SCOPE

This Standard applies to all Karara Iron Ore Project (KIOP) participants. The Contractor is responsible for the development and implementation of an approved Project specific Confined Space Entry Procedure which shall satisfy the requirement outlined in this Standard.

This Standard applies but is not limited to the following confined spaces during Site activities, including construction, commissioning and maintenance:

• Storage tanks, process vessels, boilers, pressure vessels, silos and other tank-like compartments, internal structural compartments.

• Pipes, sewers, shafts, ducts, trenches, excavations and similar.

This Standard should be considered for use in other confined spaces applicable to the Contractor's scope of work, including mobile tanks and confined spaces as found on ships.

This Standard does not apply to mines or tunnels (such as road tunnels). Requirements for excavations may include a Confined Space Entry Permit Refer to the KML Excavation Standard.

Refer also to the supporting documents listed at the end of this Standard.

3. DEFINITION

Authorised Person A person who has been duly trained and examined as to a level of knowledge of and competence in confined space entry and works. This person shall be authorised in writing by the KML Registered Manager to issue, coordinate and control Confined Space Entry Permits issued in defined areas of operations, and accepts the responsibilities associated with authorisation.

(Permit Issuer)

Competent Person A person who has, through a combination of training, education and experience, acquired knowledge and skills enabling that person to correctly perform a specified task.

For the purpose of confined space entry works this person has been authorised and trained to undertake atmosphere and/or gas testing. An enclosed or partially enclosed space that is at atmospheric pressure during occupancy and is not intended or designed primarily as a place of work, and—

Confined Space

a) is liable at any time to—

i) Have an atmosphere which contains potentially harmful levels of contaminant;

ii) Have an oxygen deficiency or excess; or

iii) Cause engulfment; and

b) Could have restricted means for entry and exit.

As defined in AS/NZS 2865:2001: Safe Working in a Confined Space.

Confined Space Entry (CSE) Permit

A signed statement by the Permit Holder, Authorised Person and the person in charge, that a job may be carried out under stated precautions in a specified area.

Contaminant Any dust, fume, mist, vapour, biological matter, gas or other substance in liquid or solid form, the presence of which may be harmful to health and safety.

As defined in AS/NZS 2865:2001: Safe Working in a Confined Space.

For the purpose of these procedures, entry is when a person's head, i.e. the breathing zone, or upper body is within the boundary of the confined space.

NOTE: Inserting an arm for the purpose of atmospheric testing is not considered an entry to a confined space.

As defined in AS/NZS 2865:2001: Safe Working in a Confined Space.

Entry

The range of flammable vapour or gas-air mixture between the lower explosive limit (LEL) and the upper explosive limit (UEL).

Explosive (flammable) Range

• Lower explosive limit (LEL)

- In relation to a flammable contaminant, the concentration of the contaminant in air below which the propagation of a flame does not occur on contact with an ignition source.

• Upper explosive limit (UEL)

- In relation to a flammable contaminant, the concentration of the contaminant in air above which the propagation of a flame does not occur on contact with an ignition source.

As defined in AS/NZS 2865:2001: Safe Working in a Confined Space.

A competent person who has been delegated to conduct or lead the work.

Permit Holder

The person who is responsible for carrying the Permit at the worksite.

Permit and all supporting documentation i.e. Risk Assessment, JHA, Take 5, Drawings, other permits etc.

Permit Package

The Karara Iron Ore Project (KML). Project

To be considered as part of the documented, local risk Recommended assessment process. Self-contained breathing apparatus SCBA A mandatory requirement. Exceptions to this requirement need the endorsement of the Project OH&S Manager and the approval of the Project Manager (or his nominee).

Should The word "should" indicates that the primary intention is to comply with the full requirements as if they were mandatory. However, there will be circumstances where local conditions may demonstrate that the requirement is either not applicable or an alternative approach is necessary. In cases where "should" has been used in a requirement, variation can only be considered as compliance if the most Senior Manager on Site approves it based on an evaluation of the risk. The risk management approach adopted must be consistent with Project requirements and follow the Hierarchy of Controls.

Stand-by person

(emergency response initiator)

A competent person assigned to remain on the outside of, and in close proximity to, the confined space and capable of being in continuous communication with and, if practical, to observe those inside. In addition, where necessary, initiate emergency response procedures and operate and monitor equipment used to ensure safety during entry and work in the confined space.

As defined in AS/NZS 2865:2001: Safe Working in a Confined Space.

