1 OBJECTIVE

This Standard defines the supplementary requirements to Inmetro´s Administrative rule no. 179, of June 16 2009, which specifies Cgcre´s mark, the accreditation symbol and governs their respective use.

2 APPLICATION FIELD

This Standard applies to Cgcre, accredited CABs and CABs applying to accreditation and assessors and experts who act on the accreditation processes.

The standard is also applicable to Cgcre, as an Auditor Authentication Body (AAB) of the Aerospace Quality Management System Standards (AQMS) of Americas Aerospace Quality Group (AAQG).

3 RESPONSIBILITY

Cgcre is responsible for the revision and withdrawing of this Standard.

4 REVISION BACKGROUND

4.1 Changes have been made in the requirements regarding the results report of subcontracted laboratories. See clauses 11.4.11 and 11.4.12.

4.2 The clause 11.4.3.1 has been deleted. Thereby, it becomes forbidden to include non-accredited test, calibration or examination results, carried out by the laboratory itself, in certificates, reports and examination reports.

4.3 Examination reports have also been mentioned in all clauses dealing with test reports. 4.4 Clause 11.4.6 now allows the use of marks, established by the regulated body to identify its

own laboratory network, in certificates, reports and examinations.

8.3 Accreditation combined mark Association of the accreditation mark with the IAF arrangement mark, indicating the condition of signatory of the multilateral recognition arrangement for the accreditation activity of product certification bodies and environmental and quality management system bodies. 8.4 Combined mark of the accreditation symbol Association of IAF mark with the accreditation symbol of product certification bodies and environmental and quality management system bodies. 8.5 Advertising Material Each and every material developed for an institutional, product or service campaign. For example: movies, spots, leaflets, folders, books. It is important to highlight that an advertisement is considered as an advertisement material, even if it is not part of an advertisement campaign. 8.6 Promotional material Each and every material used to sustain an advertisement campaign. For example: gifts (pens, diaries, key rings etc.), displays. It is important to highlight that if a material, even if it is a poster, is produced separately for a specific event, it will be considered a promotional material. 9 GENERAL CONDITION The accredited CABs shall meet Inmetro Administrative rule no. 179 which approves the Regulation for use of marks, accreditation symbols, recognition of the conformity with the Good Laboratory Practices (GLP) and Inmetro Identification Labels. 10 FORMAT AND USE OF MARK AND ACCREDITATION SYMBOL 10.1 The accreditation mark and symbol, as well as their dimensions, are defined on the Annex of this Standard. 10.2 The proportions established for the accreditation symbol, described on the Annex of this Standard, shall be kept when reproduced by the CAB. 10.3 The dimensions (width and height) of the accredited CAB mark shall have an equivalent proportion to the accreditation symbol. 10.4 The accreditation symbol and CAB´s mark do not need to be juxtaposed, but they shall be in the same side of the page. 10.4.1 The accredited CAB cannot use the accreditation symbol alone, inducing that Cgcre has performed the conformity assessment service. 10.4.2 If the accredited CAB does not use its own mark or the mark of the organization to which it belongs, it shall use one of the following options: a) corporate or trade name; or b) name of the accredited unit.

11 RULES FOR USING THE MARK, COMBINED MARKS, ACCREDITATION SYMBOL AND REFERENCE TO ACCREDITATION 11.1 General rules 11.1.1 The accreditation mark, as well as the mark combined with other marks from multilateral arrangements, shall be exclusively used by Cgcre. 11.1.2 The use of the accreditation symbol or reference to accreditation does not assign any civil or criminal responsibility, arisen from the activities of accredited CABs, to Cgcre or their collaborators.

11.1.3 Cgcre may provide the Accreditation Symbol in an electronic file, upon formal request by the CAB.

11.1.4 The accreditation symbol, to be used exclusively by accredited CABs, can only be used on: calibration certificates, test reports and examination reports; certificates of reference materials or other documents for non-certified reference materials

(ex.: statements, analysis reports etc.); reports and statements of participation in proficiency tests; management system certificates; product, process, service and personnel certificates; inspection reports and certificates; vehicle safety certificates and certificates of transportation of dangerous products; calibration labels; budgets, advertisement material (such as, advertisements in newspapers and magazines,

videos, trefoils, banners, displays, bands etc.), forms, records and letters, since they are clearly identifying to which conformity assessment activities the CAB is accredited;

walls and façades of accredited CABs; vehicles of accredited CABs; labor uniforms and clothes used during fairs and events held by accredited CABs, and

business cards of the accredited bodies.

