About the Qualtrics Survey• Qualtrics randomly assigns a randomized identification number to
each respondent which provides anonymity and confidentiality.• To further protect anonymity and confidentiality, we did not do 3-
way analyses with age, gender, and race/ethnicity.• We were told that the Qualtrics Survey was distributed to 315 Full
Time DCBOH employees—as it turns out, some part time employees also completed the Survey.
• The response rate was approximately 44%.• A $75 incentive was offered to the unit with the highest response
rate above 80%--we were not able to identify if any single unit achieved 80+%.
• Instead, we are gifting $150 to the Employee Wellness Committee in support of the Program.
• We will present information about areas of:• LOW CONCERN• HIGH CONCERN

health and nutritionist services (or the services of their own County if not DeKalb) to help employees cope with stress, anxiety, sleeplessness, support issues, or diet, or refer them to private providers if covered by their insurance plan.
2. Provide training for supervisors about employee stress, anxiety, and poor sleep, and how they can (a) be supportive of their staff and (b) refer staff members for help.
3. Promote and provide incentives to increase vaccination rates, as well as the use of other preventive services (ex., screenings).
4. Reinforce that ALL Centers/Units have the authority to plan, promote, organize, implement, and evaluate their own EWP activities / offerings.
5. Each Center/Unit should have an EWP Team comprised of 2-5 persons who will have responsibility for assessing priorities based on staff input, and planning, organizing, promoting, and orchestrating specific agreed upon EWP activities. One member of the Team will represent the Team as the member of the DCBOH Employee Wellness Committee.

sought by employees.
7. Center/Unit and Employee Wellness Committee should promote friendly competitions within and between Centers/Units, including DCBOH-wide recognition for “winners”, “high achievers”, and those “making substantial progress” as Teams and individuals. Incentives and prizes are also valued.
8. The Employee Wellness Committee should arrange for or provide Leadership Training and Assistance in support of Centers’/Units’ EWP Team efforts as requested: assessment of needs and priorities, and the planning, promotion, organization, coordination, & evaluation of activities.
9. Human Resources should provide “EWP Orientation” for all new employees no sooner than 2 months and no later than 3 months after they start working with the DCBOH. This should be customized to review Wellness activities and services covered by their insurance plan.
10.Human Resources should provide an formal “EWP Update” each year before the “Insurance Policy Open Period” to each Center/Unit for existing staff to let them know what is available from the DCBOH and community partners, and what is covered by each insurance policy.

11. DCBOH employees who provide WIC services should be able to access Wellness Break time and activities. Try block scheduling so that a block of time can be reserved for Wellness activities on a daily basis.
12.A Wellness Break Policy for part time employees needs to be communicate to all supervisors and employees. Example: persons working 4 hours per day can take a 15-minute Wellness Break.
13.At least 75% or more of all Wellness Break Requests should be approved by all supervisors. To this end, HR should develop an electronic Wellness Break Request System that (a) employees use to request approval from their supervisor for two 15-minute or 1-30 minute break, and that (b) supervisors use to approve/disapprove requests. This metric should be included in the Annual Review of supervisors.
14.Seek State of Georgia financial support and donations from non-profit and for-profit organizations in support of the EWP Program.
