey, PIH Health Hospital - WhittierA variety of free support groups provide emotional support and education.

Transportation ResourcesPIH Health Hospital - WhittierFree shuttle transportation is provided to patients with no alternative means. In addition, taxi vouchers are provided for patients based upon need.

Family Medicine Residency ProgramPIH Health Hospital - Downey, PIH Health Hospital - WhittierTrains resident physicians in primary care and offers rotations in specialty care, as well as tropical medicine, palliative care, hospice, home care, chronic disease management and community health.

Paramedic Base Station Training PIH Health Hospital - WhittierProvides continuing education for regional personnel who work with PIH Health’s Paramedic Base Station, including Fire Department personnel, Mobile Intensive Care Nurses and Emergency Medical Technicians.

Student Education: Internship, Externship, Preceptorship & Mentoring ProgramsPIH Health Hospital - Downey, PIH Health Hospital - Whittier, PIH Health PhysiciansPIH Health supports the development of the next generation of healthcare providers by educating and mentoring students from local schools and colleges in clinical, ancillary and administrative settings.

Defined as clinical programs provided despite a financial loss (after removing the effects of financial assistance, bad debt and Medi-Cal shortfalls) because the program meets an identified community need and, if no longer offered, would either be unavailable or fall to the responsibility of others to provide.

Diabetes Education CenterPIH Health Hospital - WhittierOffers free education programs, screenings and resources to those looking for help with managing their diabetes and living a healthy lifestyle.

Emergency Department (ED) On-Call Physicians PIH Health Hospital - Downey, PIH Health Hospital - Whittier PIH Health’s EDs are some of the busiest in Los Angeles County, with over 100,000 visits annually. On-call physicians ensure that eligible patients, regardless of health insurance status, have access to specialty care. PIH Health Hospital - Whittier’s ED is a STEMI (Heart Attack) receiving center, an Acute Stroke Center and features the area’s paramedic base station, ensuring access to emergency services.

High-Risk Infant Follow-Up ClinicPIH Health Hospital - WhittierA California Children’s Services approved Special Care Center that provides developmental follow-up for optimum health and development.

PIH Health Hospice HomesPIH Health Hospital - Whittier Provides community members with the option for compassionate end-of-life care and family support at two PIH Health hospice homes.

PIH Health Urgent Care PIH Health PhysiciansAs an alternative to the emergency room, urgent care providers treat a variety of illnesses and injuries.

PIH Health Clinical Trials ProgramPIH Health Hospital - WhittierClinical trials and research studies have been aimed at improving health and cancer care since 2001. Research has focused on breast, colon, kidney and lung cancers, as well as leukemia.

Community Benefit OperationsPIH Health Hospital - Downey, PIH Health Hospital - WhittierThe Community Benefit Department serves as the liaison for PIH Health’s Community Benefit Oversight Committee and is charged with ensuring successful fulfillment of community benefit mandates, as required of nonprofit hospitals, as well as establishing linkages with community partners.

Community and Health Screenings Chester Cheng OD, Deep Dudeja MD and Nathan Hinds OD partnered with local Lions Clubs to provide approximately 500 individuals with services such as free vision screenings. Those most in need were connected with resources for free eyeglasses.

Emergency and Disaster Preparedness/Disaster Resource Center (DRC)PIH Health Hospital - Whittier is one of 13 Los Angeles County designated DRC’s established by the County to ensure that the community has access to resources and the highly skilled staff needed to respond to a large-scale disaster or other public health emergency. As a DRC, PIH Health’s Whittier campus is the umbrella hospital for 10 other local hospital facilities, including PIH Health Hospital - Downey.

Emergency Clothing for At-Risk PatientsPatients who are unable to return home in the same clothes in which they arrived receive clothing from PIH Health’s Emergency Clothes Bank, donated by PIH Health employees or purchased through PIH Health Foundation funds.

Environmental and Recycling Projects PIH Health is implementing implementing more “green” initiatives, including increased usage of LED lighting to decrease consumption of electricity, and expanding a methane gas project with the City of Whittier, whereby methane gas is converted into electricity.

Every 15 Minutes Program Joined the California Highway Patrol, Los Angeles Coroner, police departments and emergency responders to hold Every 15 Minutes Programs with local high schools, simulating a fatal crash scene to create awareness of the consequences of driving under the influence of drugs or alcohol.

Eyeglass Recycling for Lions ClubThe PIH Health Eye Care Center collects used eyeglasses from the community and donates them to the Whittier Lions Club, which in turn sponsors missions to distribute eyeglasses to those in need world-wide.

Medical MissionsSy Oang MD traveled on a medical mission to provide health services in the remote countryside of Cambodia, with the team seeing approximately 5,000 individuals in a one-week period. Yong Park MD traveled to Vietnam with a team of missionaries to meet the medical needs of over 200 individuals, ranging from needy families to the elderly, and also taught at local universities. Kennith Thompson MD traveled to the Philippines with a surgical team to provide services in a remote region, completing 94 major surgeries and 146 minor procedures.

PIH Health Volunteer Contributions Some 1,030 PIH Health volunteers collectively contributed 103,468 hours of service, supporting departments throughout the organization.

Reach Out and Read ProgramPIH Health Physicians’ Department of Pediatrics has partnered with Reach Out and Read, an evidence-based initiative that promotes early literacy and school readiness. Upon each regular check-up from the age of six months to five years-old, physicians provide patients with a children’s book and discuss with parents the importance of reading aloud. Each year, over 6,000 new books are distributed.

This report is submitted to the Office of Statewide Health Planning & Development in fulfillment of California Senate Bill 697, which defines community benefit as “a hospital’s activities that are intended to address community needs and priorities primarily through disease prevention and improvement of health status.”

This report also informs PIH Health’s IRS Form 990 Schedule H tax filing, which reports a hospital’s benefit to the community through free or discounted care, health research, health improvement programs and financial support of nonprofit organizations addressing identified community health needs.