11.1.5 The use of the accreditation symbol is mandatory in the documents listed in item 11.1.4, issued by accredited CABs, if they refer to the accreditation.

11.1.6 The accreditation symbols shall not be used on products or their packages.

11.1.6.1 In the case of calibration laboratories, the accreditation symbol can be applied in calibration labels (see item “calibration label”).

11.1.6.2 In the case of reference material producers, the accreditation symbol can be applied in the labels of reference materials (see item “reference material labels”).

11.1.7 The accredited CAB may authorize the legible reproduction of the certificate, report, examination report or statement provided to its client with the accreditation symbol, as a result of the accredited service performed, for the purposes of divulgation on an advertisement material, since the advertisement refers to the accreditation scope.

11.1.8 The accredited CAB shall take due care that no report, certificate, examination report or statement nor any part thereof is used in a misleading manner.

11.1.9 The CAB can only use the accreditation symbol and/or any reference to accreditation after the formalization of the accreditation between Cgcre and the CAB and after the receipt of the symbol from Cgcre, as set forth in this document.

11.1.10 The CAB can only refer to its accredited condition for the services to which the accreditation was granted and are included in the scope of its accreditation, not inducing that it is accredited to services not included on the scope. 11.1.11 The CAB shall meet the requirements established for the use of the accreditation symbol when it refers to the accreditation without using the accreditation symbol. 11.1.12 In the event of complete suspension or withdrawal of the accreditation, the CAB shall promptly interrupt the use of all material containing the accreditation symbol or reference to the accredited condition. 11.1.13 In the event of partial suspension or reduction of accreditation, the body shall promptly interrupt the use of all material containing the accreditation symbol or reference to accreditation concerning the accreditation scope that was suspended or reduced. 11.1.14 The accredited laboratories can mention that they belong to the Brazilian Calibration Network (RBC) and to the Brazilian Network of Testing Laboratories (RBLE). The conditions for use of this statement are the same established for Accreditation Symbol use. 11.2 Rules for using IAF MLA mark combined with the accreditation symbols For its use, in addition to the rules for using the accreditation symbol, the rules below also apply. 11.2.1 The combined marks, shown on the annex, are used exclusively by accredited CABs on accreditation programs in which Cgcre is a signatory of IAF multilateral recognition arrangements. 11.2.2 The IAF MLA mark cannot be used alone, neither by CABs nor by Cgcre. 11.2.3 The IAF-MLA combined mark of the accreditation symbol is provided by Cgcre to the CAB after receiving MOD-Cgcre-023 duly filled and signed by the person in charge for the CAB. Dicor shall keep these records filed. 11.2.4 Cgcre may provide the combined mark of the accreditation symbol in an electronic file, upon formal request by the CAB. 11.2.5 The IAF-MLA combined mark shall be juxtaposed and with a proportional dimension (width and height) as described on the Annex. 11.3 Statements about multilateral recognition arrangements

Statements can be made about multilateral recognition arrangements signed by Cgcre by inserting the following sentences:

For testing, medical testing and calibration laboratories: “Cgcre is signatory to the ILAC Multilateral Recognition Arrangement”;

For testing, medical testing and calibration laboratories: “Cgcre is signatory to the IAAC Multilateral Recognition Arrangement”;

For quality and environmental management systems and product certification bodies: “Cgcre is signatory to the IAF Multilateral Recognition Arrangement”;

Note: The full name of the acronyms ILAC and IAAC may be included.