Threshold Limit Values The airborne concentrations of contaminants which represent conditions under which it is believed that workers may be exposed to without adverse effect. For the purpose of these procedures TLV's published by WorkSafe Australia will be applied.

RESPONSIBILITIES

Contractor managers are responsible for the application of, and compliance with, this Standard at work locations where they have operational responsibilities. KML will monitor and conduct audits to ensure the effectiveness of the Contractor's compliance with this Standard.

Contractors are responsible for the development and implementation of an approved Project specific confined entry procedure which satisfies the requirements of this Standard. Contractors are responsible for their personnel and their subcontractor's personnel compliance with this Standard and/or other standard as agreed per Section 2 of this document.

All Supervisors are responsible for the day to day application of this Standard.

All KML employees and contractors have a responsibility to comply with this Standard.

Personnel have a duty of care, to their work team and others in relation to the application and maintenance of this standard, for work they, their work team or others undertake.

REQUIREMENTS

General 5.1

Identi f icat ion and Registrat ion of confined spaces 5.1.1

Contractors shall have a process in place to define and identify confined spaces.

A register of confined spaces shall be maintained and regularly updated by the Contractor for the Contractor's work area.

Notices shall be placed at each confined space identified on the register, stating that a permit is required for entry. This shall also apply to temporary confined spaces, such as trenches, as well as a permanent confined space. For permanent confined space, the notice shall be placed on the entry to the Confined Space.

5.1.2 Permit

Contractors shall ensure that confined space entry permits issued are for entry into a confined space only. The Contractor's confined space entry procedure shall include but is not limited to the following requirements;

• All work in a confined space is subject to an additional safety risk assessment including but not limited to the overarching KML Permit to Work Standard.

• A competent, authorised person (Permit Authority) shall issue permits.

• A register of authorised and trained persons shall be maintained for working confined space. See Appendix 1 for sample.

• The person issuing the permit shall personally inspect the work site, prior to issue of the permit to ensure that all precautions required prior to commencement of work are in place.

• Prior to the start of the work, the Permit Holder shall review the Confined Space Entry Permit with all involved personnel and shall ensure that all precautions listed on the Confined Space Entry Permit are adhered to.

• The Confined Space Entry Permit shall be posted prominently and maintained at the worksite for the duration of the tasks. When the tasks are completed and the final check is conducted, the Permit Holder shall return the permit to the Permit Issuer.

5.2 Assessment of hazards

Before a person may enter a confined space, safety precautions shall be undertaken. The extent of these precautions will depend upon a Confined Space Risk Assessment. Examples of potential hazards are provided in section 6.1. Guidance on risk assessment may be found in AS/NZS 3931 and AS/NZS 4360.

When entry is to be made to a confined space, consideration must be given to the safety of persons working in such a location. Not only for adequate atmosphere and ventilation but other aspects such as fire risk, ease of access, egress and emergency access must be catered for.

The completed Confined Space Risk Assessment shall be attached to the Confined Space Entry Permit and shall be retained by the office of issue for a period of 7 years from the time of their validity on completion of the work (or longer if mandated by local Regulation).

5.3 Training

Training in the critical aspects of confined space work shall be completed prior to mobilisation and the commencement of any confined space work. Contractors shall ensure their own personnel and sub­contractors are trained (nationally accredited) and competent in confined space entry. Contractors are also responsible for the provision of this training.

Training in confined space work is a specialist function and requires a trainer with proven expertise and experience. The suitability of training courses should be verified with the KML Training Superintendent or KML Safety Manager.

All training programs will include at least the following;

a) The requirements of State OSH legislation and AS/NZS2865 "Safe Working in a Confined Space".

b) Hazard recognition.

c) Assessment procedures.

d) Control measures.

e) Breathing equipment including types, selection and practical use.

f) The selection, use and maintenance of safety equipment.

g) Atmospheric monitoring.

h) Ventilation.

i) Isolation/Lock-out methods (to Project standards),

j) Senior First-aid.

k) The use of rescue equipment (entry/exit devices).

I) Fire prevention.

At the completion of personnel training the Contractor shall provide evidence of competency (i.e. National Standard in the form of a certificate issued by the confined space trainer) to the KML Training Superintendent. Contractors shall ensure that their personnel be reassessed at appropriate intervals to ensure their ongoing competency to perform activities relevant to their entry and work associated with confined spaces.

