Choose a health plan that works hard. One with dedicated support and tools to help you and your employees stay healthy.

Maybe save money, too.

Health benefits and health insurance plans are offered and/or underwritten by Aetna Health Inc., Aetna Health of California Inc., Aetna Health Insurance Company of New York, Aetna Health Insurance Company and/or Aetna Life Insurance Company (Aetna). In Florida, by Aetna Health Inc. and/or Aetna Life Insurance Company. In Idaho and Wyoming, by Aetna Health of Utah Inc. In Maryland, by Aetna Health Inc., 151 Farmington Avenue, Hartford, CT 06156. Each insurer has sole financial responsibility for its own products.

* Plan availability varies by market. A health savings account (HSA) needs to be paired with a qualified high-deductible-based plan.

** HRAs are currently not available to health maintenance organization (HMO) members in Illinois and Small Group members in Florida.

Aetna HealthFund One-to-One plans start with you choosing any major medical plan you like.* Your Aetna representative can help you decide on the one that’s best for your workforce.

After you choose a health benefits or insurance plan, you then decide to pair it with either a health fund or health savings account. Integrate medical with behavioral health and pharmacy for better overall health coverage.

Employees typically have higher out-of-pocket costs with these plans, but may pay less in total because of:

• Lower premiums

• Funding options to help offset out-of-pocket costs

• Preventive care options with no out-of-pocket costs

• Online tools that help them manage costs and make more educated health care decisions

You can choose the funding option that works best for you. We’ll even help you figure out what fits your budget and business culture.

Health reimbursement arrangement (HRA)**

First choose a medical plan.

Then pair it with an HRA — also called a health fund. This fund helps employees pay for covered health expenses, especially when they’re in the deductible part of their plan.

When you contribute to an HRA you’ll help offset your employees’ costs. And get extra pluses because it’s:

• Tax deductible for you.

• Tax free for your employees.

• Flexible — you choose run-out periods, contribution schedules, rollover options and reimbursement expense types.

• Convenient — we automatically reimburse eligible Aetna medical and pharmacy claims to your employees.

How it works

• Each year, you create a fund for each covered employee’s plan.

• Eligible health care claims (doctor visits and more) are reimbursed to the member from their fund.

• Expenses paid by the fund help offset the cost of the deductible.

• Once the fund is used up, members pay their share of costs from their own pocket.

• Money left in the fund at the end of the year can be rolled over and added to next year’s fund.

• Members forfeit the fund once they leave the HRA plan.

*** HSAs are currently not available to HMO members in California and Illinois. † To be considered a high-deductible health plan, the plan must meet deductible and out-of-pocket requirements set each

year by the U.S. Treasury. Your Aetna representative can tell you more about these requirements and the plans we designed to meet them.

Health savings account (HSA)***

First choose a high-deductible health plan.†

Then pair that plan with an HSA. This account helps employees save and pay for eligible health expenses, throughout the year and in the future.

There are many advantages. An HSA is:

• Tax free.

• Owned by employees. They get to keep the money from year to year or job to job.

• Flexible — you and your employees decide how much to fund.

• Easy for employees to use with the PayFlex Card® account debit card or online payment.

• Easy for you to manage. Just upload the funding file to the employer portal. We use ACH debit to pull funds from your bank account.

• Convenient for employees. They can use the fund to pay for eligible expenses as they are incurred, or save the funds for future medical expenses.

How it works

• You and/or your employee can put money in the account.

• HSA funds can be used to pay for qualified medical expenses (as determined by Internal Revenue Code 213[d]). This includes prescription drugs, doctor visits and more.

• Funds in the HSA earn interest tax free and roll over from year to year.

• Investment services may be offered for HSA balances above $1,000.

• Employees can keep the account even if they change their job or health plan.

Flexible spending account (FSA)

An FSA makes it easy for your employees to contribute pretax dollars from their paycheck to pay for eligible health care expenses.

FSAs help employees budget for yearly costs. They can put aside pretax dollars to pay for eligible health expenses throughout the year. Since rolling over funds to the next year is limited, it is important for employees to be able to plan for annual medical expenses. Employers can choose if they want to carry over up to $500 each year or a grace period may apply (but not both).

FSAs have other great benefits. They are:

• Flexible for your employees — they decide how much to put into the FSA.

• Flexible for you — you can choose run-out periods and payment methods (autopay and debit cards).

• Easy for you to administer. You just upload a file to the employer portal. We post deposits to accounts for the contribution amount and payroll schedule.

• Convenient, because we automatically reimburse eligible Aetna medical and pharmacy claims to the member. Or you can elect a debit card.