11.4 In certificates, reports and laboratory examination reports 11.4.1 The use of the accreditation symbol is mandatory in certificates issued by accredited CABs, if they refer to the accreditation. 11.4.2 The accreditation symbol shall be placed on the first page of the report or certificate. When the report or certificate has more than one page, the laboratory can place the following sentences, instead of the accreditation symbol, from the second page on: a) for an independent testing laboratory: “Testing Laboratory accredited by Cgcre according to

ISO/IEC 17025 under number CRL XXX”; b) for a testing laboratory of a factory: “Testing Laboratory accredited by Cgcre according to

ISO/IEC 17025 under number CLF XXX”; c) for medical testing laboratories: “Medical Testing Laboratory accredited by Cgcre according

to ISO 15189 under number CLC XXX”; d) for a calibration laboratory: “Calibration Laboratory accredited by Cgcre according to ISO/IEC

17025 under number CAL XXX”. 11.4.3 In order to use the accreditation symbol in certificates, reports and examination reports, these documents shall contain only: a) accredited calibration or testing results performed by the laboratory itself, respecting the

quantities, ranges, best measurement capabilities, tests, examinations, methods, standards and types of products specified for each accredited service; and

b) calibration or test results performed by a subcontracted laboratory accredited for these tests and calibrations, as established in 11.4.4 and 11.4.5.

Note: As a consequence of this requirement, certificates, reports and examination reports cannot make reference to any other documents comprising non-accredited calibration and test results.

11.4.4 In calibration certificates, test reports or examination reports bearing the accreditation symbol, the results from subcontracted laboratories shall be clearly identified in order to indicate that they have been performed by a subcontracted laboratory. This identification shall clearly show the subcontracted laboratory that performed the service, its accreditation number and the respective accreditation body. 11.4.5 The laboratory cannot issue a calibration certificate, test report or examination report with the accreditation symbol or any reference to accreditation, if all results are from a subcontracted laboratory. (see requirement 11.4.3). 11.4.6 The certificate, report or examination report that bears the accreditation symbol shall be necessarily issued by signatories authorized by the laboratory for the accredited tests and calibrations included on them. 11.4.7 The certificate, report or examination report issued by the laboratory that does not include the accreditation symbol cannot be used or construed as having been issued with an accredited status. The laboratory shall provide to Cgcre and its representatives access to these certificates, reports and examination reports, as well as the correspondent records, in order to check the compliance with the requirements of this document and NIT-Dicla-031. 11.4.8 The calibration certificate, test report or examination report bearing the accreditation symbol can only also contain the following marks: a) marks of the accredited laboratory or of the organization to which the laboratory is linked; b) mark established by accreditation cooperations, due to mutual recognition arrangements

(ILAC, IAAC) to which Cgcre is a signatory, when authorized by Cgcre;

c) mark from the CIPM Agreement, used by laboratories officially designated by Inmetro to be signatories of this agreement;

d) mark established by a governmental regulatory authority to identify its Laboratory Network, when authorized by Cgcre.

Note: By virtue of this regulation, Cgcre authorizes the use of the mark established by the Brazilian Sanitary Surveillance Agency (ANVISA) for the laboratories licensed for its Brazilian Network of Analytical Health Laboratories (REBLAS).

11.4.9 If opinions and interpretations are included in the certificate, report or examination report, based on the results of the services performed, the laboratory shall mention them after all results and preceded by the statement: “The opinions and interpretations expressed below are not part of the accreditation scope of this laboratory”. 11.4.10 When the instrument for calibration is adjusted or repaired and this is mentioned in the calibration certificate, a statement shall be included informing that the adjustment or repair is not part of the laboratory accreditation scope. 11.4.11 In the case of regulated instruments calibration, the laboratory may include the following statement in the calibration certificate: “This calibration does not exempt the instrument from the metrology control established on the metrological regulation.” 11.4.12 The laboratory may include on its certificates, reports or examination reports statements about the compliance with Cgcre´s accreditation requirements such as: a) “This report (or certificate or examination report) meets the accreditation requirements of

Cgcre, which assessed the laboratory competence”; and b) “This certificate meets the accreditation requirements of Cgcre, which assessed the

laboratory competence and proved its traceability to national measurement standards (or to the International System of Units - SI)”.

