REQUIREMENTS FOR CREDIT• Program attendees can earn Continuing Pharmacy Education credits for this program by logging into

learning assessment/evaluation form.

• Participants have 60 days to complete evaluations. • After 60 days from April 23, 2019, no credit will be available for this program.• Credits will be electronically transferred to the CPE Monitor System. • No Statements of Credit will be issued.• Program participants can earn 5.25 contact hours of continuing pharmacy education credits.• This program is not accredited for pharmacy technicians.
