Objectives:1. To anticipate the needs of every individuals beyond those cycle of their life.2. To become much more tolerable and skillful in terms of rendering health care
services and to meet their amenities.3. To be much more aware of what the people perceive and anticipate about their
statistics of life and how to enlighten their minds.
Living with the community people is not an easy thing to do because I am not familiar with the
people. In our 5 days community immersion in Bgry. San Juan, Quezon, Isabela. I had learn so many
things and I am challenged with kind of life they had.
On our first day, we had familiarized the place before we planning the necessary things we must
do in order for us to provide adequate care to the people. Our C.I Mr. Tammy Liwag didn’t come with us
in visiting the people because he wants us to developed independent in ourselves. On the second day, I
and some of my group mates went to purok 7 or cuntapay were it was far from other purok. Going their
was very tiring for us because we had walked kilometers to reach it when we have arrived, I so how
difficult and heart breaking their condition was. The people really need attention and care. They have
told us that the health providers in Brgy. San Juan often go their because of the distance you will travel
before reaching it. I am happy dealing with them because they really kind to us. They are very thankful
to us, Because of simple things we are helping them and giving the necessary knowledge they need in
changing their life.
The experienced we had, in living with the community people is a thing should be
treasured. It had made us understand how it really hard to live with a far barrio. They should not
loose hope in making their life better. They should use their condition us their a challenge for
them to strived more for their future.

LEARNING FEEDBACK DIARY(Brgy. San Juan, Quezon, Isabela)
Objectives:1. To live with the people and to understand their health situation.2. To render health services to the people in the community.3. To develop my skills and capacity as a student nurse to serve the people without
any personal intention.
I am excited when we have our community integration at Brgy. San Jaun, Quezon Isabela. The
community people warmly welcome in their place. They treated us the way every one wants to be
treated because of their hospitability; I was able to adapt myself in a new environment.
A week of staying in the community is a great experienced in me, especially in mingling different
people and serving them. My group mates were so willing to ingrate with the people by living just the
way they live. I know that by putting ourselves in someone shoe is the best way to learn about others. I
was also able to give health service with the people by taking their vital sign and giving them simple
health information. It was a good feeling to see how happy the people are when I give simple teachings I
saw how wiling them to learn. I learned that the most important service to be given to them should be
health services because this is their primary needs.
As a student nurse i also learned to understand and respect other personality’s culture, belief of
other people. We hope somehow, we have done them good upon staying in their humble community. It
was fulfill experience to be apart of a community diagnosis especially if I see the smile of the people who
are happy on what we are doing for them. This experience have given me more inspiration to help and
served other people. All the things we made are nothing without the guidance of our very supportive C.I
Mr. Tammy Liwag. We thanked him for being there always, we learned a lot from him.
The accomplishment we did was so fulfill. I have learned so much from them and I hope they
learned something from us too. Now I know that there are a lot of people that needs to be given more
attention for their needs not only financial but more on health services.

LEARNING FEEDBACK DIARY(Brgy. San Juan, Quezon, Isabela)
Objectives:1. To integrate with people, to listen to their needs and to love them.2. To render health services.3. To enhance my knowledge about the community people and to learn more about
their culture and traditions.
Community integration for me is the complicated thing in my profession as a student nurse,
because I am not confident with dealing with the people in a certain community. But I accept this
challenge not for the grade or anything but for the experience which will enhance may capability to be a
community nurse.And also to face my fear and to leran from it.
We arrived in Brgy. San Juan Quezon, we are welcome with a nice weather. My first impression
on the place, people in the barangay are difficult to understand and no cooperation. The first thing we
did is to consult to the Brgy officials and tell to them the purpose of our community immersion. And how
can we help them in solving some simple Health problem in their barangay. And the following days we
had our home visits and conducted some health teachings in some mothers about the importance of
breastfeeding and child’s immunization.
In the 5 days of community immersion that we had I conclude that its not easy to live in a
barangay that is far from the town proper. Because some of them cannot easily avail some health
services in the RHU. After that days I could say that in the future I want to be a better community health
nurse so I could help people who are really in need of proper health services.