11.5 In Calibration Labels 11.5.1 Each calibration certificate issued with the accreditation symbol shall correspond to one calibration label. 11.5.2 The label shall be fixed on the calibrated instrument or standard by the laboratory that performed the calibration so that it can be easily seen. The material and retaining method shall not affect the characteristics or the performance of the instrument or standard. 11.5.3 When it is not possible to fix a label on the instrument or standard, due to the size or use, this shall be fixed on its case or box. The label can also be delivered to the client so that it can fix it on the most appropriate place. 11.5.4 The label shall be manufactured so that, during the recalibration, it can be removed and replaced by a new one. 11.5.5 The following information shall be inserted on an indelible manner on the label: accreditation symbol; mark or name of the accredited laboratory; number of the corresponding calibration certificate; calibration date. Note: Other information related to calibration can be included, such as: date of the next calibration,

identification of the instrument or standard, identification mark of the partial calibration.

11.5.6 For instruments that remain calibrated for a long period (e.g. glassware), the calibration label can be replaced by a painting or engraving, since this does not change their features. 11.5.7 The laboratory cannot, in any way, use the calibration label so that, mistakenly, the client understands that it means approval, quality or calibration permanence. 11.6 In certificates of reference materials or other documents for non-certified reference materials 11.6.1 The use of the accreditation symbol is mandatory in reference material certificates or other documents for non-certified reference materials issued by accredited CABs, if they refer to the accreditation. 11.6.2 The accreditation symbol shall be placed on the first page of the certificate, report or statement. When the certificate, report or statement has more than one page, the producer can place the following sentence, instead of the accreditation symbol, from the second page on: “Reference Material Producer accredited by Cgcre, according to ISO Guide 34, under the number PMR XXX.” 11.6.3 In order to use the accreditation symbol in certificates, reports or statements, they shall only mention reference materials for which the RMP is accredited and that have been produced by the RMP itself.

Note: As a consequence of this requirement, certificates, reports and statements cannot make reference to any other documents comprising reference materials for which the producer is not accredited.

11.6.4 In order to use the accreditation symbol in certified reference materials certificates, these shall mention the assigned property values, their related uncertainties and the metrological traceability statement. 11.6.5 The content of the certified reference material certificates shall meet the requirements of ISO Guide 31. If the certificate also shows non-certified values, a clear distinction shall be made between certified and non-certified values. 11.6.6 In order to use the accreditation symbol in reports of analysis or statements of non-certified reference materials, these shall contain information about the material homogeneity and stability and the validity period of the stated information. The analysis or statement reports shall contain information to the user on the proper application of the reference materials and their storage conditions. 11.6.7 In order to use the accreditation symbol in certificates, reports or statements, they shall be issued by signatories authorized by the RMP. 11.6.8 The certificates, reports or statements issued by the RMP that do not include the accreditation symbol cannot be used or construed as having been issued with an accredited status. The RMP shall provide to Cgcre and its representatives access to these certificates, reports or statements, as well as the correspondent records, in order to check the compliance with the requirements of this document and the Regulation for Reference Material Producer Accreditation. 11.6.9 The certificate, report or statement issued by the RMP bearing the accreditation symbol can only also contain the following marks: a) marks of the accredited RMP or of the organization to which the RMP is linked;

b) mark established by accreditation cooperations, due to mutual recognition arrangements (ILAC, IAAC) to which Cgcre is a signatory, when authorized by Cgcre;

c) mark from the CIPM Agreement, used by laboratories officially designated by Inmetro to be signatories of this agreement;

