Critical Incident Plan

Purpose: St Patrick’s College Townsville is a Catholic, day and boarding College, which has the responsibility of students, staff and other members of the College on the grounds during and after school hours. Critical Incident Plan

Definitions

A Critical Incident can be broadly defined as an incident which involves the possibility of immediate or

imminent threat, physical and/or emotional distress to staff, students and other members of the

College; and which may be regarded as outside the normal range of experiences of the people

affected, causing strong emotional or psychological distress among staff, students and other members

of the College resulting in trauma often described as being unexpected or shocking and involving

suffering or loss.

Examples of a Critical Incident could be but not limited to:

• Death or threat of death within the College eg: suicide, tragic death

• Serious injury or illness of within the College

• Serious incident involving the College during an off-site activity, including interstate and overseas

eg: bus accident, terror attack

• Threat or act of violence towards member/s in the College eg: bomb threat, assault/s including

sexual assaults

• Substantial criminal activity eg: vandalism

• Development of a pandemic in the College

• Structural malfunctions causing grievous harm eg: fire, explosion, gas leak

• Weather events eg: cyclone

Critical incident (CI): An adverse incident or series of events that has the potential to damage the

people, operations or environment of the College and any long-term prospects or reputation.

Critical Incident Management (CIM): The process, by which the College builds resilience, responds to

and recovers from a critical incident.

Critical Incident Management Team (CIMT): A group of staff members responsible for carrying out

the functions of planning for and responding to a critical incident.

Incident Classification: Critical incidents are classified into three levels of escalation.

Minor Event: a minor incident or minor injury has a localised impact on staff, students or members

of the community and may entail minor property damage. The incident has been contained and is

unlikely to escalate in severity. It can usually be handled by campus personnel at the organisational

unit level using normal operating procedures.

Moderate Event: an incident or event, which has a localised impact on College operations and may

threaten life or property, or could potentially escalate to a major incident. A moderate event might

include also, the serious injury or death, of an individual student or staff member. A moderate event

may involve the activation of an emergency response, and or the CIMT.

Major Event: an incident or event that has a high impact or imminent severe adverse effect on College

operations stemming from events such as explosion, large fire, shooting, material release, pandemic

or natural disaster. It may entail or threaten to cause multiple fatalities or serious injuries and/or

significant property damage or adverse media reporting. It is likely to involve an emergency response

from relevant Queensland Emergency Services and would usually necessitate activating the CIMT.

Risk Assessment

All activities, both on and off-site are subjected to risk assessment on an ‘activity by activity’ basis.

The College should participate to raise awareness and knowledge of appropriate procedures in lieu of

a possible event. Considering the premise that a CI is ‘outside the range of a normal experience’ the

likelihood of an occurrence of CI can be assessed as ‘rare’; however, the ‘consequences’ can be

assessed as ‘critical’. The College is to manage and be understanding to the reactions of students and

staff in any case of critical incidence affecting the College community, whether in the College grounds

or outside, and to offer a ‘duty of care’ regarding ‘occupational work place health and safety’ and

expected responsibilities.

Description of the College

St Patrick's College Townsville is a Catholic day and boarding College located on The Strand opposite

Cleveland Bay and adjacent to St Joseph’s Primary School (enrolling 300 students) with an enrolment

of approximately 600 girls. Within the day school there are approximately 127 staff, and

approximately 8 to 10 Boarding staff on duty outside of College hours. Approximately 140 students

reside full time during school terms in the Boarding House at the College.

St Patrick’s College Townsville considers the special circumstance of students who live away from

home and respects the individual differences and potential of each student. The boarding community,

as an integral part of the College, and in the spirit of the Sisters of Mercy, strives to build a true sense

of community of all students, parents and staff that reflects the gospel spirit of mutual respect,

freedom and optimism in interactions between all members of the community. This community based

supporting environment is vital in any critical incident and its management.

Reporting to a Critical Incident

St Patrick’s College Townsville is committed to providing a timely and appropriate response

to a critical incident to minimise the immediate and after reactions/affects from the event

and to ensure the needs of staff and students are attended to.

When a critical incident occurs it should be reported immediately to the Principal or in the absence of

the Principal a member of the Critical Incident Management Team (CIMT). A determination to

convene the CIMT and assemble an incident management room will be required and a report should

be in writing to include:

The nature of the incident (sexual assault, break in/vandalism, assault violence/death, cyclone).

The location of the incident.

The date and time the incident occurred.

The number and names of individuals involved in the incident.

Date and time of the report.

Emergency service contacted and by whom.

Name and contact details of the person reporting the incident.

