    New and improved ward boardsAs part of our ongoing patient experience improvement work - the new welcome boards (to compliment the new bedside information) have started to arrive.

    You may have seen the ones already outside Department of Critical Care and Intensive Treatment Unit. It was also the winner in the poster presentation at this year's SHWC conference, which have us the idea of spreading across DBTH. We have worked with our ward managers and matrons to develop these for our clinical areas to improve communication for patients, friends, relatives and carers. The final result is shared with you below. Maternity, Paediatrics and Outpatient areas will be following soon.

    In an effort to bring the healthcare service ‘’into the 21st century’’, all NHS organisations have been instructed to stop buying fax machines.

    This is a national campaign and all trusts will be expected to discontinue their use of fax machines by 31 March 2020.

    With fax machines being phased out over the coming months, Divisions will need to review their current working practices, so that they are no longer reliant on any outdated communication channels. In other words, if you previously used fax machines for any aspect of your work, then you will need to prepare to use the NHSmail platform as a replacement.

    For employeesIn four weeks time, everyone in the Trust will have access to ESR Self Service. With this, you will be able to log in to the ESR portal and update your personal information through a quick online form.

    You can find plenty of information on how this works via the newly created Hive page, so make sure to check it out. In the meantime, you might also want to consider booking onto a webinar for a bit of extra support.

    ESR Self-Service - One month to go!

    ESR Manager Self Service will go live on 1 October 2019. From this date, the way that managers make assignment changes to staff will change.

    Specifically, they will be able to see data on their employees including absence, training compliance and appraisals. What’s more, they will be able to do this on their PC, or via the app, which is available on Android and Apple.

    CQC inspection in 2019The Care Quality Commission (CQC) has arrived at Doncaster Royal Infirmary, Bassetlaw Hospital and Montagu Hospital this morning to undertake an unannounced inspection and consider any improvements we have made since our last inspection in late 2018. The team will be on-site for the next three days and we expect the inspection to focus upon our Emergency Departments, Maternity, Diagnostic and Imaging and Outpatients at both Doncaster Royal Infirmary, Bassetlaw Hospital, Montagu Hospital and Retford Hospital. The CQC usually speak to both patients and staff, they may also observe care, review records and policies and inspect the places in which we provide treatment. The inspection team will have identification with them and should be asked to provide identification prior to access to any areas. As was noted at the time of the last inspection, we have a lot to celebrate at DBTH. This is our opportunity to tell the inspectors about the excellent work which takes place across the organisation, building upon our successes as an organisation, as well as meeting any challenges we face. Everything is business as usual and, as always, our patient’s needs come first at all times. The inspecting team will understand and expect this but they may ask you or your colleagues some questions to help with their inspection. There are no lines to learn, we just want you to be proud as a member of Team DBTH and share the excellent work you do each and every day.     Audit of Controlled Drugs storage and record keepingPharmacy staff audit the storage and record keeping for controlled drugs (CD) by wards and departments every six months. In line with the results of monthly ‘snap shot’ audits of medicines storage on wards, the compliance with the requirement to undertake daily CD checks has improved and 95% of stock holding areas were judged to be compliant with this requirement. The accuracy of stock balances and the standard of arithmetic in the records were also extremely high and overall the standard of storage and record keeping was judged to be compliant with policy.

    The current round of audits is almost complete and while the results of each audit are discussed with the nurse in charge (or other responsible person) at the time of the audit, the team have put together a helpful summary including some general actions for us to follow to improve practice.

    Car parking update

    Works continue on the introduction of Automatic Number Plate Recognition at the Trust. The following car parks will go live on these dates:

    • 3 September – Ambulance station• 9 September – Underground• 16 September - Hut 5 & Springwood• 23 September - Women’s & Childrens

    If you have a permit to park on site, please make sure you have emailed your registration details to [email protected]. All staff who park on site and pay for car parking through salary, need to be advised that the Trust will share your name, car registrations details and permit number with Saba UK to ensure you can safely enter and exit the ANPR car parks.

    In addition to the ANPR works, CCTV is being installed at Bassetlaw Hospital and speed bumps are being introduced to the Bassetlaw triangle and Montagu car parks.

