President’s Note Dear Friends,

Looking back at 2012, there are so many things that we have accomplished together – enhanced VNA programs, the expansion to Merrimack Valley Hospice House, award-recognition for HomeCare, Inc., and branching into Maine with certified hospice services. We’ve had thousands of successful visits with our patients and wonderful care provided throughout more than 100 cities and towns.

Our organization is strong – we have been in continuous service for 118 years and our longevity is based on our ability to weather the fiscal storms that we might be faced amid political or economic uncertainty. We have a solid and stable foundation that is a hallmark of our longstanding tradition of excellence.

By anticipating changes, we have found ways to be more resourceful as healthcare dollars shrink. Investing in technology, we have been able to monitor and treat more patients by expanding access to specialized expertise. With predictive modeling, we have been able to prepare and allocate clinical support teams to ensure timely, well-coordinated care. And by empowering our patients to become engaged in their own care, we have achieved improved outcomes.

Home healthcare is becoming more critical to the success of our national and regional healthcare systems. Home Health VNA, Merrimack Valley Hospice, and HomeCare, Inc. each play an important role in meeting the community’s medical needs, collaborating across care teams, and identifying risks and areas of concern.

As we rise to the challenges presented by a dynamic healthcare landscape, what holds true is the value we bring to the thousands of patients and their families and the evermore innovative ways we maintain the highest quality care for each and every one.

Your personal pride and commitment to our patients and their families is the most important ingredient in our success – past, present and future.

While I can’t predict the future, I can be certain of a few things as I look forward to the achievements we are pursuing for 2013. Our agencies will continue to deliver the highest quality patient care. For patients of all ages, returning home following an illness or surgery, allows for recovery or care in a familiar and comfortable environment. In 2012, the services and support provided by Home Health VNA enabled patients to be at home with the resources they needed to prevent complications, reduce the risk of hospital re-admission, and monitor progress.

Technology plays a big part in timely and effective home healthcare. In the past year, newly added innovations helped our care team to reach more people and improve the quality of their health. Incorporating predictive modeling, we have been able to anticipate patient needs and identify those patients with higher risks so that we could more efficiently manage care and deliver appropriate preventive treatments.

The increasing importance of home health careWith the changes of health care reform comes an emphasis on ensuring that people receive the right care, at the right time, in the most cost-effective setting, while achieving the best possible outcomes. Home health care is becoming an increasingly important segment of the health care system, to both our patients and our provider partners.

Beginning in October, 2012, Medicare began penalizing hospitals when patients with certain conditions experienced an avoidable hospital readmission within 30 days of discharge. This was done to help ensure that patients sent home from the hospital remain healthy at home, and it presents a great opportunity for us to demonstrate the highly skilled and effective clinical services that we can provide to patients during this important time.

Our clinical professionals, with their advanced training and expertise, are vital to helping ensure that patients are safely following their discharge plans, receiving the care needed in the home, and, should a problem be identified, are directed to the right level of care before the issue requires a return to the hospital. Home Health VNA was a partner in the coordinated care of 14,300+ patients this past year – aiding in discharge from hospitals, skilled nursing and rehabilitation centers and keeping patients from preventable re-admissions. Home Health VNA is a critical part of the success of today’s changing health care system.

Our three agencies work together to help our patients and their families enjoy the special moments

of life – big and small. Home Health VNA, Merrimack Valley Hospice, and HomeCare, Inc. each offer

specialized services and medical care that keep people healthy, safe and comfortable at home.

With your support, our 800 professionals serve patients in many settings: the community, private

residences, assisted living and skilled nursing facilities, hospitals, and at Merrimack Valley Hospice

House when care at home is no longer appropriate. We are able to do all this with medical expertise,

an unwavering focus on compassionate care, collaborative care teams, and the use of technology –

made possible in part by the generosity of those like you who understand the importance of those

things that ensure the quality of life.

