Berbagai jenis sitokin diteliti terkait dengan asfiksia. Peningkatan sitokin berperan penting
dalam patogenesis jejas otak akibat hipoksia iskemik pada hewan coba dan neonatus.yang
menginduksi respon inflamasi yang berlebih. Penelitian yang dilakukan Okazaki dkk,
menentukan konsentrasi berbagai sitokin dalam serum dengan menggunakan metode
fluoresensi microsphere yang sangat sensitive. Peneliti mengukur konsentrasi 8 jenis sitokin
dalam serum yang diperoleh dari 17 kasus asfiksia, 10 neonatus normal, dan 6 orang dewasa
yang sehat. Hasil Konsentrasi serum IL-6, IL-8, dan IL-10 dalam asfiksia neonatus lebih
tinggi dibandingkan pada neonatus normal. Terlepas dari ada atau tidak adanya asfiksia
terjadi peningkatan juga pada konsentrasi serum IL-2, IL-4, IFN-gamma, dan TNF-alpha
lebih tinggi pada neonatus dibandingkan pada orang dewasa. Konsentrasi IFN-gamma pada
neonatus dengan asfiksia lebih rendah dibanding pada neonatus normal. Tingkat serum IL-10
lebih tinggi pada asfiksia neonatus dibandingkan pada neonatus normal. Tingkat serum IL-6,
IL-8, dan IL-10 pada asfiksia neonatus dengan prognosis yang buruk atau bahkan
meninggal bila dibandingkan dengan yang prognosisnya tidak buruk. Kesimpulannya
konsentrasi serum berbagai jenis sitokin berbeda pada neonatus, beberapa dari mereka dapat
meningkat lebih tinggi dibandingkan sitokin yang lain selama asfiksia. IL-10 meningkat
segera pada asfiksia neonatus dan berhubungan dengan fungsi neuroproktektif.

Asfiksia dapat menyebabkan jejas pada substansia alba (white matter injury) pada otak
yang menyebabkan gangguan perkembangan dan disabilitas jangka panjang. Kondisi ini
menyebabkan kerusakan otak pada bayi yang bersifat ireversibel. sehingga diperlukan
prediktor diagnosis yang spesifik untuk mengindentifikasi asfiksia neonatorum. Penelitian
eksperimental menunjukan bahwa mediator inflamasi berperan penting dalam patogenesis
jejas otak hipoksia iskemik pada hewan coba dan bayi yang mengalami asfiksia neonatus.7,8,9
Saat ini dikenal sekitar 200 jenis sitokin yang berbeda pada manusia. Sitokin ada yang
bersifat proinflamasi dan anti inflamasi, bersifat neuroprotektif dan destruktif tergantung dari
keberadaan dan konsentrasinya.11Hampir semua sel berinti menghasilkan sitokin, terutama
diproduksi oleh sel-sel glia di sistem saraf pusat (SSP) atau oleh sistem imun seperti sel T-
helper dan makrofag. Rangsangan yang menstimulasi produksi sitokin dapat berasal dari SSP
atau jauh dari SSP. Aksi sitokin dengan mengikat reseptor permukaan sel spesifik dan
menginduksi mekanisme pensinyalan intraseluler dengan menaikkan atau menurunkan
regulasi faktor-faktor transkripsi sehingga memicu reaksi pro inflamasi atau anti inflamasi12
Sitokin-sitokin anti inflamasi seperti IL-8 dan IL-10 akan meningkat selama fase akut
kerusakan setelah terjadinya hipoksia iskemik pada 1-12 jam pertama kehidupan neonatus.
Sitokin anti inflamasi merupakan serangkaian molekul immunoregulator yang mengontrol
respon sitokin proinflamasi dan mengurangi jejas dari reaksi inflamasi yang berlebihan.
Semua sitokin anti inflamasi memiliki beberapa sifat proinflamasi. Efek murni dari sitokin
tergantung pada waktu pelepasan sitokin, lingkungan lokal dimana sitokin bekerja,
keberadaan elemen kompetitor atau sinergis, kepadatan reseptor sitokin, dan respon jaringan
terhadap setiap sitokin. 7,8,9 Kok semua daftar pustaka nomor 7,8,9 ????
Clarkson dkk menemukan bahwa kosentrasi IL-8 pada cairan serebrospinal dan
serum dapat meningkat pada bayi baru lahir yang mengalami asfiksia bila dibandingkan
dengan kontrol. Terdapat hubungan yang signifikan antara keparahan hipoksik iskemik
ensefalopati (HIE) dan konsentrasi IL-8.Derajat beratnya tergantung dari luasnya kerusakan,
ketidakseimbangan metabolik selama periode reoksigenasi, dan status perkembangan dari
area yang terkena. Fungsi kemoatraktan neutrofil dari IL-8 berperan penting dalam
perkembangan awal kerusakan saraf yang diinduksi hipoksia iskemia. 7,8,9 Kok semua daftar

Okazaki dkk melakukan penelitian berbagai sitokin dengan menggunakan metode
fluoresensi microsphere. Membandingkan peningkatan konsentrasi 8 jenis sitokin dalam
serum yang diperoleh dari 17 kasus asfiksia neonatus, 10 kasus neonatus normal, dan 6 orang
dewasa yang sehat. Kosentrasi serum IL-6, IL-8, dan IL-10 pada asfiksia neonatus lebih
tinggi dibandingkan neonatus normal dan orang dewasa. Daftar pustaka nomor berapa ini ???