If it is determined that a Critical Incident Management Centre (CIMC) needs to be established, the

Director of Finance & Facilities is to coordinate the preparation of the Board Room. This configuration

is to be facilitated through the pre prepared CIMC box in appendix which is located in the office of the

Director of Finance & Facilities and second in Director of Boarding office.

Critical Incident Management Team (CIMT)

The Critical Incident Management Team has the specific role of assisting the Principal to manage

critical incidents involving the College. These incidents may be either on or off campus or involve an

overseas student for whom the College has undertaken care responsibilities. The Critical Incident

Management Team can comprise of the following personnel:

COMMUNICATION

1. The Critical Incident Director (CID) will communicate with the community by the following means:

Convening a College Leadership Team meeting at the earliest opportunity outlining duties.

Convening a staff meeting at earliest opportunity (preferably before a full announcement to

the general community).

Preparing a factually accurate statement to all staff (academic and others) and

communicating this to them.

Directing the relevant team members to locate, gather and inform closest friends of the

deceased or injured BEFORE any general announcement, and AFTER parents/families have

been notified.

Relay information to student community either in smaller groups via Homeroom teachers or

to school assembly outlining facts of the incident at the earliest opportunity in Sports

Complex.

Communicate to media and other stakeholders, insurer.

Within 72 hours of the incident, providing information to parents by letter (checked by

lawyers).

RESPONSIBILITIES

Critical Incident Management Team

Manage College activities and responses to identified critical incidents.

Conduct risk assessments of hazards and situations which may require emergency action, analyse

and document the requirements to address these hazards.

Establish liaison with relevant emergency services e.g. police, fire brigade, ambulance, hospital,

poisons information centre, community health services.

Ensure 24 hour access to contact details for all students and their families (for overseas students

this will also include agents, home stay families, carers, consular staff, embassies and interpreting

services if necessary).

Ensure 24 hour access to contact details for all relevant staff members that may be required in

the event of critical incident e.g. legal services, College security.

Regular review of critical incident plans

Implementation of required critical incident plan if required.

Budget allocation for emergencies.

Managing the Media

All enquiries from the media should be directed to the Principal or Marketing & Communications

Manager.

No comments should be made by staff to the media.

No interviews with students should be permitted.

Camera crews should be managed by Marketing & Communications Manager.

No student should be photographed without parental permission.

The Principal should confirm all facts before speaking to the media. If accurate information is not

available or is sensitive, the Principal should explain that questions cannot be answered at this

time.

Avoid implying blame or fault for any part of the incident.

No information to be communicated via Social Media.

SPECIFIC RESPONSIBILITIES

Principal (Critical Incident Director)

The coordinator of the Plan, and the person responsible for the initiation of the plan, is the Principal.

In her absence or as per delegation, the Deputy Principal will assume this role. He/she will have overall

responsibility for coordinating the College’s response to the critical incident.

Initiate and supervise the Crisis Management Plan.

Mobilise the Critical Incident Management Team quickly.

Be visible, available and supportive to all.

Gather factual information on the crisis, thereby dispelling rumours.

Contact parents of students directly involved.

Communicate with College Board.

Arrange for contact with emergency services if required. This may be delegated to the Deputy

Principal after initial contact.

Direct the Marketing & Communications Manager to formulate a statement for staff and students

and Receptionist.

Arrange and chair a meeting of all staff to inform them of the situation and to advise regarding

action to be taken.

Convey information to students about incident at a whole school assembly, by year levels or

individual classes depending upon nature of the incident. (Never announce on Assembly unless

close friends have already been notified.)

Prepare a statement of fact to issue to Pastoral Care Teachers, if needed.

Have legal advice obtained regarding the College’s policies and procedures and adjust

accordingly.

Be the ONLY College contact with the media unless delegated to Marketing & Communications

Manager.

Visit site of incident if appropriate or arrange for the Deputy Principal to visit.

Provide updated information to all concerned.

Deputy Principal

Decide who should receive information and how information will be disseminated to staff and

students. i.e. note to each class, assembly, alarm system or if a lockdown procedure is warranted.

Be aware of privacy and confidentiality in releasing information and remind staff of privacy and

confidentiality issues.

Release follow up information as agreed with the Principal and Marketing & Communications

Manager, following the initial announcement.

Release only necessary details and make sure information is factual.

Notify Bishop’s Office, if necessary.

Include in communication information with regard to when and how students will be released to

parents/caregivers, if applicable.

Maintain College routine if required.

Arrange de-briefing at end of day for staff if required in consultation with the Director of Mission

and College Psychologist.

Marketing & Communications Manager (under direction of CID)

If required initiate a parent’s refreshment and waiting room in Interview Room.

Notify police if the need for extra security requires police knowledge.

If required ensure that personnel are in place to prevent encroachments onto school campus.