As a not-for-profit health care provider, we are responsible and accountable to the communities and

people we serve. We place importance on providing the expertise and resources needed for you, your

neighbors, and friends to be cared for and supported in a safe and comfortable setting – providing

compassion along the journey.

New technology aids stroke recoveryParalysis caused by a stroke is one of the most common conditions treated by Home Health VNA’s physical, occupational and speech therapists.

“Right or left arm paralysis can be very frustrating to patients,” says Christine McQuade DPT, Therapy Program Manager. “With the use of new technology purchased with a grant from the George C. Wadleigh Foundation, we help patients get movement back in their arms and increase their ability to live independently.”

MYOMO (short for “my own motion”) Mobility Systems is a recently developed technology to aid in stroke rehabilitation. It’s a neuro-robotic arm brace that fits like a sleeve on a person’s arm. Sensors detect even a very faint muscle signal so that when the patient tries to move his arm, the robotics engage to assist in completing the desired movement. Over time, this movement re-teaches the brain to move the arm again. Along

with regular, routine physical and occupational therapy at home, MYOMO can help a stroke survivor begin using their arms for routine, daily tasks that were not possible before, helping them to return to independence.

“Very few home health care agencies have this advanced technology,” says McQuade. “We are thrilled to be able to offer it to our patients.”

Wound CareWound treatment requires careful monitoring, skillful treatment and, if complex, numerous visits to ensure proper healing. Using technology, we are able to enhance care for these patients and engage the wound specialist in a timely manner, ensuring the right care at the right time.

WoundMatrix Mobile is a precise, web-based, digital wound measurement and documentation platform that enables our visiting nurses to document and send images to a wound care specialist at our main office. The recent introduction of this innovative technology, provides specialized consultation and support to many more patients than if a physical visit by the specialist had to be made to the hundreds of wound patients we care for every day. And by sending the images to our patients’ physicians, we keep them connected to their patients’ progress and alert to them to medical needs that may require intervention.

Bridging CareMany of our young patients have long-term, chronic or complex illnesses that are managed by a primary Home Health VNA nurse. These professionals are often a constant in the lives of patients and their home caregivers, helping to care for and coordinate services.

Home Health VNA pediatric nurse Sharon Tetrault, RN, was assigned as primary nurse for a 7-year old patient of the Jimmy Fund Clinic and Boston Children’s Hospital – a child receiving cancer treatments for a complex tumor. For nearly two years Sharon oversaw the child’s medication regimen, wound dressings, and clinical needs while she was at home. She also coordinated care with the many physicians and other clinicians from various specialties – incredibly important for the patient’s mother who was overwhelmed at times by the challenges of caring for a child with a terminal illness.

A frantic call to Sharon one evening by the child’s mother revealed a serious change in the girl’s condition. Sharon calmly tended to the young child’s emergency, relayed critical medical information to the Boston medical team, and gave reassurance to the child and her family.

When it came time several months later for the child to transfer to Merrimack Valley Hospice, Sharon assisted in the transition to a new care team, continued to provide support to the family, and educated the hospice team on the complex dressing changes that were part of the child’s care.

Sharon is one of many in our family of agencies who bring compassionate support, in addition to their clinical expertise, to our patients and their families. In addition to medical expertise, our staff bridge services among care teams and across our agencies, to ensure that patients smoothly continue their journey.

Home Health VNA pediatric nurses provide specialized, medical care for a variety of conditions. They are often the constant in the child’s care team which can span hospitals, specialties and a variety of conditions being managed, helping both the young patient and the parent along the way.

Hospice is a form of compassionate, comprehensive care for those who are facing a terminal illness that is no longer responding to curative treatment. It is designed to improve quality of life by meeting physical, social and emotional needs with a team of professionals and volunteers. With the concerns of the patient and loved ones being met – at home, in a hospital, in a living facility, or at Merrimack Valley Hospice House in Haverhill, MA – focus can be on meaningful interactions. During such significant times, hospice care fosters a peaceful and comfortable setting, and delivers support for patients and their families.