    Nursing and midwifery workforce

    Midwifery Following the appointment of the new Head of Midwifery, Lois Mellor in January, the midwifery workforce has been reviewed with two tabletop Birthrate+ tools undertaken, one in January 2019 and one in June 2019.

    The new midwifery structure has also been created to ensure that all areas have a Band 7 Manager and Band 8A Matron. The workforce in each area has been reviewed, and agreed with the relevant ward managers. There have been a number of specialist midwifery posts created to support the service and move it forward.

    A full Birthrate+ assessment has been commissioned by the DoNMAHP and the HoM, which will be undertaken at the end of September.

    The Associate Director of Nursing and the Head of Midwifery will then work out the skill mix for the areas and have any changes signed off by David Purdue in his capacity as Director of Nursing, Midwifery and Allied Health Professionals.

    Nursing The nature of patient needs and ward activ-ity is changing. Inpatients tend to be more ill than they used to be, many with complex needs often arising from multiple long-term conditions. At the same time, all hospitals in the NHS face the challenges of a shortage and high turnover of registered nurses.

    This summer, we signed a licence for the Trust to use the Safer Nursing Care Tool (SNCT). Although we had a similar system in place, the SNCT is endorsed by NICE and now freely available to all acute trusts. SNCT will be used on all adult inpatient wards, adult assessment wards and children’s ward twice a year, to give these areas an average establishment.

    There are a few changes this time from previously:

    1. Data is to be collected at 3pm each day

    2. Data is to be collected Monday to Friday for a period of four weeks. No data is required on weekends.

    Physiotherapy Musculoskeletal Outpatients leading the way in student education

    Universities across the region are struggling to find placements for Physiotherapy students and as a teaching hospital we are aiming to increase our student capacity to cope with this demand while maintaining high quality student placements. In MSK outpatient physiotherapy at DBTH, we have piloted a 2:1 model of clinical education to help increase our student allocation. This involves one educator taking on two students at the same time rather than a traditional 1:1 model to give more students quality placements here at DBTH.

    NewsR&D Conference Research and Development (R&D) is a vital part of what we do here at DBTH, as it plays a significant role in improving the care and treatment that we deliver.

    With that in mind, it’s important that we take the time to acknowledge the massive difference that research makes and recognise all of the contributions that have been made by Trust staff.

    Which is why we host an annual conference, celebrating our recent R&D achievements, showcasing practical examples and offering people advice on how to get involved. It always proves to be a fascinating event and this year looks to be no exception, featuring a wide range of speakers and engaging topics to sink your teeth into.

    Among other things, the 2019 conference will entail:• Informative talks with our Trust researchers• An eye-opening patient testimonial, covering

    the impact that research can have on someone’s life

    • Presentations from statisticians, the Research Design Service and other experts from outside the Trust.

    The event will take place on Wednesday 6 November, in the DRI Education Centre. Although it will last from 9am – 4pm, the conference is broken up into separate morning and afternoon sessions. So feel free to come along to whichever suits you best. Or both!

    ENT Masterclass: National Tracheostomy Masterclass The 12th national Tracheostomy Masterclass is on Saturday 19 October, in the Education Centre at DRI.

    This intensive full-day course is free to attend for nurses and junior ENT doctors. The day will include sessions on: Anatomy and physiology of adult and paediatric airways, evolution of tracheotomy, mini-tracheotomy, ITU care, hospital and community care, disaster avoidance, speech therapy, dietitian’s roles and much more.

    Transgender Educator to visit the TrustWe are pleased to announce that guest speaker Jessica Lynn will soon be visiting the Trust.Jessica is a world-renowned transgender advocate and educator. Her experiences as a transgender woman and parent have motivated her to spread awareness and acceptance for LGBTQ+ communities around the world.

    Over the last several years, Jessica has crisscrossed the globe sharing the story of her journey over 750 times in 27 different countries. She is now internationally acclaimed as one of the foremost transgender speakers, due to her dynamic, refreshingly honest speaking style and signature “ask-me-anything” Q&A sessions.

    Commode cleanliness sees vast improvementsThe latest Commode cleanliness audit has revealed that we're 30% more compliant than the previous audit!

