WESTERN LANE AMBULANCE DISTRICT REGULAR BOARD MEETING

November 30, 2017

DIRECTORS ABSENT:

OTHERS PRESENT: Matt House, Jim Langborg, Karin Guy, Jim Dickerson, Ali Gharib, Zach Werner, Charlie York, Darrek Mullins, Rob Chance, Ben Taylor, Danielle Hanson, John Harder, David Rossi, Ronnie Pearson, Zach Werner, and Mark Brennan with the Siuslaw News.

PUBLIC COMMENT: None.

NEW BUSINESS: Director Farnsworth discussed the Board Policy Review whereby Operations Chief, Chief Director & staff can request assistance from Board members who have skills to assist with various tasks, e.g., analyzing data. A standard of governance which can appropriately respond to requests for accounting or financial analysis, contract negotiations. Director Webb noted a policy re-write will be conducted soon. Chief Director Langborg suggested working at both Board policies and create identical policies between the two Boards. Director Farnsworth also suggested for that there be better & timely communication to all Board members. That the communication comes from the Chief Director to Board President Webb who then can communicate with the other Board Directors. Director Webb suggested Chief Director go to the Board to avoid creating a public meeting. Chief Director Langborg offered to provide a weekly update. Director Webb suggested he sent to all Board Directors but remove the ability to “reply to all” so as not to create a Board meeting.

APPROVAL OF MINUTES: Director Farnsworth made a Motion to approve the minutes as written for October 26, 2017. Director Russell seconded the Motion. Roll Call with Motion passing unanimously.

FINANCE/HR REPORT: Ross Schultz, Interim Admin Manager, who arrived Tuesday afternoon, has just begun reviewing the finances. The October financials did not include the payroll but he was able to determine that the total payroll liability to be approximately $128,000 which one is added in, shows that we are within budget on payroll. Other departments and sub sections are also well within budget. Director Yecny requested that the revised October statement be provided prior to the next Board meeting. Ross will provide it next week. Director Webb requested that the EFTS also be reported on the Transaction Report, but the Deposits do not need to be shown. He also requested that the billing system provide information on the A/R trending, and would like an A/R Aging Report. Director Yecny also asked for the A/R Aging trends. Director Yecny noted that no checks were issued during one week in October which coincided with Karin Guy’s five day absence for knee surgery.

TRANSACTION REPORT: Director Yecny made a Motion to approve the transaction list. Director Farnsworth abstained since he is a Vendor. Director Russell seconded the Motion. Roll Call with Motion passing unanimously.

CHIEF/DIRECTOR’S REPORT: Chief Director Langborg discussed the following: 1. Strategic Plan notes are in progress of being developed 2. Seismic Grant: met with architect last week, expect to start in January at WLAD facility. 3. Administrative practices review involved several consultants, some who have already provided

reports. As a result, several recommendations such as how we document and how we process financial procedures will be implement by the interim manager, Ross. Chief Director also asked Ross to provide suggestions for improvement. Ross has met with Admin Team to re-delegate duties, and the plan will be presented at this evening Advisory Committee meeting.

4. Citizen complaint via the police department which resulted in the Chief Director meeting with the Police Chief and discussing the issues with payroll and taking appropriate steps. Director Farnsworth inquired if the complaint was substantiated. Chief Director stated that no current or former employee is being investigated, nothing was done unethically illegally nor intentionally. Chief Director has as that a timekeeping process be implemented by January 1, 2018 for both Districts.

General discussion where Director Yecny asked if Chris Mahr’s audit is complete and if any issues were found. Chief Director stated approximately 90% complete, and no issues were found in either District. Director Webb wanted to ensure that Ross has no restraints and has a lot of freedom to create processes. Chief Director replied that they are closely communicating to ensure they are on the same page, and Chief Director is actively engaged in the process. Director Webb wanted to ensure that the processes that need to happen are happening.

OPERATIONS MANAGER: Matt House discussed the following topics: 1. Call volume was up by 52 for October, but annual total is down by 200. 2. ImageTrend was discussed as how the dispatch calls create a patient record 3. Darrek Mullins will be implementing OperativeIQ on January 1, 2018 which is the inventory

program. 4. MIH Chris Martin has started working. His workflow is being established by the ED Resource person.

The three initial metrics are: decrease of re-admits within 30 days; decrease in non-transport calls, and decrease in Emergency room visits.

5. EMT Basic training: the State exam is on December 10, 2017, the training started with 17 people and now have 16.

CORRESPONDENCE: Chief Director Langborg mentioned the annual movie night which is open to Directors. Matt House reminded the Directors about the awards banquet dinner on December 8, 2017.

FUTURE BUSINESS: Board confirmed the next Board meetings to be held on December 28, 2017 at Noon, both held at SVFR Station 1, with lunch provided.

DIRECTOR COMMENTS: Director Farnsworth requested that there is PERS training. Chief Director replied that there is a plan to do the training this year. Director Yecny inquired about the bid info in the Board packet, and Matt House clarified that it is for replacing the carpet with linoleum, and the bids came under budget. No other comments from the Directors.

JOB SUMMARY Under the direction of the Chief / Director, directs, manages, supervises, and coordinates all finance, human resource, and general administrative support activities of both Districts.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned as required;

1. Maintains detail knowledge of all accounting information and software for the two

districts and the consolidation of them in accordance with the IGA.

2. Supervises all administrative staff in accordance with District Policy and procedures.

3. Maintains a cheerful public image. Works to make the Admin Department responsive to

District Staff, Districts Boards and Citizens.

4. Is the Administration Representative at all board meetings and other related functions.

5. Tracks and manages all district Cash. Reports to management and the Board on the

Districts financial status of both Districts.

