As president of the Board of Directors during 2010-2012, I am pleased to present this bi-annualcommunity report in recognition of the generous and dedicated donors, volunteers, and partnerswho make comprehensive, effective, and quality care available to people who are homeless.
For me, these years were remarkable, representing transformation and recommitment for the vision of Albuquerque Health Care for the Homeless (AHCH). The Board of Directors andAHCH Staff made some necessary realignments aimed at maximizing our impact while sustain-ing services essential to ending homelessness.
Our renewed focus builds on a time tested principle that housing is health care. A mix of services focused on comprehensive primary health care, supportive services and permanentsupportive housing is AHCH’s legacy.
Your financial support enabled AHCH to sustain the services necessary to address the healthrelated causes of homelessness while advocating for an end to low paying jobs, unaffordablehousing, domestic violence and a lack of health care to people who need it most. Other impactsyour donations made possible:
• Renovations and repairs to our buildings were completed
• The City of Albuquerque contracted with us to offer our Case Management Support Services to people housed through the Albuquerque Heading Home Initiative
• The Board is now more diverse and represents more of our community racially, ethnically and professionally
• The National Endowment for the Arts funded ArtStreet’s first professionally published book Synergy: La Onda de la Palabra
• The Affordable Care Act awarded $3.65 million to expand services and program space adjacent to our First and Mountain campus
This community report provides a two-year scan of the health of our organization andrecognizes the community supporters who sustain us over time. I appreciate the opportunityto report on behalf of the clients, volunteers, staff, and Board of Directors.

YOUR IMPACT
Your belief in the work we do makes it all possible. Albuquerque Health Care for the Homeless strives to be the best, continually re-evaluating business andprogram strategies. As a nonprofit organization serving people experiencinghomelessness in our community, we have focused our effort on expanding services to people who are most vulnerable to the negative health and socialoutcomes and/or at greater risk of death due to their homelessness.
“BEING ADDICTED IS NOT EASY; IT TAKESYOUR TIME, YOUR MONEYANDYOUR INTERACTIONWITH OTHERS.” CLIENTS TOLD US THERE-ENTRY COLLABORATIVE PROGRAM HELPED BREAK THAT CYCLEOF EVENTS, OFTEN FOR THE FIRST TIME IN THEIR LIVES.

For participants who completed 6 months in the Re-Entry Collaborative Program, key changes overtime included a 79% abstinence rate, a change from sleeping on the street or in a shelter to doubled-up,transitional housing or permanent housing (supportive or otherwise), and a 37% increase in the numberof participants who are employed or attending school.
The Albuquerque Heading Home Initiative makes it possible for people who are chronically homelessand medically vulnerable to receive the more intense mix of services necessary to end their homelessnessand maintain housing. Since the inception of this partnership,AHCH has seen a significant improvement in ourrelationships with other agencies and has helped to build the coordinated community response to home-lessness that we’ve strived for over the last few years.
These two programs, like all AHCH programs,build on and substantiate national evidence thatAHCH integrated and comprehensive strategiesimprove health outcomes, quality of life and socialconnections while making it possible for people toreclaim or learn for the first time, their abilitiesand improve, the resources availabe for them togo back to school or work… all necessary to endone’s homelessness.
AHCH provides critical services that address the health-related causes and conse-quences of homelessness. AHCH reaches over 7,000 men, women, and children each year.Outreach and site-based programs include:
• Primary medical, dental, and specialty care;
• Con Niños medical/social outreach treatment team for homeless children and their families;
• ArtStreet, a free community-based arts and art therapy studio project;
• Harm reduction street outreach, including syringe exchange, auricular acupuncture detox, overdose prevention and training on administration of Naloxone, and a peer education/prevention project;
• Social and behavioral health continuum;
• Drop-in center & social services (comprehensive case management, social work and client advocacy)
• Re-Entry Collaborative Project (REC), opiate-replacement treatment for individuals addicted to opiates, recently released from jail/prison and experiencing homelessness.
• Integrated behavioral health program, including counseling and therapy and brief interventions.
• Housing placement and assistance via 50-70 Shelter+Care unit subsidies each year, in addition tohousing assistance and placement in other private market and subsidized housing units.
• Albuquerque Heading Home a coordinated approach to housing first with wrap-around, innovativeand integrated primary medical, and behavioral health care and social services
