Surgical oncology involves a logical progression of answering the questions “what is it, where is it and how bad is it?” to come up with an optimal treatment plan for surgical oncology patients. There are instances when a fine needle aspirate, incisional biopsy or excisional biopsy are most appropriate. Staging may involve lymph node aspirates or biopsy, thoracic radiographs, abdominal ultrasound or a CT scan — depending on the definitive or presumptive diagnosis. Each type of surgery for cancer can be thought of as a dose: intralesional, marginal, wide and radical. Each is appropriate in different circumstances, and wide or radical excisions require aggressive pain control. Multi-modal pain management strategies will be discussed.

Dr. Kerrie Lewis will cover the current literature on sedation, partial intravenous anesthesia (PIVA), total intravenous anesthesia (TIVA) and local blocks that can be useful tools in developing well-rounded analgesia and anesthesia protocols in our food animal species.

This two-hour lecture will focus on the gastrointestinal system from the esophagus to the colon, and it will include the secondary systems – biliary, pancreatic and lymph nodes. We will discuss techniques to optimize visualization of the gastrointestinal tract (GIT) for evaluation of conditions such as hiatal hernia, esophageal reflux, gastric infiltrates and ulcers, pyloric and intestinal obstruction, and common bile duct insertion and obstruction. Clinical cases will be demonstrated for these conditions as well as intestinal infiltrates, inflammatory disease including inflammatory bowel disease (IBD), lymphangiectasia, feline infectious peritonitis (FIP) and vascular/infarctive disease. Localization and identification of associated lymph nodes will also be discussed as well as the differences between canine and feline examinations.

Oral tumours are often picked up on dental prophylaxis — a great opportunity to do a thorough oral examination. If an oral mass is discovered during a dental exam, a systematic approach to management is recommended. This will include documenting the mass in the dental chart and with digital photography (plus or minus radiography). If possible, three-view thoracic radiographs should be taken during this visit as well as lymph node aspirates. An incisional biopsy is recommended to obtain a histopathological diagnosis. It is very important not to disrupt the mass. Because the oral cavity heals so quickly, it can be difficult to see where the mass was if it is shaved off the gingiva. For malignant oral tumours, mandibulecotmy or maxillectomy are recommended as part of surgical management. These procedures are well tolerated in dogs and even cats.

With the ever-increasing focus on animal welfare, strategies to minimize pain and discomfort in food-producing animals are critical. Recently, there has been renewed interest and significant activity in the use of non-steroidal anti-inflammatory drugs (NSAIDs) in food animals. Veterinarians are reassessing how they use older NSAIDs due to novel formulations or routes of administration. New indications and food animal species are also being approved for these drugs. Many veterinarians routinely use NSAIDs for a variety of extra-label indications. However, despite many anecdotal reports of NSAID use in food animals, recent well-designed clinical trials have produced mixed results in terms of efficacy. This session will highlight recent studies evaluating NSAID use for a variety of food animal species and indications.

Learning Objectives• Provide veterinarians with current evidence for

(or against) NSAID use in a manner that can be easily shared with clients when discussing pain and analgesia options for their animals.

We call these “shoppers.” In this seminar you will learn how using a simple technique, you’ll be able to perfectly handle these ”shopper” clients and get them to choose your practice — turning them into clients for life!

There are two types of veterinary hospitals – busy ones and barely surviving ones. What do they both have in common? Both need a commitment from all staff to create a culture and environment that focuses on the pet’s emotional well-being to the same extent that it focuses on the pet’s physical well-being. Both need to address their patients’ fear, anxiety and stress in order to provide optimal healthcare, and in order to provide the best experience imaginable for pets, pet owners and veterinary staff. Learn how hospitals can step up their approach by addressing the fear, anxiety and stress issue head on by exploring strategies from the trenches of general practice. Using video examples from my practice, we will take a walk through the typical outpatient Fear Free℠ exam, and we will solve the most common challenges of the outpatient visit. It will change the way you practice.

