This 55-year-old patient presented with an 18-month history of worsening adhesive capsulitis of the right shoulder. Arthroscopic coracohumeral ligament release and capsulotomy was performed. After capsular release, the axillary nerve was easily identifiable within the axillary pouch just beneath the inferior capsular pouch. The image shows a relatively unusual branching pattern of the axillary nerve in which the anterior and posterior branches bifurcate proximal to the inferior border of the subscapularis muscle belly. The anterior branch continues through the quadrilateral space to innervate the teres minor muscle while the posterior branch wraps around the humeral neck to innervate the deltoid and provide the terminal cutaneous branch.
