Protocol1. If an Athlete takes a shot to the head take

them out of the game for 15 minutes.2. Do an initial assessment, any LOC EMS

must be activated to evaluate the athlete.3. After a FULL 15 minutes, use the Check-

off list to determine return to play. If they have any signs or symptoms during second check off list, DO NOT CONTINUE!!!!

4. If they do not have any signs and symptoms after 15 minutes, continue onto function tests.

Head Injury Check-off List

Must pass all tests in order to return to play.

Use Functional tests to confirm your physical

examination. You don’t need to do all of them but enough to confirm to you

that they are okay.

Remember this even if they say they just blacked

out. And blacking out is LOC, EMS needs to be

called.

If you suspect a concussion, this form is stapled to the

check-off form and should be given it to a person who will be with the athlete for the next 24

hours.