Assist in co-ordinating any infrastructure support that may be required. E.g. telephone rooms,

catering, photocopy access.

Provide regular updates to community as required.

Director of Performance, Planning & Analytics

Provide direct assistance to the Deputy Principal.

Develop a plan for emergency coverage of classes.

Assist in briefing supply teachers and visitors to the College as to their requirements.

Check that substitute teachers are in place in classes where Critical Incident Management Team

members were rostered to teach or rostered on duty.

Assist the Director of Boarding in co-ordinating parent and/caregiver communication where

required.

Be prepared to provide direct liaison with members of the Police and Emergency Services if

required.

Director of Students, Director of Mission and Director of Boarding

Coordinate welfare services, assisted by the College Psychologist, Director of Students, Heads of

House, Teachers and qualified outside agencies as required.

Oversee welfare aspects of the Plan.

Chair meeting with College Pastoral Care Team (and community Health representatives if

appropriate) – Mercy Rose Room/Head of House room as venue.

Record ‘at risk’ students nominated by Pastoral Care Team and/or teachers, Pastoral Care

Teachers and Head of House (initially those most directly involved in the incident).

Set up Recovery Room for students – Church.

Send students needing support to recovery room in the Church.

Arrange debriefing at end of the day for Welfare Services Team and arrange subsequent plan.

Determine staffing needs with Pastoral Care accordingly (specifically staff trained in critical

incident stress debriefing and grief and loss counselling).

Contact Community Health where required to inform them of the crisis and if appropriate,

arrange for them to visit the College to discuss needs/assist in management of the situation.

Assist other Community Support Services in arranging long term service provision to individuals

(both students and staff) and families.

Work with Community Health staff in assessing readiness of student involved to return to school.

Support the College staff and students; and offer assistance to parents, if appropriate, after the

Principal has made initial contact.

Contact relevant church support groups/personnel and arrange visit to school as appropriate.

Organise resources such as cardboard, colours etc for students to use if necessary.

College Psychologist

Be available. Cancel other activities and appointments.

Designate a counselling space.

Seek additional assistance if warranted through reciprocal arrangements.

Provide counselling where required or referral to other external agencies.

Support the school staff and students; and offer assistance to parents, if appropriate, after the

Principal has made initial contact.

Contact ACCESS support services at Centacare for staff support if needed.

Director of Finance & Facilities

If requested co-ordinate the instigation of a Critical Incident Management Meeting Room.

Develop necessary information sheets: such as incident report forms, sign-in/out forms, lists of

personnel to be notified (including siblings, bus drivers, feeder schools where students may have

friends), de-briefing sheets etc.

Maintain communication with support staff and contractors if applicable, this is to include any

external sporting or co-curricular staff.

Act as coordinator for the acquisition and storage of goods as required, e.g. bed linen for students

etc.

Assist the Principal.

Commence and continue to record actions and events as they develop.

Undertake an immediate and ongoing security assessment to instigate additional security

procedures where required.

support requirements.

THE ROLE OF DIRECTOR OF STUDENTS, HEADS OF HOUSE AND CLASS TEACHERS

A statement of fact may be provided for you to read to students or parents.

Identify students at risk.

Be available to students or refer to the Director of Students or College Psychologist as considered

necessary. Record names of all students so referred.

Record of all students identified during the day to be returned to Deputy Principal.

Talk with other Welfare Team Members and CIMT to consider management of crisis in short and

long term.

Dispel rumours and discourage speculation.

ADVICE FOR DIRECTOR OF STUDENTS, HEADS OF HOUSE AND CLASS TEACHERS

Director of Students, Heads of House and Teachers have the responsibility of reinforcing the Principal’s

‘Statement of Facts’.

Remain calm.

Assess students who may need to be referred to the Welfare Team.

Before you send specific students for de-briefing:

Answer parental enquiries – College is proceeding normally and that students should come and/or

stay.

Thank people offering to help and take a name and contact number if the offer needs to be taken

up.

Other Office Staff

Remain calm.

Dispel rumours and discourage speculation.

Continue with routine duties and prepare to assist as required to handle personal enquiries.

Longer Term

1. Within a week of the incident arrange for a memorial service at the College if students and/or

staff have been killed in the incident:

Encourage close friends and others directly involved in the incident to participate and be

involved in the planning.

Encourage older students to attend funerals.

2. After three to four weeks convene a meeting of parents of students involved in the incident;

Enlist the aid of counselling professionals to encourage parents to express and share their

feelings and concerns about their children’s welfare and to help them understand their

children’s reactions.

3. Conduct a review of this Critical Incident Plan

The Critical Incident Management box (CIM box) is to be maintained in the College safe located in the

office of the Director of Boarding with a duplicate box held with the Director of Finance & Facilities.

