The COVID-19 vaccine passport is free.

It’s proof that you are protected against COVID-19.

Who can get a vaccine passport?

• Vaccinated people age 13 and older

• People age 13 and older who have a clinical contraindication to the COVID-19 vaccines

People under age 13 will be able to enter venues that require the vaccine passport, where permitted.

Why get a vaccine passport?

To help limit the spread of COVID-19.

So you can enter certain places and do certain things.

Places where you have to show your vaccine passport

• Outdoor sports and physical activities (sports with frequent or prolonged contact) Examples:

• All indoor sports activities

• Outdoor events and festivals with more than 50 people

• Performance venues, movie theatres, sports venues

• Bars and restaurants

• Arcades, theme parks, amusement parks and centres, recreation centres, and water parks

• Casinos and gambling halls, including bingo halls

• Cruises and boat tours (whale watching, dinner cruises, sightseeing tours). Exception: ferry services

• Conferences and conventions

How does the vaccine passport work?

When asked for your vaccine passport, you must show proof of vaccination against COVID-19.

Proof of vaccination can be provided in three ways:

1. On paper

2. As a PDF on your mobile device

3. In the VaxiCode app

You will also be asked to show photo ID.

A word to the wise:

• Don’t post photos of your proof of vaccination on social media.

Physical copy of your QR code

You can show the QR code emailed or texted to you after your most recent dose.

1. Didn’t receive a QR code? Download it from the self-service portal at https://covid19.quebec.ca/PreuveVaccinale/en.

2. Follow the instructions to print your proof of vaccination.

If you have trouble downloading your proof of vaccination, ask a friend or family member for help.

Or visit a vaccine clinic to get help obtaining your QR code.

Request by phone

If you don’t have access to the Internet and a printer, please call 1-877-644-4545 and request a physical copy of your proof of vaccination by mail.

QR code as a PDF on your mobile device

You can use the QR code sent after your most recent dose.

Didn’t receive a QR code? Download it to your mobile device from the self-service portal at https://covid19.quebec.ca/PreuveVaccinale/en.

Then you can just show the code on your device.

QR code in the VaxiCode app

You can add the QR code emailed or texted to you after your most recent dose to the VaxiCode app.

Didn’t receive a QR code? Download it to your mobile device from the self-service portal at https://covid19.quebec.ca/PreuveVaccinale/en.

Then download the VaxiCode app and follow the prompts to add your proof of vaccination.

If a venue requires a vaccine passport for entry, just open the app.

Protection status

The vaccine passport indicates whether you are protected against COVID-19.

If you are protected, when your proof of vaccination is scanned, the message “Adequately protected” will appear in green.

If you do not meet the criteria to be considered adequately protected, “Not adequately protected” will appear in red.

To obtain “Adequately protected” status and avoid being turned away, you can:

• Make an appointment for your first or next dose of the vaccine atQuébec.ca/vaccinCOVID

• If you were vaccinated outside of Québec, make an appointment to haveyour vaccines recognized and obtain your proof of vaccination