LEARNING FEEDBACK DIARY(Brgy. San Juan, Quezon, Isabela)
Objectives:1. To be able to differentiate the various methods of family nurse contact.2. To be able to apply the essential principle in home visit.3. To be able to determine the family health status of the people in the community.
Last September 28 to October 2, 2009, we, Group 7, had done community immersion at
Brgy. San Juan, Quezon, Isabela. The experienced was great! “Incomparable.” It made me easier
to visualize and recognize situations regarding the community problems and their environment.
With regards to their health activities, our group conducted house-to-house visit. Such as we’ve
done imparted health teachings to those clients especially pregnant women, families who had a
problem of malnutrition and hypertension. We also interviewed some of their Barangay Officials
especially with their Committee on Health and some member or group of community people,
regarding of what are their expectations why we were there to stayed five days in their
community. And we have done one-on-one getting of vital signs in every Purok assigned to us,
we experienced also to assisted mother who delivered a baby, cutting nails for children and
distributed herbal soap, too.
Through there, I was once inspired by them, it’s just amazing that in a little way, I can
help those people whose at first strangers to me. But before all these things there are factors that
hinder our investigation and integration such as most of our time with the families we were find
it out they did not coordinate well because they are busy in the farm field and in their
livelihoods. Those it served me as a challenge. Despite of these things the people are still nice
and hospitable. And most of them are approachable and friendly which facilitates me at ease.
Sometimes we think that they are just an ordinary people who live their life simple and others are
hopeless. But honestly they are more than that. I am glad that you can see unity and strength of
the family helping and holding into one anothers hope. On this thing alone, I can say that they
are so blessed. I am just wishing that this will continue until the next generation come and I hope
that they will start here to improve more their living.
Studying at the four corners of school are not enough to enhance and practice more about
the necessary things needed especially on health status of our country. To experience live with
community people will be one of the great challenge to become a better professional nurse
someday. Brgy. San Juan is one of my memorable moment in my life, I felt my heart with full of
loving and caring to the community people. Things that I have learned from the school and
shared for the people is one of a great opportunity that I never forgotten.

Objectives:1. To be able to know the health status of the community people.2. To be able to implement nursing care plan to the adopted families.3. To establish rapport to the community people.
In our five days community immersion in Brgy, San Juan, Quezon, Isabela we have
learned a lot for ataying there. Our C.I. Mr. Tammy Maurice T. Liwag doesn’t come with us in
going house-to-house visit because he want us to become independent, it is nice because we
learned how to use our therapeutic skills in communication.
Here in Brgy. San Juan, you’ll meet different people, there’s a family that is
approachable to us but some are not, but we use our therapeutic skills in communication so that
we can interact with them.
In our second day, me and my three groupmates was assignes in Purok 7 or Purok
Cuntapay. The Purok Cuntapay was far in the center of barangay, it was more than 1.5 km going
there. We walk going there, it’s far but we enjoyed as we walk going to the said purok. As we
can see the Purok 7, we are sad to see their situation, poverty is the most problem, there’s no
electricity and the health services was not complete.
Although we did not complete the six (6) days for staying there because of typhoon

Objectives:1. To better understand and know the factors that affects their health status.2. To be able to develop our learning experience in the community area.3. To be able to apply our knowledge that we learned in school.4. To integrate with the community people in order to establish trust and rapport.
As we continue our learning experience in our chosen profession, we practice and apply
our knowledge not only in the hospital setting but in community too. Being exposed in the
community of Brgy. San Jan, Quezon, Isabela, we had given the opportunities to enhance our
own capability and ability to integrate in the lives of the people. Our exposure in this community
teaches me a lot of things which is very important in my chosen field.
It is necessary to assess first ourselves in dealing with different kinds of people before
assessing the lives of every individuals. Self awareness is very essential. I learned to recognize
the uniqueness of every persons, develop my own’s strengths and weaknesses. I realized that
community health work is a struggle based from the experiences and difficulties we encountered.
In this area where we exposed every minutes of the people’s time is very precious to them
because of their major sources of income is farming so for them it simply means a day without
work would simply means a day without food. I appreciated also the goodness of the people they
have shown to us, they gave us information without hesitation.
Learning and working experience serves as a mile stone that metamorphosized us into
beautiful characters. I had learned more integrating the people although in my own part I have a
problem in communicating with them because most of them are purely Ilocano in which one of
my weakness but then I made myself easily adapt and adjust. Inside the school it is hard for me
to appreciate what is really the communication means, but then I came face-to-face with realities
of life. All I knew was I read in the book and what I learned in the school but then I realized that
nothing was more real than being where the problem really was. The things that we have learned
from our instructors, books, and experiences may prove to be indespensable and it will prepare
us to face the competitive world where success does not come in terms of high grades but of
fulfillment, security and satisfaction.
I really enjoyed my stay in this area full of fun, excitement and learning experince and it
will forever treasure these memories of mine, it will serve as a guide as we go along with our
chosen profession and continue to journey the path toward achieving my dreams.

“Application of knowledge is wisdom,” this principle have been always in my mind to be
able for me to learn, develop my skills, and to understand more about nursing profession…
nursing is not only about earning money by serving rich people in cities and hospitals. It is also
reaching out to those who are in need especially in the remote areas in communities. That is why,
I do believe that rendering basic health services to the people are essential in the aspect of
nursing.
I had learned how to deal with the community people as a whole in which the principles
and steps in PHC and COPAR serves as my guidelines in doing so. I had gained the trust of the
people and their full participation as well, in which it is a great factor to achieve the ultimate
goal--- “self-reliance.” For how can we achieve a certain goal if there is no cooperation from the
group? To achieve it, I had motivated and empowered the community people that they have the
capacity to change and they are able to bring about change. I had learned also to focus on the
interests of the people and not my own interest. I had learned to prioritized their needs. Also to
work with them, and to let them think critically by recognizing the root cause of their problems
and how to solve it. This how I had studied and understand more that health is a multifactorial
phenomenon, in which it is affected with different factors.
I had realized how sick are we…in which it had motivated me to continuously help and serve all
those who are in need because it brings and give a feeling of great pride and achievement/ satisfaction
to one self when I see their ungrantifiable smiles and happiness…in one way or another I have imparted
one graet value and good change to make a certain community a better and healthy place to live in.

