Introduction : Complete denture is an structure fabricated by laboratorian for usage of the patients which does not have main aspects of ideal oral cavity such as 1- functional inability of jaws : lack of power in bite or missing teeth which are in charge of bite and even pronouncing words .2- aesthetic lack : patients who are in lack of aesthetic aspect of smile , jaw and whole face .

Step by step : Examination of patientTaking the primary impressionMaking a custom tray for the patient Border moldingBoxing the final impressions Making the record bases Wax occlusion rims setting upChecking plane of occlusionUsage of facebow apparatusRecording centric relationArrangement of teethTry in procedureFinal step polishing and finishing .

Primary impression :Seat the patient in a comfortable positionSelect a tray which can cover all the landmarks of the jawsMix the alginate with water ( or any other universal impression material )Load it to the impression tray Seat the tray posteriorly then push to the front to avoid any over loading of material .Tray handle should be in one line with the medial line of the faceAfter semi-solidation of impression material remove the trayInspect the impression and check if it is acceptable .

Different impression trays :

Making a custom tray :Receive the impression model .Mark the vestibular depth minus 2 mm with a pencil(for marking the trimming area)Block and fill all of the undercuts in the cast with baseplate wax by melting and applying on the cast slowly .Apply Vaseline or likewise material to serve as separating layer .Place a sheet of custom tray and adjust it from the depth to the top to avoid air while adjusting the custom tray material .Trim on the pencil marker that we marked before Make the borders of the tray as smooth as possible Cure the material in trial unit for 2minutes Remove the cast and put the custom Tray again in the unit for complete curing.

Border molding : 1.Soften the molding compound by flaming it and apply to the tray along the anterior border2.Place it to the patient mouth and smoothly massage it upward downward and circular form3.Result of this step : the molding compound will look alike the anterior part of the jaw , but smooth and without any edges .4.Apply this steps to the other parts of the custom tray and complete the molding .5.Paint the tray with a adhesive to avoid stucking the impression material to tray.6.Put impression material in the tray and insert the the dry oral cavity.7.Remove the tray after setting of the impression material after 4-5min.8.Inspect the impression if it is acceptable or not .

Boxing the final impression :Place a piece of playDough on a flat surface and make it a round shape.

Place the final impression into the playDough and smooth the edges with wet finger.

Wrap a boxing wax around the playDough and seal it with hot wax spatula

Flow the fabrication material into the boxing wax and make it flow to all Parts of the final impression

5.Wait for the curing process to occur and you will have the Master CAST .

record bases :1.Apply vasine once again on the Master cast.

2. Apply the base plate material to the master cast and avoid any bubbles.

3.Trim the extra base plate from the master cast .

4.Cure the record base in triad curing unit for 4 mins .

5.Remove the record base from the master cast and cure it for 8 mins once again for complete curing .

wax occlusion rims :1.Apply sticky wax all over the record base which is mounted on the master cast.2.Take a base plate heat it and fold it until the plate is finished .

3.Form a horseshoe shape and apply it on the record base .

4.Heat a mixing metal spatula and form the occlusal plane.

5.Cut the extra base plate .

Checking the plane of occlusion:1.Placing the wax rimmed record base in the oral cavity .2.Approximately 1-2 mm of the wax rim should be visible .

3.Determine the relationship of the wax rim to the interpupilary line using the plane indicators.4.Determine the relationship of the wax rim to the ala-tragus plane .5.Adjust the rim to be parallel to the interpupilary plane and alaTragus plane by trimming or adding wax rim .6.Mark the medial line of the face to represent inter-incisal line .

7.Mark the high lip line perpendicular to the medial line . This represents when the patient smiles.

8.Last step is to mark the right and left canine at resting position.

Face-bow:Make 2 V shape grooves on the wax rim one in premolar area and one in molar area .Repeat on the other side of the wax rim .(2 grooves on right 2 grooves on left )Place the record base in the patients mouth .Place small amount of bite registration material on the face bow fork .Place it in the patients mouth and wait a few moment.Assemble the facebow apparatus and make it ready .Do following step showing in the video for adjustmentof face bow apparatus .8.After adjustment of facebow , simply unattach the deviceAnd have your base record mounted on facebow fork .

Face-bow is a dental instrument that is used in the field of prosthodontics .Specifically, it transfers the relationship of maxillary arch and temporomandibular joint to the casts.

Recording the centric relation :Take the mandibular record base and trim 1mm of wax posteriorly from both sides to the canine line that we marked while the record base was in patients mouth .

2.Put bite registration wax on the mandibular record base where we trimmed from themolars until cover the premolars .

3.Place the recording bases to the oral cavity and guilde the patient to bite on the Centric relation position .

4.Now remove the recording bases togather , and you will have Recorded centric relation on the casts .

Always remember , there should be no contact between the acrylic bases ,Wax rims or the master casts .

Arrangement of teeth :This is work of dental technician and its not for the dentists but , I show a fast aspect of how this job is done cause some may not know . 1.Taking the artificial teeth and placing in on the acrylic base record and marking the horizontal and vertical endings . arrangement of teeth acrylic base.2.Mark the place of the tooth with a sharp wax carver .

3.Use a sharp wax carver to cut place of tooth setting .

4.Heat the carver and melt the socket , then place the Tooth inside the socket and press and hold until the melted Wax became solid and kept the tooth .

5.Use a flat surface to check occlusal plane.

6.Use articulator to measure different aspects of teethMeasures and see if any mistakes happened.

After this , repeat the procedure for other teeth.

Checking (try in) the full fabricated denture :1.Inspecting the shape , size , arrangement of teeth on the articulator .

2.Inserting the try in base into the patients oral cavity and check the medial line.

3.Check the occlusal plane and stability of denture in oral cavity.4.Check vertical dimension and occlusion with both dentures on5.Check the functional and esthetic factors .6.Phonetic factors should be checked by asking the patientTo pronounce F V S N T H sounds .

In case of patients complain on the denture , we check the occlusion again by occlusion papers and grind the black marked points off the denture .
