NURSING DIAGNOSIS  ANALYSIS GOALS AND OBJECTIVES INTERVENTION RATIONALE EVALUATION Diarrhea r/t Infectious processes.   Acute Gastroenteri tis is an inflammatio n of the stomach and intestinal tract that primarily affects the small bowel. GOAL -- After 2-3 hours of nursing interventions, the patient¶s mother will gain knowledge about diarrhea. OBJECTIVE - Verbalize understanding of causative factor. -Verbalize the rationale for treatment regimen. Long Term: -- After 1-2 daysof nursinginterventions,t he patient willbe free of diarrhea. ExpectedOutcome:-- Re-establishand maintainnormal bowelmovement.-- Reduction infrequency of stools. - Return of thestool to thenormalconsistency.  Independent: 1.) Auscultate theabdomen.2.) Discuss to themother the differentcausative factors andrationale for treatmentregimen.3.) Restrict solid foodintake.4.) Provide forchanges in dietaryintake.5.) Limit caffeine, highfiber foods and fattyfoods.Dependent: 1.) Administer anti- diarrheal medications ,as indicated.2.)  Administermedication s, as ordered. Independent: 1.) For presence.Location andcharacteristics of bowelsounds.2.) For the education of the patient¶s mother.3.) To allow for bowelrest and reduceintestinal workload.4.)