Objectives:
1. To learn and understand the tradition of different people in the community.2. To enhance my communication skills.3. To point out the characteristics of a healthy community.
The community health nurse provides care to clients who are members of families, to
aggregates of clients who form families, and to the familiey as a whole. Each family is unique in
ways of meeting the needs of its members and society. Therefore, the nurse must know his
family client, a dynamic and complex unit that adpts to each individuals changing roles and
functions.
Our community immersion was at San Juan, Quezon, Isabela. We stayed at the
community for six days but due to the typhoon Pepeng we went home earlier. At first, I am
nervous because it’s my first time in San Juan but I’m glad that because of this community
immersion I met a lot of friends. The people in this barangay was approachable and friendly.
And I learned to become independent because I’m far with my parents but I still have a family,
my Community Instructor and my group mates. Eventhough sometimes we don’t have
cooperation, some of us committed such mistakes, our bonding become stronger because we
treat each other not only as a groupmates but as one family and we build a good friendship from
that.
I realize a lot of things in staying in the community and I treasure all of those experiences
when we conducted house-t-o-house visit.

Objectives:1. To identify the role of the community health nurse in the community.2. To learn how to deal with the different traditions of the community.3. To improve my skills in interacting with the community people.
Community immersion is about dealing with the community people, home-visiting and
interviewing about their health status.
Living with the community people in Barangay San Juan, Quezon made me improved
more of my communication skills in interacting with the community people. I learned on how to
deal with them easily. Community people in San Juan were very hospitable and friendly, because
they gave us an overwhelming welcome in their community.
In five (5) days that I stayed there with my groupmates, I learned how to deal with them
one-by-one. We worked as a group for us to finish all of our requirements. Although sometimes
we lack cooperation.
I am happy because I developed my different skills in the community. O thank our very
nice Community Instructor, Mr. tammy Maurice Liwag for making us working independently
because we learned how to worked on our own. I hope that there would be more time for us to
help other people and share our knowledge to them for us to have a better health.

LEARNING FEEDBACK DIARY

On October 2009, we had our community immersion in San Juan, Quezon, Isabela. We are
scheduled to stay there for six (6) days. But due to unpleasant weather, we are sent home early.
San Juan is an isolated area, a place which I think civilization does not occur, and does not fully
exist. Far from everything which you can noticed the face of poorness of our unfortunate fellow Filipino.
On the first day, we are deployed in every purok to conduct interview with the people, asking
some questions to gather some important facts, datas and informations that could serve as the basis to
help them to be more aware not only about their health, but the whole health status of their
community.
While staying and interacting with this people, I had observed some qualities which the
community people showed and of course I really appreciate. They are approachable and hospitable
which showed us a generous welcome in visiting them.
The important thing that I’ve learned in the community is to deal and live with the people into
simplicity and humbleness.

Community integration was already a part of us students not only because it is a part of our
curriculum but also in reality, in our chosen profession. We were exposed to this kind of activity for us to
understand more what are the different factors affecting the health situation of the community people,
to develop our communication skills as we apply what we have learned in therapeutic communication as
we interact with the community people and for us to be familiar with the different roles of a
mommunity health worker/ nurse and for us to put into action the things that we have learned.
The things that we have learned in the four corners of our classroom is not enough, for us to
improve and broaden not only our knowledge but also our skills is that we must be exposed in the field
just like what we are doing in this so called, community immersion.
Barangay San Juan is one of the remote areas in Quezon wherein most of the community people
here are can not afford to avail medical care. And with our house-to-house visit, I discovered that some
of the sickness of the community people are related to their problem lack of financial matter.
Living with this kind of community is not that easy. But through our willingness to learn and
adapt with their living and lifestyle helps a lot for us not to experience any difficulties living with the
community people. At first, I have to adjust a lot of walking, wading through the muddy paths, in going
to houses of the community people, but I had no difficulty in adjusting. I don’t m ind the small sacrifices,
as long as I am able to help the people, as long as I can serve them, I am already fulfilled. But we should
also limit what we can render to them for them not to become dependent to us. We should impart to
them what we knew on how to prevent and also how to treat the diseases.
When dealing with the health care of the community people, we also have to deal with their
economy, their source of livelihood, their education, their housing, family and many more. And from
then on, I realized how really sick we are especially those who are in the authority, not only on our
country but with the other countries as well, war instead of housing, roads and bridges, privatization,
deliberation instead of health services. At the haelth center, the services are lessened because the
budget for health is lessened.
I also realized that I learned more from them than they from me as we go on during our
collaboration. As a community health worker, the aspiration of the community people becomes your
aspirations.