    Many of our wards had 100% compliance and have been utilising the 'I am clean' stickers and tape.

    The tape has been standardised across areas and should be used on all commodes. The IPC team have been offering education and advice to maintain complaince on wards and improve areas which were below 100%.

    This week’s Friday Lunchtime Lecture will focus on medico-legal topics including consent after Montgomery, legislation relating to FGM and the implications of GDPR in clinical practice.     Joanne and Nicola Triumph at HOTHWell done to Joanne Derx, Pharmacy Technician, and Nicola Wilkinson, Cancer Trials Nurse, who recently participated in the Hell On The Humber (HOTH) endurance run.

    The notoriously demanding event has runners complete as many four mile laps as possible in an allotted time frame. Joanne managed an impressive 60 miles, whilst Nicola did 70!

    The pair are members of Danum Harriers running club and Nicola has been running on a daily basis (now on day 275) in order to raise funds for a cancer charity.

    We’d like to say a big ”Thank You” to Priory Lodge Freemason’s Benevolent Fund, for the donation they made to our hospital radio last week.On Thursday 29 August, the charitable organisation presented a cheque for £300 to Trust AM, which was grateful received by the station’s Steve Roberts and John Morris.

    Clinical Audit Awareness Week Clinical Audit Awareness Week is running from 25 November to 29 November and DBTH will be organising all kinds of activities to mark the occasion. There will be an educational drop-in session and celebratory awards on 28 November, Learning Room 3, Education Centre. In addition, you’ll have a chance to nominate your Clinical Audit Hero and the winners will be announced at the event.

    This will be the perfect opportunity to find out more about what Clinical Audit actually means to the Trust, shining a light on some of the best examples from the past year.

    DrDoctor helps to reduce missed appointmentsOur patients are now benefiting from our brand new text reminder service, DrDoctor, which helps them to monitor and keep their clinical appointments as well as access service specific information.

    Part of the DBTH Digital Transformation programme of work, the system launched Thursday 1 August within the Trust’s Ophthalmology, Gastroenterology and Respiratory services and has so far sent thousands of messages, reminding patients about upcoming hospital dates. The system has been so successful that Doncaster Royal Infirmary’s Ophthalmology service has seen missed appointments reduce by more than half, with its average ‘did not attend’ rate going for 13.5% to just 7% in one month.

    To help ensure the continued success of this project, we are asking colleagues to inform patients about this new system and to make sure that we hold up-to-date contact details for individuals. This is critical to support timeliness of appointment alerts and managing your appointment. Nationally, around one in 10 appointments made are missed every year in England, costing the health service millions of pounds. Unfortunately, DBTH is within the top 25% of all trusts in the country for patients not attending appointments, recording around 140 missed a day or 50,000 each year. Equating to around £6 million in lost funding annually for the local hospitals, in real-terms this wasted cash is the equivalent of 200 nurses, 747 hip replacements or 22,388 MRI scans.

    With early successes and positive engagement from patients, we launched DrDoctor within ENT, Trauma and Orthopaedics, Gynaecology, Obstetrics and Midwifery on 2 September 2019.

    Patient safety is the avoidance of unintended or unexpected harm to people during the provision of health care. We support providers to minimise patient safety incidents and drive improvements in quality. Patients should be treated in a safe environment and protected from avoidable harm.

    To mark the first World Patient Safety Day, we would like to invite each inpatient area to attend a morning discussing the key issues that affect patient safety at DBTH.

    From 12pm, we will be launching the new department Quality Accreditation Tool (dQAT).

    By making the most of new technologies, staff can work better together, to improve the quality of care for patients.

    DBTH Digital Transformation is a Trust wide project which will enhance many systems and working practises. These improvements will bring new efficiencies to the day-to-day operations and workflow within the Trust.

    The current key projects:

    Measures the availability of beds and provides real-time admissions, transfers and discharges.

    Allows forms and assessments to be completed on the computer, rather than on paper, reducing paperwork.

    New communication tools for evening and weekend care. Allows staff to keep better track of their tasks.

    Patient observations can be recorded on a secure mobile device to get the most up to date record of their health.
