WARRANTY

Product shall be free from defects in material and workmanship for two (2) years.Gel Pads (including Vacu-Gel™, Vacu-Form™ and FlexiForm™), Foam Positioners,OR Table Surfaces, Armboard Surfaces and Stretcher Surfaces shall be free fromdefects in material and workmanship for one (1) year. Contact Customer Serviceor your Sales Representative for additional warranty information.

RETURNS

The guidelines below must be followed in order to initiate a return for credit:

• A Return Material Authorization (RMA) # must be obtained within thirty(30) days of invoice date by calling our Customer Service Department(877) 458-4884.

• An RMA number is valid for fourteen (14) working days. Productreceived beyond this fourteen (14) day period will be refused.

• Product must be in new, unmarked, saleable condition. We will issue fullcredit following inspection of product. Product not in new condition issubject to a 20% restocking fee.

• Gel Pads (including Vacu-Gel™, Vacu-Form™ and FlexiForm™), FoamPositioners, OR Table Surfaces, Armboard Surfaces and Stretcher Surfacesreturned within thirty (30) days of the invoice date are subject to a 20%restocking fee.

• Shipment damages should be reported to Customer Service Departmentwithin fourteen (14) days of invoice date.

• Custom product is not returnable.
