INTRODUCCIÓN
• Alta incidencia de hipotensión arterial tras stenting
carotídeo (7-34%, y hasta 76%)1,2.
• La inestabilidad hemodinámica per y post-revascularización
carotídea se asocia a aumento de complicaciones
cardiovasculares y neurológicas 3.

para la hipotensión arterial post-SC.
¿ Es posible la realización del SC transcervical en Cirugía Mayor Ambulatoria prolongada (12h) ?
CASOS SELECCIONADOS
NO episodios sintomáticos agudos.
NO complicaciones per / postoperatorias graves.
Vía transcervical con flujo reverso.
Comorbilidades.
¡¡¡ HIPOTESIS !!!
OBJETIVOS
Estudio descriptivo retrospectivo.
97 pacientes
Stent carotídeo vía transcervical con flujo reverso
entre 1 de enero 2008 al 31 de diciembre de 2009.
Criterios de exclusión:
- Vía transfemoral.
- Reconversión a Cirugía abierta.
- Indicación por restenosis de TEA / stent previo ipsilateral.

HIPOTENSIÓN ARTERIAL POST-OPERATORIA
TAS<90mmHg ≥5min. y/o drogas vasoactivas
3 GRUPOS DE ESTUDIO:
1. Hipotensión precoz (1-6h. Post-SC)
2. Hipotensión tardía (6-12h. post-SC)
3. Normotensión.

SELECCIÓN INDIVIDUALIZADA de pacientes para
plantearse su realización en CIRUGIA MAYOR
AMBULATORIA PROLONGADA (12h).
○ Control estricto las primeras 6h. / Control hospitalario entre 6-12h.
○ Asintomáticos.
○ Apoyo familiar / social.
74.22% PACIENTES ¿CANDIDATOS?
COMENTARIOS (I)
Tras 12 horas 72 pacientes SIN complicaciones (60 Normotensos / 12 Hipotensión tardía)
Sería necesario desarrollar circuito organizado
para correcta ASISTENCIA DOMICILIARIA.
○ Hospitalización domiciliaria ( control 12 y 24 horas).
○ Telemedicina.
○ Educación sanitaria.
○ Visita hospitalaria y eco-döppler de TSA a las 24-48h.