FRIDAY, SEPTEMBER 28Conference Session Descriptions

The odds of feline injection site sarcomas (FISS) occurring in cats that receive injections is between one in 1,000 and one in 10,000. Vaccinations are often a cause of FISS because this is the most common injection given to cats and because many vaccines cause a local inflammatory reaction. In cats with FISS, the inflammatory process transforms and becomes neoplastic. FISS is an extremely aggressive tumour in cats, with local control being the biggest challenge. It is critical to recognize these tumours early and to do an incisional biopsy to determine the tumour type. An excisional biopsy should be avoided as it can make definitive surgery less effective and results in a much worse outcome. The current recommendation for treatment is to remove these masses with radical excision, which means five-centimetre radial margins and two fascial planes deep. Long-term success can be expected in most cases of FISS that receive radical excision and have clean margins. However, the metastatic rate is 25 per cent and this surgical approach is extremely aggressive. In patients where surgical excision is not feasible with such wide margins, radiation alone or surgery combined with radiation can be considered. Changes in vaccine site protocols are a critical piece of the treatment of FISS; vaccinating distally on the limb or on the tail will result in many more treatment options for these patients.

This two-hour lecture will focus on the gastrointestinal system from the esophagus to the colon, and it will include the secondary systems – biliary, pancreatic and lymph nodes. We will discuss techniques to optimize visualization of the gastrointestinal tract (GIT) for evaluation of conditions such as hiatal hernia, esophageal reflux, gastric infiltrates and ulcers, pyloric and intestinal obstruction, and common bile duct insertion and obstruction. Pruritus is the most common presenting complaint in dogs, accounting for 30 to 40 per cent of skin-related visits to small animal veterinarians. Many are diagnosed with allergies. We have come a long way since we called this disease “AIDS” (allergic inhalant dermatitis). The liver is a central component in the vast majority of the body’s biochemical processes, immune surveillance, drug metabolism, and biliary function. It is not surprising that the liver is susceptible to metabolic derangement, infectious and inflammatory agents, biliary dysfunction, hypoperfusion, and hypoxia, as well as being sensitive to maladies throughout the rest of the patient. Attempting to efficiently and effectively identify specific liver diseases can, therefore, be quite challenging. This presentation will briefly examine time-tested strategies and new possibilities in the diagnostic work-up of liver disease in dogs and cats.

Some liver conditions (e.g. single, extra-hepatic portosystemic shunt) can be successfully treated with little more than a chewing gum wrapper (i.e. cellophane band), while other liver diseases require a complex combination of anti-inflammatory, immunomodulatory, choleretic, chelating, and antioxidant medications. This presentation will briefly review standard treatments, introduce newer treatment strategies, and examine the theory behind the practical use of various “liver-friendly” nutraceuticals.

Dr. Craig Webb is a Diplomat of the American College of Veterinary Internal Medicine, and Professor and Head of the Small Animal Medicine Section at Colorado State University. He received a PhD in Neuroscience from Hahnemann Medical School and then completed his Doctor of Veterinary Medicine degree at the University of Wisconsin. Dr. Webb specializes in gastroenterology and hepatology and teaches courses in both GI Endoscopy and Diagnostic Laparoscopy with Drs. David Twedt and Eric Monnet. His areas of research include the role of oxidative stress in small animal diseases, and he is currently working on the use of adipose-derived stem cells for the treatment of IBD in cats. There is a growing body of evidence indicating that horses, much like cats and dogs before them, are suffering from widespread obesity. For predisposed breeds and types of horses, being obese may put them at further risk of developing equine metabolic syndrome (EMS). This session will review the most recent theories on the pathophysiology of EMS as well as current diagnostic approaches and treatment/management recommendations. Current research investigating novel biomarkers for the diagnosis of EMS will also be discussed.

These two sessions will review nutritional basics that are important when making dietary recommendations, such as daily requirements for different classes of horses, voluntary feed intake and balancing for common deficiencies or excesses. These sessions will also highlight Dr. Robinson’s approach to taking a nutritional history and the interpretation of hay analyses. We will also review the feeding behaviour of horses to help attendees make best practice recommendations for their clients.
