Vision

◦ Powerful mental images of what

individuals want to create in the

future.

◦ The compulsion from comparing

mental image of future with today’s

reality is what drives a vision into

practice.

Practice

The act or process of doing

something; performance or action

The actions that are an outcome of the

mental images in one’s life

Health care reform in resource

• Demands a greater focus to equitable

preventive and promotion of health

care

• Demands to develop Mental health

strategies that brings remarkable

change in mental health disparities

within the communities.

Summary of Article

Title: Mental health services-

maintaining strategic direction,” by

McFadyen and Farrington (1998).

Presents a Mental health services

strategy for mental health promotion

and guidelines for health care planner.

Aim

To increase the proportion of

skilled mental health

professional by training and

team building and operating

strategy in the different levels

to accomplish the mental

health services strategy.

Findings

CMHNs (community mental health

nurse) fail to meet the needs of the

mentally ill, because of different levels

of resistors and forces.

Continuing of traditional planning

approaches follow the old method and

failed to establish the innovative

competitive strategic development.

Mental health services Strategic

management was spotted as

addressing the objectives as

Addressing the objectives of strategic

planning and implementation

Move mental health services to a

community focus in support for people with

severe mental illness

Equitable mental health services and

promoting community intervention

strategies

To integrate health professional and

communities

Two classification of change ;

i) Categories of organizational change (from Greenberg & Baron 1993)

Internal

external

ii) Levels of organizational change, classified by degree of change (from Wilson 1992)

Status quo

Expand reproduction

Evolutionary Transition

To integrate the mental health and objectives and a long term commitment process to community building.

Plan the Approach

For strategies and action steps that

will achieve the mutually set vision

and can be monitored

Should be congruent to the

community’s needs and wants

Integration of health care system

should be adopted by mental health

professional and communities

Critique & Analysis

Understanding theoretical basis of

strategic management provide better

understanding of concepts and

consequent actions.

Reason;

• To avoid loss of control and ultimate

failure.

• Agree with the above statement.

Community care for the severely

mentally ill is failing in UK or in the

other world.

Reason;

• Perceived failure in primary care and

interest of Community and Mental

Health Nurses (CMHNs) has shifted

away.

• Partially agreed

(Other health care professional. have

equal role)

An annual or biennial strategic

planning process can be done

routinely .

Reason;

To make decision and action in

response to the change vision, and

scanning is necessary to control

conflict and disagreement

• Agree with above statement

Traditional planning approaches has

failed to establish the strategic

development.

Reason and recommendation;

Useful for while establishing vision for

any organization. To protect from

internal conflict & help to take action

around a competitive strategy.

Agree with above statement

Although, the author has focused on the control of external forces like NHSE, DOH and government but public sector health organization has direct influence from these sectors which is difficult to control and resist.

Therefore, the government and NHS executive should start with strategic developmental planning and implementation from their previous organizational experiences to come up with a vision of what would be a better community based Mental health services intervention project for severely mentally ill that would promote health of community

Recommendation

A vision may start small and grows as

time goes and later matures with

compulsion , into innovative practices.

Providing equitable health services &

promoting community intervention

strategies are major paradigm for

affecting health and addressing health

disparities

Efficient working with the community means understanding their need and involving them in health care delivery system thereby reducing resistance whilst promoting access to care

Strategic planning and implementation provides a remarkable and innovative strategic model to design health services plan, and promote mentally healthy communities.

Knowledge of local people at the site

of health care delivery, measurement

knowledge and an understanding of

how to turn a vision into reality are all

essential bases for a health

professional to positively change a

system of care not just professional

knowledge.

It can be an ideal model for mental

health care delivery system in Nepal

Question?

Could this strategy be effective in our

community? If yes then, how, and

what are the constraints?
