Key Changes

Assessment forms– No more Core Part 1 and Core Part 2 – just Core

Assessment– The completed by and completed date fields have been

removed from most assessment forms.– A number of forms now have links to other related forms

or areas such as care planning – these links open those forms in a new window.

– The 3 Referral Related forms that made up Core Part 1 have now been merged into one form called Presenting Situation and Referral Outcome Decision. The presenting situation is a single text box as is the Current Interventions including medication.

– Previous versions of the forms will either be available in the index or in the case of Current Interventions including Medication and Referral Outcome Decision they are in the (new) archived forms section.

– MCA and Information Sharing & Consent forms are now in a separate section from the rest of the Core Assessment

Impact – Assessment Forms

• Mostly good!– quicker to navigate, fewer boxes to enter information in, Risk

Assessment pulls through text from the previous version when you create new (can be edited)

• Potential concerns:– where forms have changed (Personal & Social Information form

becomes Personal & Family History etc) if you create new, only some of the previous information will be pulled through to the new form. The previous version can be accessed and edited (including copy & paste to the new) so the existing form can continue to be added to during the existing episode of care.

– Where there are fewer text boxes, a wide range of information can go into one box and this means there is more potential to overlook an aspect of assessment or for it to be harder to find in the form particularly after a few additions.

– Links to ‘Current Medication’ go to the prescribing part of RiO which is not currently in use

– Preferred name used to be added to the Additional Personal Information form in order to go onto the front of the case record. It will now come from the demographics screen and the preferred name box has been deleted from the Additional Information form. It is not clear whether this will remove the preferred name from the case record. If it does, it will need to be manually added in the Client Names bit of the demographics.

• Please note – the Forensic History form becomes the Forensic & Probation History form and NO data from the existing version is pulled through to the new one when you select ‘create new’.

• The existing form will continue to be available when you click on Forensic & Probation History and this can continue to be edited during an existing period of care.

• However, if someone is discharged and comes back to services at a later date, a new form would be created and relevant data from the old form would need to be copied across using copy and paste.

Risk Assessment

• This form now pulls through text from the previous version when you create new – this is editable.

• There is an additional field asking if safeguarding has been considered in relation to this client

• There is a tick box to indicate if a victim should be notified of leave – if this is left blank, it shows as ‘no’ on the front of the case record, if ticked it shows as ‘yes’.

• Everything else remains the same

NotificationsThis is the Notifications icon. Notifications is a basic messaging system within RiO. It has not been switched on so far as we need to test it first.

RiO generates some automatic messages when you carry out certain actions so if we do switch it on you will find some messages waiting

When it is switched on, we will issue some guidance regarding it’s use.

The key thing to remember is that it is not a substitute for email – there is no trail, if you delete or forward one you will have no record of the notification.