In 2012, Merrimack Valley Hospice celebrated achievements large and small to reach more families than ever. With the approval of our Board of Trustees, we broke ground and commenced construction on a new wing of Merrimack Valley Hospice House. We welcomed the York, Maine community into our service area with a partnership with York Hospital. But, the most important achievements were providing the unique moments that the 1,487 patients and their families were able to have together – surrounded by the love and support of each other, respectful and dignified.

Moving up the seacoastIn 2012, expanding care for more patients and families included our bringing expertise to new communities. In May, Merrimack Valley Hospice partnered with York Hospital to bring the highest quality end-of-life care to the York, Maine community.

As York Hospital Hospice, we are one of only two not-for-profit hospices in Southern Maine, providing comprehensive, compassionate care, including pain and symptom management, counseling and support for patients nearing the end-of-life. We are proud to bring our 33-year tradition of hospice care to a new community and welcome York, Maine to our family.

One size does not fit all

When it comes to grief, one size does not fit all and it certainly doesn’t have a stop watch. That’s why our bereavement support programs go above and beyond to give our patients a wide range of options including locations, times of day, and type.

Group support is available in each region we serve across northeastern Massachusetts and southern New Hampshire, and in York, Maine. One-on-one visits are provided for those who have requested personal attention or who are identified as needing a more private approach. Special programs just for children take place during the year in addition to personal sessions. All bereavement services are open to the entire community, whether or not someone received care on our hospice program.

Since bereavement services are not a billable service – they are not reimbursed or covered by any type of health insurance payment – charitable giving, donations from individuals and grants, is how we are able to provide this vital element of care.

Bereavement services, which may incorporate expressive therapies of music, art, crafts and

other activities, have a ripple effect on positive coping by those who have lost a loved one. Our staff and volunteers continue to check in on families who have experienced a death for up to two years.

Merrimack Valley Hospice’s bereavement services are there for all who need help coping with death – it is a commitment to our community and a promise we deliver.

In 2012, Merrimack Valley Hospice made more than 3,400 check-in calls and visits to family members, hosted more than 125 support groups, and answered more than 475 community calls for bereavement assistance.

Merrimack Valley Hospice HouseThe majority of hospice care is provided to our patients in their homes. But, when it’s no longer possible for someone to remain at home, Merrimack Valley Hospice House provides a comfortable, family-friendly alternative to a hospital or other facility. It is a place that brings trained hospice professionals – physicians, registered nurses, nursing assistants, chaplains, social workers and volunteers – under one roof to provide comprehensive end-of-life care to patients and their loved ones.

One of only two inpatient hospice facilities in the state of Massachusetts licensed to care for children as well as adults, Merrimack Valley Hospice House addresses intense medical needs with compassion, in a home-like setting. While not used by every hospice patient, it is available to all if needed.

In 2012, the Board of Trustees approved an expansion of Merrimack Valley Hospice House from 14 to 21 suites; a decision made after operating the existing facility at or near capacity for more than a year. Groundbreaking for the new seven-suite wing began in August with the exterior construction completed just before the first snowfall. The addition of the new wing and its anticipated opening in early June 2013, will mean that an estimated 800 more patients each year have the chance to spend their last days in the safety, comfort and peace of Merrimack Valley Hospice House when care at home is no longer an option.

With early support of foundations and pledge commitments by several lead donors, the Compassionate Journey Campaign for Merrimack Valley Hospice House was developed to help fund the costs of the expansion as well as the charitable care and extensive support services offered to patients, their families, and the community at large.

Part of your CommunityWe are grateful for the early outpouring of support in continuing this compassionate journey. As a unique and valuable hospice resource, the Hospice House is available for you, your family, your neighbors and friends, if ever needed.

