IntroductionErisma®-LP Evolution is a posterior pedicle screw system designed for stabilizing the thoracic and lumbar spine.
The design team turned its attention to creating a top loading system that combines reduced implant volume with ergonomically designed instruments. Erisma®-LP Evolution has been developed to meet the needs of surgeons and their teams with regard to reliability, safety and ease of use in the course of surgical procedures. Erisma®-LP Evolution offers a comprehensive range intended for the treatment of thoracolumbar disorders. Erisma®-LP Evolution can be combined with Erisma®-Reduction Screw.

In accordance with safety procedures in force, the patient is placed in the ventral position on the operating table. The surgical approach is based on the usual techniques used by the surgeon.

The pedicle path is carried out using the curved lumbar probe. The markings situated every 10 mm along the probe indicate the pedicle depth and therefore make possible the selection of the appropriate screw length.
The intactness of the pedicle canal is verified by probing the internal walls using the pedicle sounder.
Erisma®-LP Evolution screws are self-tapping and do not require use of a tap. Nevertheless, depending on the surgeon’s usual practice, optional tapping can be performed. The colour code identifies the diameter of the tap for the screw to be inserted.
After exposing the spine, the pedicle entry point is located and prepared by perforating the cortical bone with the bone awl.

If necessary, the french bender is used to fit the desired rod contour. The bender of the Erisma®-LP Evolution system offers several bending ranges.
If the screw is over-inserted or insufficiently inserted, its depth can be modified by using the T20 wrench.
Radiological control is carried out to verify the position of the screws in the pedicles.
The screwdriver 18710001 is used for the monoaxial screws.The screwdriver 18710024 is used for the polyaxial screws.
The screw of appropriate diameter and length is selected from the implant set.

The Erisma®-LP Evolution system offers a range of pre-bent rods (30mm to 100mm). Longer rods are straight and require bending.
Rod preparation3
The screw is inserted into the pedicle.
The screwdriver is disengaged by unscrewing the upper black tightening wheel while the rotation of the screwdriver is blocked by maintaining the handle.
The screw head positioner is used to orientate and align the screw heads.

The rod is inserted using the straight rod holding forceps.
Erisma®-LP Evolution offers three rod introduction options:
The rod pusher is used to firmly insert the rod in the screw head.
The pongs of the rocker are engaged in the holes located on the external sides of the screw head. The instrument is tipped over to press on the rod such that the rod is positioned into the head of the screw.

The set screw is placed on the snap fit T30 tightening wrench then placed on the screw head and screwed in by engaging the threads.
The rod gripper or the hexagonal wrench can be used to apply a rod rotation movement and restore appropriate curvature along the sagittal plane.

The set screw is placed on the set screw holder then placed on the screw head and screwed in by engaging the threads.
The counter torque is placed on the screw where final locking is carried out. The snap fit T30 tightening wrench connected to the torque limited T handle is inserted into the counter torque until contact with the set screw. Final tightening is carried out.

A crosslink can be added to enhance the stability of the construct.
A • Measurement of the crosslink
Using the crosslink holder, the transverse connectors of the crosslink are placed on the rods and the set screws are tightened using the T20 wrench. Once the lateral set screws are locked, the central set screw can then be tightened.
The appropriate size (S, M, L, XL, XXL) is established using the crosslink template. The straight rod holding forceps can be used as gripper for the crosslink template.