5.4 Requirements of permit

The permit shall detail:

c) The personnel who will carry out the work on the Job Hazard Analysis (JHA).

d) The tasks to be executed.

e) The overarching permit (hot or cold).

f) Other tasks requiring additional permits.

g) Tools that may be used.

h) Where the tasks are to be carried out.

i) The hazards considered.

j) The precautions to be taken.

k) The duration for which the permit is valid.

I) Stand by person.

Permits shall automatically expire in the event of;

• A change in the working environment that might impact on the permit to work conditions.

• An emergency including any activation of the Site / work area emergency alarm system.

If work tasks are to continue (due to a shift change for example), the current and active Confined Space Permit may be used, however the JHA and testing shall be redone.

5.5 Precautions

Work in a confined space shall be carried out in safe atmospheric conditions - less than 5% of the lower explosive limit (LEL) for flammable substances and less than threshold limit values (TLVs) for toxic gases, and oxygen content between 19.5% and 23.5%.

The person issuing the permit shall detail all precautions that shall be undertaken in executing the work tasks. These include:

a) Results of gas tests show less than 5% of the LEL flammable and less than TLVs for toxic gases, and oxygen content is between 19.5 and 23.5%.

b) The confined space has been cleaned and gas free and is well ventilated. Gas freeing shall require positive isolation of the equipment.

c) Rescue equipment including safety belts, harnesses and lines are available.

d) A rescue plan is in place and has been reviewed by rescue team members.

e) All dangerous moving parts have been locked out (De-energised).

f) All sources of electrical and fluid power have been locked out (De-energised). All electrical equipment which is to be used in a confined space and is connected to an external supply should comply with AS/NZS 3100 and, where required, installed in accordance with AS/NZS 3000.

g) A warning sign notifying 'Confined Space Entry in Progress' has been placed adjacent to the entry point and shall indicate that only authorised entry is permitted and comply with AS 1319.

h) Protective barriers (i.e. bunting, safety barrier mesh) erected.

i) Atmospheric temperatures inside the confined space are below 50° C or above 5° C.

j) Self-contained breathing apparatus requirements are established.

k) Fall arrest devices are checked and available.

I) Presence of Standby personnel,

m) Requirements for communications capability,

n) Purging, flushing and cleaning of vessels and piping,

o) Notification of other areas.

p) Restriction of activity in neighboring areas (Simultaneous Operations),

q) Requirements for ladders and scaffolding,

r) Requirements for lighting.

Additional safety checks may apply according to the specific work that is being conducted inside the confined space and the safety precautions specified by the Confined Space Risk Assessment.

Any exceptions to the above shall be submitted to the Registered Manager for approval.

5.6 Execution of work

Only personnel trained in Confined Space Entry shall enter Confined Spaces. Training records shall be kept for the term of the person's employment. (See Section 5.2).

Evidence of current Confined Space Entry / Standby Person training shall be sighted by the Permit Authority before authorisation of that person to enter a confined space or act as a Standby Person is approved.

The Confined Space Entry Permit shall be displayed at the work activity entry location until the work is completed.

All confined space entry shall be in compliance to the AS/NZS 2865:2001: Safe Working in a Confined Space.

On completion of the work tasks, including clean-up as required, the permit shall be returned to the Permit Issuer who is responsible for assuring that all close-out requirements have been met.

5.7 Standby Person

Personnel undertaking the duties of a confined space Standby Person shall be able to;

a) Understand the Permit to Work system as it applies to confined space entry.

b) Communicate via oral English (and / or any language(s) with those executing the work AND those who would have to provide emergency response).

c) Operate a portable radio, and signal lines.

d) Conduct relevant First Aid.

e) Participate in emergency and rescue procedures.

f) Use self contained breathing apparatus.

g) Operate gas detection equipment.

h) Understand isolation requirements, if relevant.

i) Use of equipment used for Confined Space entries, i.e. airline breathing apparatus, ventilation fans, etc.

5.8 Confined Space Entry Log

The Contractor shall ensure that the Standby Person records all movements of personnel into and out of the confined space on a Confined Space Entry / Exit Log (see Appendix 2 for sample). The name of each person entering the confined space shall be listed on the work permit as a Permit Worker.

5.9 Safety Equipment

All safety equipment for working in confined spaces shall be tested at regular intervals to ensure it is fit for purpose and shall comply with Appendix J of AS/NZS 2865:2001. Particular attention shall be paid to:

• Gas-testing equipment.