Terlepas dari ada atau tidak adanya asfiksia neonatus terjadi juga peningkatan pada
konsentrasi serum IL-2, IL-4, IFN-gamma, dan TNF-alpha dibandingkan pada orang dewasa.
Konsentrasi IFN-gamma pada neonatus dengan asfiksia lebih rendah dibanding pada
neonatus normal. Kesimpulan penilitian ini konsentrasi serum berbagai jenis sitokin berbeda
pada neonatus, beberapa dari mereka dapat meningkat lebih tinggi dibandingkan sitokin yang
lain selama asfiksia. IL-10 meningkat lebih cepat pada asfiksia neonatus dibandingkan sitokin
lainnya. IL-10 berfungsi sebagai neuroproktektif dengan mengurangi jejas inflamasi pada
substansia alba.11
McAdams dkk, menemukan adanya hubungan antara peningkatan IL-8 yang tinggi
dengan jejas substansia alba (WMI) pada bayi preterm dan sepsis pada 84 bayi dengan berat
lahir rendah, sebanyak 27 (32%) dengan WMI dan 57 (68%) tanpa WMI sebagai kelompok
kontrol. Hasilnya WMI meningkat pada bayi dengan klinis sepsis early onset dengan kadar
IL-8 > 100 pg/ml memiliki sensitivitas 96%, spesivitas 83% dan negative predictive value
98%. IL-8 dapat merupakan prediktor utama untuk WMI. Walaupun peningkatan kadar
konsentrasi IL-8 dalam cairan serebrospinal dikaitkan dengan WMI, namun konsentrasi
sitokin plasma tidak menggambarkan kadar sitokin dalam cairan serebrospinal atau rangkaian
inflamasi di otak.9 Alinea ini sebaiknya di Tinjauan Pustaka tidak di Pendahuluan.
Berbagai penelitian eksperimental yang telah dilakukan pada hewan coba dan bayi
dengan asfiksia menunjukan kosentrasi IL-10 dalam serum dan cairan serebrospinal lebih
tinggi pada bayi dengan ensefalopati hipoksik-iskemik (HIE) sedang sampai berat
dibandingkan bayi dengan HIE ringan. Sistem imun bayi memiliki kemampuan untuk
mengekspresikan peningkatan kadar IL-8, IL-10, IP-10, dan MIP-1α serum untuk
menginduksi respon inflamasi selama asfiksia perinatal dan infeksi perinatal atau
nosokomial. 7,8,16,17. Ini juga kok digabung ???? karena belumtentu statemennya sama.
Pemeriksaan anti inflamasi ini dapat digunakan sebagai prediktor diagnostik spesifik
dan prognostik sehingga diharapkan dapat meningkatkan kemampuan untuk mendiagnosis,
melakukan monitoring, pencegahan dan kuratif pada bayi asfiksia berupa manajemen
resursitasi dengan benar Daftar pustaka nomor berapa ini ???
B.1. Patofisiologi
Sepsis merupakan akibat interaksi yang kompleks antara mikroorganisme patogen dan
pejamu. Pada sepsis terjadi keadaan hiperinflamasi yang melibatkan beberapa komponen,
antara lain bakteri, sitokin, komplemen, sel netrofil, sel endotel dan mediator lipid. Beberapa
mekanisme yang memegan peranan penting dalam patofisiologi sepsis adalah inflamasi,
koagulasi dan gangguan fibrinolisis.34
Produk-produk bakteri serta berbagai jenis protein kuman gram positif , bertindak sebagai
antigen yang akan memicu respon innate antara lain monosit, makrofag dan sel
polimorfonuklear. Proses molekuler yang muncul sebagai respons terhadap sepsis tergantung
pada mikroorganisme penyebab namun tahapan respons yan gmuncul pada sepsis adalah
sama dan tidak tergantung pada mikroorganisme penyebab. Respon inflamasi sistemik
muncul akibat pelepasan sitokin proinflamasi, molekul adhesi, mediator vasoaktif, dan
terbentuknya reactive oxygen species (ROS).i Makrofag dan sel mononuklear yang
teraktivasi melepas sitokin proinflamasi sebagai regulator reaksi tubuh terhadap infeksi,
inflamasi atau trauma terutama TNF-α dan IL-1 dan selanjutnya terjadi stimulasi produksi IL-
6, IL-8, IL-10 yang menyebabkan keradangan lokal dan dapat memperburuk keadaan,
menimbulkan demam, proses inflamasi, dan destruksi jaringan. Pelepasan sitokin
proinflamasi akan menyebabkan lepasnya berbagai mediator sekunder seperti mediator
vasoaktif dan SOR oleh sel-sel monosit, neutrofil dan sel endotel vaskular yang mengawali
terjadinya serangkaian proses imunoinflamasi. Sitokin proinflamasi mengaktivasi jalur klasik
dan alternatif sistem komplemen yang merupakan komponen utama innate immunity.