If the incident has occurred outside of the College grounds:

Investigate the information received from students, staff, parents or police.

Do not ignore any rumours, notes, letters, emails, messages etc. sent to staff by any student. Immediately investigate and follow up each one as extremely serious.

Seek the affected family’s permission/wishes or give advice on how to advice the school on the incident: o The needs of the family o The impact of misinformation/rumours o What is appropriate to divulge to the school o The needs to other affected parents to support their children o The options of privacy protection of the student and family, ie: not releasing the students

FIRE EVACUATION (Outside of School Hours – Boarding House)

In the case of a fire, a continuous automatic alarm will be activated and all fire doors will close automatically. A whistle will be used in case of power failure to alert all staff.

Supervisors on duty including all casual staff will evacuate their area via the nearest Fire Exit to - Directly over the road from the Boarding House on the grassed area next to the children’s playground. Supervisor Instructions:

1. Stop what you are doing and remain calm.

2. Ask Students to move quietly and to take the nearest fire exit.

3. Check the supervisor’s room (live in staff) in your area.

4. Collect your emergency roll and move to the grassed area directly over the road from the Boarding House to the left of the children’s playground

5. Mark your roll and ensure that all students are present and inform the Deputy Fire Warden (Senior Supervisor)

CYCLONE 8.15am - 3.15pm Need to notify the bus drivers of any changes to scheduled routine for both the day and boarding school regardless of time of cyclone warning. In the event of a cyclone or severe weather regularly updated information will be available on the following:

Social Media Listen to local radio stations (ABC in all regions) plus various local commercial stations for updates; (4TO 102.3 FM; 103.1 FM; 106.3 FM; 4KIG; ABC Radio 630 AM).

The cyclone watch is issued at six hourly intervals if there is a potential cyclone or gale force winds within the next 24-48 hours. If the cyclone watch turns into a cyclone warning it is issued at three hourly intervals for the event in the next 24 hours. When a cyclone is close to landfall the updates are hourly and the Standard Emergency Warning Signal may be activated.

In the days prior to the expected arrival of a cyclone the school will send home to parents a letter detailing the arrangements for the event. Information will be broadcast on the above radio stations from 7:00am to keep schools open or if special circumstances require a particular school to close. Parents should note that if the official position for the College is to remain open, parents can and should use their own judgement as to whether it is safe to send their child/ren on the day. If a decision is made overnight to close schools, this message will be broadcast from 7:00 am on the above radio stations. This decision to close will apply to all Townsville Schools (State and Catholic). Parents who normally provide transport home for their daughters will be advised by radio stations to

collect students from school. Students who normally travel home by bus or ferry will travel as normal

BUT AT ADJUSTED TIMES unless alternative arrangements are advised to the College by parents.

The College is in a Red Evacuation Zone of highest risk of flooding from cyclone/storm tide. In the event of an evacuation follow these preparation steps:

Step 2: Alert all students to take “Go Pack”.

Exit the boarding College via DOOR 2 Fire Exit Level 1

St Teresa’s Dormitory to evacuate first, lock doors following a sweep of the floor

Followed by level 2, sweep floor and lock doors

Last level 1 with a final sweep and lock doors.

Assemble orderly in the Dining Room (Café by the Sea)

Step 3:

Boarding Supervisors will commence roll marking in Year Levels. Once all students are accounted for Supervisors will direct their students to walk to the bus parked at the rear of the College. Students will exit the Boarding House via DOOR 2 Fire Exit level 1 (evacuation will commence with Year 7).

Step 4:

The Director of Boarding (or delegate) will give instructions to the Bus Driver to disembark once rolls

are correct and all students and supervisors are on the bus. (The number of buses required will depend

upon how many students remain at the Boarding House. Some parents will choose to have their

daughter remain with extended family and therefore the number of students remaining could be

minimal.)

Step 5:

When the bus departs the College the Director of Boarding will secure the Boarding School and drive

the boarding school vehicle to the Mater Hospital.

Step 6:

Once the Bus (students and staff) arrive at the Mater Hospital, they will remain in the bus until

instructions are given to move to the Mater Conference Centre.

Step 7:

Once in the Mater Conference Centre instructions will be given to all students and staff by the Director

of Boarding (or delegate) regarding the housekeeping plan, meals and expectations. Boarding

supervisors are to remain with their designated group at all times.

Step 8:

The Director of Boarding will assure students of their safety and give further information and updates

regarding the cyclone when necessary.

Step 9: The Director of Boarding will inform (and keep updated) the Principal that all students and supervisors

have successfully evacuated the Boarding School and are safe.