11.6.10 The RMP shall include information in certificates, analysis reports or statements on the use of subcontractors and which activities were performed by them. 11.7 In Reference Material Labels 11.7.1 The label shall identify the material, the producer, the reference material lot and the catalogue number, as well as any other information necessary to allow the material to be univocally identified and referenced to its certificate, analysis report or statement. 11.7.2 Reference material labels shall be fixed to the package and may or may not include the RMP´s accreditation symbol. 11.7.3 When it is not possible to fix the label on the package of the reference material unit, due to its size, the information shall be included in the certificate, analysis report or statement and the label shall direct the user to such information. 11.7.4 The RMP shall not, in any way, use the package label of the reference material unit so that, mistakenly, the client understands that it means approval, quality or reference material validity permanence. 11.8 In Proficiency Testing Reports 11.8.1 The use of the accreditation symbol is mandatory in proficiency testing reports issued by accredited CABs, if they refer to the accreditation. 11.8.2 The accreditation symbol shall be placed on the first page of the report. When the report has more than one page, the PTP can place the following sentence, instead of the accreditation symbol, from the second page on: “Proficiency Testing Provider accredited by Cgcre according to the standard ISO/IEC 17043, under the number PEP XXX”.

11.8.3 In order to use the accreditation symbol in proficiency testing reports, they shall mention the accredited programs performed by the PTP itself.

Note: As a consequence of this requirement, proficiency testing reports cannot make reference to any other documents comprising proficiency testing schemes for which the provider is not accredited.

11.8.4 In order to use the accreditation symbol in proficiency testing reports, they shall be issued by signatories authorized by the PTP. 11.8.5 The proficiency testing report issued by the PTP that does not include the accreditation symbol cannot be used or construed as having been issued with an accredited status. The PTP shall provide to Cgcre and its representatives access to these reports, as well as to the correspondent records, in order to check the compliance with the requirements of this document and the Regulation for Proficiency Testing Provider Accreditation. 11.8.6 The proficiency testing report bearing the accreditation symbol can only also contain the following marks: a) marks of the accredited PTP or of the organization to which the PTP is linked;

b) mark established by accreditation cooperations, due to mutual recognition arrangements (ILAC, IAAC) to which Cgcre is a signatory, when authorized by Cgcre;

c) mark established by a governmental regulatory authority to identify its Provider Network, when authorized by Cgcre.

Note: By virtue of this regulation, Cgcre authorizes the use of the mark established by the Brazilian Sanitary Surveillance Agency (ANVISA) for the providers licensed for its Brazilian Network of Analytical Health Laboratories (REBLAS).

11.8.7 The PTP shall include information in proficiency testing reports on the use of subcontractors and which activities were performed by them. 11.9 In Certificates and Reports of Certification Bodies 11.9.1 The accreditation symbol is exclusively used by the accredited CAB, which may only use it on the certificates that correspond exclusively to the accredited services. 11.9.2 The certificate shall contain the certification body´s name, as established in its accreditation certificate and the company´s name and/or mark to which the body belongs. 11.9.3 In order to use the accreditation symbol in certificates, they shall be issued by signatories authorized by the certification body for the services included in the certificates. 11.9.4 The certificate that does not include the accreditation symbol cannot be used or construed as having been issued by an accredited body. 11.9.5 For the quality management system certification of a laboratory, whether the laboratory is accredited or not, the certification body cannot allow that the laboratory uses its accreditation symbol on reports or calibration certificates or tests or medical testing, because such reports and certificates are deemed, in this context, as products. 11.9.6 The accredited bodies for certification of quality management system, food safety management or environmental management shall not allow that the certified companies refer to the system certification on products and/or product packages, including the accreditation identification. 11.9.7 When the certification body for quality management systems, food safety management or environmental management has more than one accreditation, the accredited certificate may contain the accreditation symbol, always taking into account the respective scope. 11.9.8 The bodies accredited by Cgcre for product certification and quality and environmental management system may include in their certificates a reference to the Recognition Arrangements signed by Cgcre with IAF. The reference is made by using the combined mark, as established in the Annex, and may include the following sentences: a) "Cgcre is signatory to the IAF Multilateral Recognition Arrangement for Quality Management

System" or, in the abbreviated way, "Accredited by IAF MLA signatory for QMS". The certificate issued in English shall follow the IAF ML 2:2011 procedure.

b) "Cgcre is signatory to the IAF Multilateral Recognition Arrangement for Environmental Management System" or, in the abbreviated way, "Accredited by IAF MLA signatory for EMS". The certificate issued in English shall follow the IAF ML 2:2011 procedure.

c) "Cgcre is signatory to the IAF Multilateral Recognition Arrangement for Product Certification" or, in the abbreviated way, "Accredited by IAF MLA signatory for Product Certification". The certificate issued in English shall follow the IAF ML 2:2011 procedure.

