Comprehensive Exam Committee Appointment Form

The Associate Dean for Research and Graduate Studies, in consultation with the graduate supervisor appoints the Chair of the COMPREHENSIVE EXAMINATION COMMITTEE. The supervisor in collaboration with the student nominated the remaining members of the committee. The students should contact each prospective committee member, and get signatures on this form if the committee member is willing to serve. Please return the form to the Office of the Associate Dean of Research and Graduate Studies in your college for placement in the academic file. A copy needs to be filed with the Office of Graduate Studies.