Supporters of the Compassionate Journey Campaign include those who live, work and care for our community. Scott Cote, president of Haverhill-based Pentucket Bank, serves as co-chair for the Compassionate Journey Campaign to support the expansion of Merrimack Valley Hospice House. Knowing that local charities strengthen the community we live and work in, Pentucket Bank made an early pledge to the campaign in support of a unique and important facility. The bank was joined by other area businesses and foundations such as The Rogers Family Foundation and The Abbot and Dorothy H. Stevens Foundation in late 2012 at the start of the campaign. We are truly grateful for the support of the expansion to date and look forward to the continued generosity of those who wish to support compassionate, end-of-life care in our community.

Remaining independent and healthy at home sometimes depends on just a bit of assistance. HomeCare Inc. helps people remain safe, healthy and comfortable in their own homes with services such as homemaking, personal care attendance, transportation, and other services.

The role of home care professionals in the care team has increased. In 2012, this agency of Home Health Foundation became more instrumental in being able to help monitor the health and wellbeing of those in our communities as well as the patients we serve through Home Health VNA. Additionally, our community-based health programs such as preventive clinics, exercise programs, diabetes education, and immunization clinics delivered through HomeCare, Inc. improve health and quality of life for many of the region’s senior population.

The Power of Communication Technology In 2012, HomeCare, Inc. received a grant to add Interactive Voice Response (IVR) to help reduce avoidable re-hospitalization of medically fragile patients who require medical supervision and disease management after they have finished skilled nursing service with Home Health VNA. These Medicare patients are often left “in between,” not on active treatment but still requiring some oversight to prevent or identify complications.

IVR is an automated telecommunications system that functions as an in-home monitor, educator and counselor for patients. IVR engages patients in regular automated telephone conversations designed to identify worsening chronic disease or risks leading to hospitalization.

Operating over standard phone lines, the system uses computer-controlled speech to ask patients yes or no questions that a nurse would typically ask during a home visit. Patients answer these questions via their touch-tone telephone keypad. Based on these responses, IVR provides appropriate feedback to the patient and sends the information to our IVR monitoring nurse. If intervention is needed, a nurse or home health aide can be sent to check on the patient.

With IVR, we have been able to efficiently serve the medical needs of these patients who otherwise would be untended. Furthermore, by engaging them in self-care, medication compliance improves and early warning signs can be detected to avert serious medical issues.

During a time when we are asked to do more with less, being able to invest in technology that truly expands the reach of our staff is more significant than ever. Charitable support of these investments provides the resources we need to work efficiently as well as to deliver an even higher quality care.

HCI Community ProgramsIn 2012, we continued to partner with town-based councils on aging and Elder Services of the Merrimack Valley to deliver preventive health clinics for seniors in a number of communities in Massachusetts and New Hampshire. Bringing needed preventive care clinics to the places where people feel comfortable gathering and making sure health needs are tended to, we helped to keep folks who are living on their own, active and healthy.

Get up and move, go out and live!HomeCare, Inc. works in partnership with the City of Lawrence to provide healthy opportunities to seniors. Keeping people active and promoting lifestyle changes can go a long way to improving quality of life.

A vibrant group gathers Monday through Thursdays at the Lawrence Senior Center. When you step into the large room, you see 50 to 80 men and women, of various shapes and sizes. You hear different languages spoken. You also see many smiles and friendly greetings before the dynamic Lyn Licciardello begins to lead the exercise class in both English and Spanish.

Talks about healthy eating and fitness start the program before the music begins and the room fills with upbeat energy and movement. And while exercise and physical health is the purpose, the program does even more for the variety of people who come.

“My first time coming to the exercise class, I didn’t know anyone, but people were greeting me with warm smiles and made me feel so welcome,” said a Lawrence participant. “They immediately had me feel like I had a roomful of friends. I look forward to coming to class and seeing other people.”

Providing healthy, social outlets for those in our community who may not otherwise have access to such opportunities is just one of the ways we help keep people safe in their own places of residence.

