Medical Plan Changes • No increase in Medical Plan premiums or plan design changes!

• Addition of an expanded list of women’s preventive care services with no cost-share. The plan will restrict coverage to in-network providers and generic drugs only.

• Health Savings Account participants can contribute more to their

account:

• Individual Coverage: $3,250 (up from $3,100)

• Family Coverage: $6,450 (up from $6,250)

• Reminder to Healthy Rewards participants - your Health

Reimbursement Arrangement (HRA) has no maximum roll-over

limit

New Pharmacy Vendor - Aetna • No changes to current plan design

• Access to wide network of retail pharmacies. To find a pharmacy near you go to www.aetnanavigator.com.

• You will receive a new Aetna medical ID card in December which contains the pharmacy group information. Be sure to present your new Aetna ID card beginning January 1, 2013.

• Aetna Voluntary Mail Service - You can order refills online, by phone or by mail.

• Most Open retail and mail-order prescriptions will be honored by Aetna.

• Open prescriptions for some medications, including narcotics, will require a new prescription beginning January 1, 2013.

• If you are utilizing the Specialty Pharmacy you will receive an out reach call from Aetna Pharmacy Management.

Teladoc New Wellness Tool

What is Teladoc?

• A national network of board-certified physicians who specialize in internal

medicine, pediatrics, family medicine and emergency medicine and who provide

quality healthcare through the convenience of phone or online video

consultations. (video conference not available in all states)

• Available to Willis Associates and their dependents enrolled in the medical plan (due to state law this service is not available in Oklahoma)

• Teladoc physicians can diagnose, treat, and write prescriptions, when

necessary, for routine medical conditions for members of any age

- Cold and flu symptoms - Urinary tract infections

- Bronchitis - Respiratory infections

- Allergies - Sinus problems

RedBrick Health New Wellness Partner

RedBrick Health is our new vendor effective October 16. With RedBrick, we can

take our program to the next level! With RedBrick, you can:

• Analyze your current wellness level using a combination of an online Health

Assessment and Health Screening blood test.

• Work with a Health Advisor to analyze your current wellness profile and suggest

ways for you to utilize RedBrick Health’s resources.

• Join a team of co-workers and compete against other teams to lose weight, get

moving, or eat healthy (coming in 1st quarter!)

• Use online educational tools and programs to help you learn about current health

conditions and ways that you can prevent future issues.

Step 3: Complete a Health Screening (Screening cost is $55.00 if you are not enrolled in the medical plan and will be payroll deducted).

Upon completion of these 3 steps you will receive an online “HealthMap” and

your journey begins!

Wellness Discounts Your medical premium discount for the 2013 plan year is based on your

Total Prevention Score as measured in The Prevention Plan as of

October 15, 2012

• The Total Prevention Score is made up of your:

• Assessment – the results of your Health Risk Assessment and blood test

• Intervention – the activities you have done to help better your health and

wellness

• Both you and your covered spouse or domestic partner qualify

independently for the discounts

• The better your Total Prevention Score, the higher the discount you

could receive

• Scores of at least 796 will qualify

• The Associate’s discount will be applied against the Associate’s portion of the medical cost (if children are covered, the Associate’s discount will also apply to the children’s portion).

• The Spouse’s discount will be applied against the spouse’s portion of the medical cost.

• You must be enrolled in the Willis Medical Plan in 2013 to receive the discount

• The premium credit will first be applied to your medical plan cost. If there is a credit remaining you can use it toward any pre-tax benefit with the exception of a FSA or HSA.

• Any credit not used will be forfeited.

Wellness Discounts

Dental Plan Changes

• Premium for the MetLife Low and Ultra High Dental options will increase by 8%

• No premium increase for the Humana High Dental option

• If you currently waive dental, you will be limited to the Low or High plan options

in 2013. You can increase options by one level in subsequent annual

enrollments.

• If you are currently enrolled the Low Option Dental Plan you are restricted to the

High Option. You can increase by one level during subsequent enrollment

periods.

• If you are enrolled in the High Option Dental you can enroll in any option.

• You can add an eligible dependent to your current plan and/or change to any

dental option as outlined above.

• If you are adding a dependent for the first time, documentation will be required

Spending and Health Savings

Account Changes New Health FSA and Limited FSA limit is $2,500 (prior limit $5,000)

New Limited FSA vendor – Aetna (formerly Benefits Solutions) (Healthy Savings Plan only)

• Limited FSA participants will be issued debit cards

• Debit card can be used for only dental and vision expenses until the deductible is met.

