Discharge Navigator Workflow (Provider) The following steps cover the patient discharge workflow for providers. 1. Click Discharge navigator. 2. Click Cosign Orders and then sign any remaining orders, as appropriate. 3. Click Providers/Locations and then, if known add the PCP / follow up or click Add ME if you are the follow up provider. 4. Click Discharge Summary and then enter your D/C note. 5. Click Discharge Instructions and then enter your discharge instructions SmartPhrase, or an Epic Discharge SmartText (e.g DC ADULT). 6. Click Charge Capture and then document any charges, if appropriate. Order Reconciliation 1. Review Orders for Discharge – Complete Order reconciliation of all current orders (add pharmacy if e-prescribing medications and there is not a pharmacy on file) 2. New Orders for Discharge – Complete the Discharge order and then add any additional orders to be carried out (e.g. referrals, DME, etc) and medications that are to be sent to the pharmacy for patient pick up. ① ② ③ ④ ⑤ ⑥
