3. How to add/edit a site

You need to add the site to one or more packages so you can view it on the tablet:

4. How to edit questions

You must be careful when editing questions as this affects previous audits that

have used this questionnaire. If you are unsure please contact Cypad Support.

5. Analysis of audits

Score Analysis - at the top level this shows the final % score for all of your sites. If a site

is monitored more than once in the package it shows an average score. You can drill down

to see the % score by Question Group.

Question Analysis – this shows the % pass and fail for individual questions in a

questionnaire

Score Trends (repeating) – this shows audit scores on a monthly basis over six months. It

is most relevant to cleaning contractors who monitor and analyse sites on a monthly basis.

Action Analysis – this shows an analysis of actions by site with the number unresolved or

overdue.

Action Trends – this shows a summary of actions by type (priority).

Inspection Analysis – this shows the same as Score Analysis, showing the average score

and combines both questionnaires (primary and secondary) in one spreadsheet.

Inspections by officer – this shows the number of audits carried out per tablet use. Audits

are shown by area for weekly, monthly, quarterly or annual periods.

6. How failed questions affect the monitoring score

The monitoring score for an inspection is calculated as a percentage of the maximum

possible score. Individual scores are added up for each question in the Question Group and

Question Sub-Groups, taking account of any weightings. Each Question Group can have a

validation type of Not Required or Answer All.

The monitoring score is affected by the critical question option your select from within

the Edit Question Details box for each question, and by the weighting you give a question.

Normal

A normal question with no special attributes.

Critical

If you fail this question it scores zero. The Question Group and Question Sub-Group –

defined by a header-type question - are unaffected. The inspection is highlighted on the

desktop and the critical question on the report.

You can also affect the monitoring score when you set up or edit a Package. Go to Monitor

> Packages > [choose package] > Click to Edit Package

Lower the overall score by x percent for each failed critical question

Reduce the inspection failure limit by x percent

Compulsory

You must answer this question to sign-off the audit – if you fail, the entire Question Group

is failed. If it is in a Question Sub-Group, the sub-group is failed and its scores are set to

zero (questions outside the Question Sub-Group are not affected).

Red Flag

If you fail this question, the Question Group is failed. If it is in a Question Sub-Group, the

sub-group is failed and its scores are set to zero (the main Question Group is not

affected).

At Risk

If you fail this question you put the unit/site at risk of prosecution. Choose to fail either as

Red Flag or as At Risk - the entire group fails and immediate action must be taken.

Weighting 1

A weighting used in the calculation of a score for a particular inspection. The default

value is 1, enter zero if you don’t want the question to count towards the score

Where you are comparing scores with an external organisation enter their alternative

weighting here