• Once family deductible has been met the funds in your Limited FSA can be used for any eligible expense – including medical expenses (debit card will not work for medical – must use paper claim form)

• New IRS defined Health Savings Account limits (Healthy Savings Plan

only) • Single - $3,250 (up from $3,100) / Family - $6,450 (up from $6,250)

Aetna/Allstate New Group Accident Carrier

Enhancements to the plan including:

- Outpatient Physician’s Treatment benefit

- Greater ICU confinement, Accidental Death, and ER benefits

• - Does not terminate at age 70

If you are currently enrolled, your election will move to the new carrier.

If you wish to retain your Continental American/Aflac policy via direct bill,

you will need to:

•1) Visit the eEnrollment site (myhr.willis.com) to Waive the Group Accident

• Go online to enroll only if you want to:

• Complete your annual Tobacco-Free Declaration for you and/or your covered spouse/partner to receive your Tobacco-Free Credit

• Make changes to your current plan choices or enroll in a coverage you are not currently enrolled in

• Re-enroll in a Health FSA, Dependent FSA, Limited FSA or Health Savings Account (they will not carry over into 2013)

• Waive Group Accident if you are currently enrolled

What happens if I “do nothing?”

• Your 2012 elections will default (carry over) and become your 2013 elections

• Your elections for the following accounts will be waived for 2013:

• Health Flexible Spending Account

• Dependent Care Flexible Spending Account

• Limited Health Flexible Spending Account

• Health Savings Account

• You will be charged the Tobacco User rate even if you are not a tobacco user! This applies if you also cover your spouse or domestic partner (even if you declared you were tobacco free during last year’s annual enrollment). This is an annual declaration.

• You must enroll online and on time - Late submissions cannot be accepted!

• Credit Amount: $18.50 per pay period each ($37/month), for you and a covered spouse/partner (if applicable). Maximum credit - $74 per month.

• To receive your credit, you must affirmatively declare you and a covered spouse/partner, if applicable, are tobacco-free via the Willis Online Enrollment System each year (i.e. haven’t used tobacco more than 5 times in the last 90 days.) This is an annual declaration.

• The system assumes everyone, including a spouse or domestic partner, is a tobacco user. You will be asked to answer Yes or No for both yourself and your covered spouse/partner. We ask that you answer the question truthfully.

• If you have declared that you are tobacco-free via the online enrollment system, and you have made your benefit elections, you will see a Tobacco-Free Credit on your online Enrollment Summary page.

Tobacco is defined as any product containing, made or derived from tobacco that is

intended for human consumption, whether chewed, smoked, absorbed, dissolved, inhaled,

snorted, sniffed or ingested.

Dental Plan

• Three Plan options: Low, High and Ultra High • MetLife for the Low and Ultra High Options

• Humana for the High Option

• If you currently waive dental, you will be limited to the Low or High plan options in 2013. You can

increase options by one level in subsequent annual enrollments.

• If you are currently enrolled the Low Option Dental Plan you are restricted to the High Option. You

can increase by one level during subsequent enrollment periods.

• If you are enrolled in the High Option Dental you can enroll in any option.

• You can add an eligible dependent to your current plan and/or change to any dental option as

outlined above.

• If you are adding a dependent for the first time, documentation will be required.

• All Plans include 4 cleanings per year- combined regular and periodontal cleanings

• Implants covered subject to the annual plan maximums

Additional Plan features:

• Humana’s network includes many retail vendors, like LensCrafters and Pearl Vision.

• Members receive additional fixed payments on lens options including: anti-reflective coating and scratch resistant coatings.

• Members receive a 20% discount on a second pair of eyeglasses. Discount is available 12 months after the covered eye exam and available through the VCP network provider who sold the initial pair of glasses.

• After co-pay, standard polycarbonate are available at no charge for dependents less than 19 years old

Vision Plan

With a Health FSA, you can use your pre-tax dollars to help you pay for many health care

expenses that are not covered by a medical, vision or dental plan.

• $100 min/$2,500 max (max changed in 2013 due to Health Care Reform)

• Can incur expenses through March 15, 2014 towards 2013 balance

• If enrolled in the Healthy Rewards Plan, you can determine if the Health FSA or your HRA

will pay first

• To keep process totally electronic, choose Health FSA pays first option

• OTC medications are not reimbursable under the Health FSA without a prescription due to

Health Care Reform. A full list is available on www.willisbenefits.com

• To be reimbursed from your Health FSA for an OTC medication you will need to submit a

doctor’s prescription.

• Your Aetna Debit Card cannot be used at the checkout for over the counter medications that

require a prescription. You can still use your Debit Card for other OTC items such as

contact lens solutions or bandages.