In the latter part of the fiscal year, HomeCare, Inc. was awarded a grant for a Population Health Program to target high-risk, frail elders by monitoring them between episodes that require VNA service. This program will help support them safely at home and prevent re-hospitalizations. We look forward to reporting on the successful results of this grant-funded program in 2013.

As a not-for-profit organization with a long history of caring for those in our community, charity care is provided to those in need. The generosity of our donors enables us to uphold this promise of quality, compassionate home health and hospice care for all, especially those who are without health insurance or the means to pay.

Special EventsSpecial events provide an opportunity for our supporters to come together on behalf of a common cause that

is of interest to them. With the dedication and efforts of many – generous sponsors, committee volunteers,

donors, and attendees – these annual events help fund the work of our agencies throughout the year.

Honoring a Legacy of LeadingEach year, the Legacy of Leading pays tribute to those in our community who show great support of the health and wellbeing of the Greater Merrimack Valley communities, as well as a commitment to the mission of the agencies of Home Health Foundation.

In 2012, this event was held at the Atkinson Country Club in honor of Scott Cote, President and CEO of Pentucket Bank. Merrimack Valley Hospice is among the many organizations benefiting from Scott’s dedication to community support. As a community business leader committed to improving the region through philanthropy and volunteerism, Scott has also championed a legacy of caring for hospice patients and their families, most notably upon the opening of Merrimack Valley Hospice House in June 2009 and the ongoing support to the organization.

Food, Wine and Beer Pairing Along Route 66 More than 350 sponsors, supporters, volunteers and guests from throughout the region helped drive home Merrimack Valley Hospice’s premier fundraiser Road Trip USA Food, Wine and Beer Pairing on September 20th. More than $110,000 was raised through sponsorships, auctions and raffles which will support programs and initiatives that benefit Merrimack Valley Hospice and its patients.

The Black Swan Country Club in Georgetown, MA was transformed into the historic Route 66 where guests sampled Americana cuisine and

beverages while enjoying live music. The signature silent auction showcased nearly 100 items donated by community members in the Merrimack Valley and throughout New England.

Each year the annual Food, Wine and Beer Pairing raises the critical funds needed to provide medical and supportive care to those patients and families who are either under-insured or uninsured.

Candles of Remembrance 2012This special event offers those in the community a chance to come together in quiet solidarity and support amid the darkness of winter. An open invitation was given to join in this celebration of remembrance.

With a quiet ceremony led by Chaplain Terry Holbrook of Merrimack Valley Hospice, each guest lit a candle and passed the flame to honor his or her loved one. Refreshments and the music of a harpist were enjoyed by guests and volunteers who gathered to spend time together after the ceremony.

We received more than $19,000 in voluntary donations to this event, and memorial gifts on behalf of more than 230 individuals. Funds will be used to provide resources to help thousands of current and future patients and families who turn to us for comprehensive hospice and palliative care.

Ways to Give Thousands of people are touched by the generosity shared with our agencies, and support can come in many forms:

Cash ContributionsCash contributions are fully tax deductible. Many companies match the gifts of their employees – check with your employer to see if your gift can qualify.

Gifts of Appreciated PropertyYou can donate appreciated property (stocks, bonds and real estate) to support any of our agencies. When you donate property that you have held for more than a year, your gift is exempt from capital gains taxes. Your charitable deduction is based on the fair market value of the asset at the time of your gift.

Vehicle DonationsDonate for Charity, our recycling partner, offers an opportunity to support our agencies by donating a car, truck, van, boat, RV, or motorcycle by calling 866-392-4438. Donors receive a tax deduction of the auction sale price of the vehicle or $500, whichever is the greater amount.

Income-generating GiftsYou can also use cash and appreciated property to establish a charitable remainder trust or charitable gift annuity that will provide income and tax benefits to you, while benefiting one of Home Health Foundation’s agencies.

BequestsA simple way to support is through a bequest in a will or living trust. Any part, percentage, or component of estate or trust assets can be designated to Home Health Foundation or one of its agencies and will help to reduce inheritance taxes.