6. Responsible for the creation and management of both District’s budget.

7. Ensures the general administrative support needs of both Districts are met.

SUPERVISION: Works under the direction of the Chief / Director. Supervises up to 5 FTE’s, professional and part time. May also manage some contract employees as needed.

EDUCATION and/or EXPERIENCE: Four-year degree in business, accounting or finance from a regionally or nationally accredited university is highly desired. Four years similar work experience is highly desired. Any combination of education, work experience, or training that demonstrates the ability to successfully fill the needs of this position.

PHYSICAL DEMANDS & WORK ENVIRONMENT: The physical demands and work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Employee may be required to lift up to 25lbs to perform the job duties.

Requirements: Candidate must sign a contract with the fire district agreeing to be a non-tobacco user. Candidate must have a current Oregon Driver's License and be insurable by the Fire District. Candidate must agree to and pass a District drug test.

Board Portal Software

We have completed a test run for this month and hope to have full access to Board

Members by January. I will keep you posted as we make progress on this project.

California Strike Teams

Both crews have returned from California. They are in good spirits and good health. The

experienced they gained was invaluable. Matt and I have discussed finding ways to get

our Medic Crews involved in out of District mobilizations to create opportunities for them

to gain experience working at large incidents.

Time Keeping Software and Other Payroll Administrative Updates

We continue to work on implementing the new time keeping software and creating a

long-term plan to implement the recommendations made by the consultants.

Employee Resignations

Julie Brown resigned in late November. We are currently filling the Admin position with

an Interim Office Administrator and working on plans to fill the position.

EMT Class

The EMT Class has wrapped up and the students are waiting to take their National

Registry Written Exam. I will keep you posted, as they become certified.

District Activities and Social Events

Both Districts have had multiple holiday related events to include Christmas Parties and

a holiday movie that was well attended by members of SVFR, FPD and the USCG. On

December 12th the District sponsored a Toys for Tots event at Station-1. The event was

held in conjunction with the US Marines, USCG, Florence PD, and Western Lane

Ambulance. Over 230 children received toys. Boa was the lead on this project and did

great job. On December 16th, the SVFR assisted the Soroptimists with their annual

holiday food basket and toy give away.

Board Policy Review

The SVFR Board has been briefed on the Ambulance District Board’s desire to review

and update their Board of Directors policies. They were also informed that Staff

recommends this would be a good time to review and update the Fire Board’s policies’

and create a similar set of policies. Staff also recommended that each Board appoint

two members to an ad hoc committee for the purpose of updating the policies. I

recommend making these appointments in January and create a committee meeting

schedule.

December 2017 Chaplain Program Coordinator’s Report Prepared by Lori Severance, LCSW

Chaplain Program Update:

In the past month, the following tasks have been completed for the SVFR Chaplain Program:

- A Volunteer Chaplain Task Book has been developed to accompany the Chaplain Training

Manual that was completed last month.

- A booklet entitled “First Responder Resilience and Health” has been developed to address the

issue of first responder stress. This booklet has been distributed to WLAD and Florence Police

Department as well as SVFR.

- A meeting is scheduled for Friday, December 8 with Sgt. Greg Rice of Lane County Sheriff’s

Department and Tony Miller (SVFR Fire Fighter) to discuss Lane County Sheriff’s Department’s

very successful Peer Support Program. Ideas regarding what works and what doesn’t work will

be brought back for discussion with WLAD and Florence Police Department representatives, as

we continue attempts to set up a tri-agency Peer Support program for first responders in

Florence and outlying areas.

- Several recruitment activities have been conducted this month in an attempt to find new

volunteer chaplains (to be interviewed after the holidays in January 2018), due to the

resignation of one of our four chaplains.

December 2017 Training Report

Target Solutions topics for the month are Hematology and Fleet Program Legal Considerations for

Emergency Vehicle Operators

QRT this month was Pediatrics, Obstetrics, and Gynecology. I have completed the curriculum and

schedule for QRT classes in 2018.

I have completed PALS instructor criteria and now can begin recertifying others. Darrek and I are

qualified to instruct PALS and ACLS. I recertified George, Ben, Aaron, Tom, and Doug on 12/17 in ACLS.

The EMT class has come to an end. We began with seventeen, fifteen of them were cleared to take

their state practical/psychomotor exam, with fourteen of them doing so. Overall, the class did not

perform as well as we hoped for with only eight passing. Three of the six that failed need to retest in

one station while the others need a full retest. One person that needs a total retest barely made

minimum attendance requirements after having to make up several hours. The other two were

completely unexpected.

We will be starting a formal staff training process on the first of the month, most will be hands-on but I

have been putting some Power Point presentations together to supplement.

Chris has been doing some training with the crew, as well as going on some 911 calls. I asked him to

review our protocols as a priority for getting on the units, as well as familiarization with kits/equipment

and SOGs/PPs. He’s been really great and helpful.

Darrek has been training the crews on Operative IQ for truck checks going live next month.

Ms Hoffman was out on a hike today with her husband when she collapsed with left sided paralysis and right-sided gaze deviation.She was life flighted from the field directly to Riverbend Hospital where she was found to have right MCA syndrome. Her NIH strokescale was 19. There was no family accompanying her and there was no contact information for family, so two physician consent wasobtained for IV TPA and thrombectomy. Her noncontrast head CT demonstrated ASPECTS score 10 with a hyperdense right MCA.CT angiogram of the head and neck showed occlusion of the right carotid terminus and proximal right MCA. She had moderatecollaterals on delayed phase imaging. She was taken to cath lab emergently after IV TPA was started. Thrombectomy resulted incomplete recanalization of her right internal carotid and right middle cerebral arteries. She had the return of movement in her left armand leg on the table During hospitalization, she had new onset of AF. This is felt to be the cause of her stroke.