11.9.9 In the case of Recognition Arrangements signed by Cgcre with IAAC, only the sentences below may be used: a) "Cgcre is signatory to the IAAC Multilateral Recognition Arrangement for Quality

Management System" or, in the abbreviated way, "Accredited by IAAC MLA signatory for QMS".

b) "Cgcre is signatory to the IAAC Multilateral Recognition Arrangement for Environmental Management System" or, in the abbreviated way, "Accredited by IAAC MLA signatory for EMS".

c) "Cgcre is signatory to the IAAC Multilateral Recognition Arrangement for Product Certification" or, in the abbreviated way, "Accredited by IAAC MLA signatory for Product Certification".

11.10 In Inspection Body Certificates 11.10.1 The accreditation symbol is exclusively used by the accredited CAB, which may only use it on the certificates that correspond exclusively to the accredited services. 11.10.2 The certificate shall contain the inspection body´s name and address, as established in its accreditation certificate and the company´s name and/or mark to which the body belongs. 11.10.3 In order to use the accreditation symbol in certificates, they shall be issued by signatories authorized by the inspection body and approved by Cgcre for the services included in the certificates. 11.10.4 The certificate that does not include the accreditation symbol cannot be used or construed as having been issued by an accredited inspection body. 11.10.5 For the compulsory services, the inspection bodies shall use on the certificates the accreditation symbol, and, if required, the mark established by the regulatory body in a specific legislation/regulation. 11.11 Advertisement material, forms, records, letters or Web page 11.11.1 The accredited CAB may use the accreditation symbol or refer to the accreditation since the accredited conformity assessment activities are clearly identified. 11.11.2 When the advertisement material has only non-accredited services of CAB, it cannot include the accreditation symbol or refer to the same. 11.11.3 If a certification body has many offices and uses the accreditation symbol or refers to it, it shall clearly identify the offices with the respective accredited scopes. 11.11.4 If an inspection body has many facilities, renders many kinds of services and uses the accreditation symbol or refers to it, it shall clearly identify the facilities and services which are accredited. 11.11.5 If the CAB is accredited to many accreditation types or modalities and it is not possible to include the accreditation symbols on advertisement materials or in the web page, the following sentence may be used instead “Accredited by Cgcre as [include types and modalities], under the identification [include the identification granted by Cgcre]”. 11.11.6 In letters containing the accreditation symbol or referring to the accreditation and that are sent together with reports or certificates including accredited and non-accredited services, a note about this fact shall be included, mentioning which are and which are not accredited.

11.11.7 The forms and records containing the accreditation symbol or referring to accreditation shall include, exclusively, results and/or information regarding the services to which the CAB is accredited. 11.12 Budgets 11.12.1 The budgets containing the accreditation symbol or reference to accreditation and mentioning non-accredited services shall clearly distinguish those services that are accredited and those that are not accredited. They shall also specify for what services the certificates or reports are issued with the accreditation symbol. 11.12.2 When the budget is exclusively related to non-accredited services, it cannot contain the accreditation symbol or refer to accreditation. 12 GENERAL RULES FOR REFERENCE TO ACCREDITATION 12.1 The accredited CAB may refer to its accredited status without using the Accreditation Symbol in printed or audio materials for advertisement, for instance, cars with loudspeakers and audiovisual advertising pieces. This reference shall be made in the following way:

[CAB´s name] accredited by Cgcre for [text contained on the upper part of the symbol] under the number XXX; or

[CAB´s name] accredited by the General Coordination for Accreditation of Inmetro for [text contained on the upper part of the symbol] under the number XXX.

13 Deadline for the accredited CABs to replace the acronym Cgcre/Inmetro The CABs accredited by Cgcre shall replace the acronym Cgcre/Inmetro by Cgcre in all advertisement material, documents, reports, manuals, budgets, advertisings, folders, among other documents that refer to their accredited status, until May 2015.