Meskipun demikian bila terjadi aktivasi berlebih akan menyebabkan kerusakan endotel,
selanjutnya dapat menimbulkan syok septik, disfungsi organ dan kematian. Sitokin
antiinflamsi seperti IL-Ira, IL-4, IL-10, dan IL-12 berfungsi menekan infeksi dan
mempertahankan homeostasis dengan menghambat produksi sitokin dari leukosit. Produksi
sitokin pada bayi belum sempurna, sehingga respon inflamasi dapat terganggu. Hilangnya
keseimbangan antara sitokin proinflamasi dan antiinflamasi dalam tubuh mendasari
terjadinya sepsis yang mengakibatkan peningkatan pembentukan ROS.38
Protein komplemen yang dihasilkan oleh makrofag pada umumnya berada dalam keadaan
inaktif dan akan diaktifkan oleh suatu kaskade inflamasi oleh kompleks imun, yang disebut
jalur klasik dan oleh bakteri yang disebut jalur alternatif menjadi komplemen aktif. Aktivasi
komplemen C5 sampai C9 akan menyebabkan terjadinya cedera membran berupa lisis
eritrosit, kebocoran membran plasma dari sel berinti dan lisis bakteri gram negatif yang
disebut dengan kompleks membran litik. Lekosit PMN merupakan salah satu mediator selular
utama pada kerusakan jaringan dengan cara berdegranulasi dan selanjutnya menghasilkan
protease dan ROS yang memproduksi Oxygen Free Radical dan Hydroxyl radical. Jaringan
atau organ tubuh yang sensitif terhadap ROS antara lain : bakteri, eritrosit, lekosit, limfosit,
fibroblas, sel tumor, endotel serta mitokondria.38
Ada 3 mekanisme terjadinya infeksi neonatus yaitu saat bayi dalam kandungan / pranatal,
saat persalinan / intranatal, atau setelah lahir / pascanatal. Paparan infeksi pranatal terjadi
secara hematogen dari ibu yang menderita penyakit tertentu, antara lain infeksi virus atau
parasit seperti Toxoplasma, Rubella, Cytomegalovirus, Herpes (infeksi TORCH),
ditansmisikan secara hematogen melewati plasental ke fetus. Infeksi transplasenta dapat
terjadi setiap waktu selama kehamilan. Infeksi dapat menyebabkan aborsi spontan lahir mati,
penyakit akut selama masa neonatal atau infeksi persisten dengan gejala sisa. Infeksi bakteri
lebih sering di dapat saat intranatal atau pascanatal.ii
Selama dalam kandungan janin terlindung dari bakteri ibu karena adanya cairan dan lapisan
amnion. Bila terjadi kerusakan lapisan amnion, janin berisiko menderita infeksi melalui
amnionitis.
Neonatus terinfeksi saat persalinan dapat disebabkan oleh aspirasi cairan amnion yang
mengandung lekosit maternal dan debris selulermikroorganisme, berakibat pneumonia.
Paparan bayi terhadap bakteri terjadi pertama kali saat ketuban pecah atau dapat pula saat
bayi melalui jalan lahir. Pada saat ketuban pecah, bakteri dari vagina akan menjalar ke atas
sehingga kemungkinan infeksi dapat terjadi pada janin (infeksi transmisi vertikal).35
Paparan infeksi yang terjadi saat kehamilan, proses persalinan dimasukkan ke dalam
kelompok infeksi paparan dini (early onset of neonatal sepsis) dengan gejala klinis sepsis,
terlihat dalam 3-7 hari pertama setelah lahir.
Infeksi yang terjadi setelah proses kelahiran biasanya berasal dari lingkungan sekitarnya.
Bakteri masuk ke dalam tubuh melalui udara pernapasan, saluran cerna, atau melalui kulit
yang terinfeksi. Bentuk sepsis semacam ini dikenal dengan sepsis paparan lambat. Selain
perbedaan dalam waktu paparan kuman, kedua bentuk infeksi ini sering berbeda dalam jenis
kuman penyebab infeksi. Walaupun demikian patogenesis, gejala klinik, dan tata laksana dari
kedua bentuk sepsis tersebut tidak banyak berbeda.39