• Lifting and fall-arrest devices. Suitable safety harnesses and safety lines or rescue lines, complying with AS/NZS 1891.1, should be worn where:

- There is a risk of falling during ascent or descent.

- Rescue by a direct route, either vertical or horizontal, is possible.

The selection of the type of safety harness or safety line or rescue line should be in accordance with AS/NZS 1891.4 and should take account of the possible hazards/evacuation arrangements. The equipment should be stored, maintained and inspected in accordance with AS/NZS 1891.4.

• Self-contained breathing apparatus' (SCBA's). Respiratory protective devices should be selected, fitted, used, stored, maintained and inspected in accordance with AS/NZS 1715 and AS/NZS 1716.

5.10 Additional Requirements

5.10.1 Atmospheric Testing

Atmospheric monitoring and testing shall comply with Appendix I of AS/NZS 2865:2001.

Atmospheric testing is required for two distinct purposes: evaluation of the hazards of a confined space and verification that entry conditions are acceptable.

Atmospheric testing and monitoring shall be carried out consistent with the hazards identified and the risk assessment.

Atmospheric testing should include testing by scientific means for:

a) Oxygen content.

b) Airborne concentration of combustible contaminants.

c) Airborne concentration of potentially harmful contaminants.

The evaluation of the atmosphere and a survey of other hazards shall be performed from outside the confined space, before any entry occurs. No person shall enter a confined space to conduct atmospheric testing or monitoring without a written authority from the Contractor's Project Manager.

Monitoring shall be carried out by a competent person continually when confined spaces are occupied, documented on Confined Space Atmospheric Testing Sheet (See Appendix 3 for sample) and shall be included in the Permit Pack. Monitoring equipment shall comply with AS/NZS 2381.1.

Ensure compliance with the following;

• NOHSC: 1003(1995): NATIONAL OCCUPATIONAL HEALTH AND SAFETY COMMISSION. Adopted National Exposure Standards for Atmospheric Contaminants in the Occupational Environment.

• NOHSC: 3008(1995): NATIONAL OCCUPATIONAL HEALTH AND SAFETY COMMISSION-Guidance Note on the Interpretation of Exposure Standards for Atmospheric Contaminants in the Occupational Environment.

• WorkSafe Australia-Workplace Exposure Standard.

5.11 Emergency Response

In the Contractor's confined space entry procedure the Contractor shall ensure that appropriate emergency response and first aid procedures and provisions shall be planned, established and rehearsed.

In cases of emergency response, those persons involved in the response shall be made aware of the conditions in the confined space prior to any entry.

ADDITIONAL GUIDANCE 6.

Risk Factors Found In Confined Spaces 6.1

The list below is not exhaustive, and local circumstances shall be considered. Risk Factors may include:

a) Geographic remoteness from support services.

b) Arrangements for rescue, first aid, and resuscitation.

c) Number of persons occupying the space.

d) Language skills of person/s planned to occupy the space

e) Number of Standby Person(s) required.

f) Proposed work activities to be undertaken within the confined space.

g) Soundness and security of the confined space structure.

h) Identity and nature of the substance/s previously occupying the Confined Space.

i) Confined Space preparation work requirements.

j) Work in adjacent areas outside the confined space.

k) Requirements for cleaning of the Confined Space prior to and during entry.

I) Mobile equipment location and use.

m) Possibility of water inundation/flooding of confined space.

n) Atmospheres with insufficient oxygen, leading to asphyxiation.

o) Atmospheres containing harmful components, causing; fires, poisoning, intoxication.

p) Solids or liquids with harmful properties.

q) Collapse of excavation walls.

r) Collapse of piles of bulk materials, causing engulfment.

s) Bulk materials that do not support the weight of workers, leading to engulfment.

t) Extreme heat or cold,

u) Moving parts.

v) Restricted entry and exit opportunities

In addition, the hazards of rescuing a person need to be addressed.

6.2 Initial Cleaning and Purging

Where practicable all solids and liquids, which are liable to present a hazard to persons inside the confined space, shall be removed from the confined space prior to the entry of personnel.

Where necessary the confined space shall be cleared of contaminants by use of a suitable purging agent

Care should be taken to ensure cleaning agents do not create additional hazards.

When flammable contaminants are to be purged, purging and ventilation equipment designed for use in hazardous places (AS2430 "Classification of Hazardous Areas") shall be employed and precautions taken to eliminate sources of ignition.

All contaminants should be exhausted so that they do not create a further hazard external to the confined space.
