    VHF Radio is your first choice of technology to ask for professional help in case of an emergency. If no response, try to call MRCC using Satellite- or Cell Phone (numbers listed on page #7). Range of VHF is 25nm (50km). General Rule: Mobile Phones shall not be used in case of Emergency!

    Decompression Sickness (DCS)

    Till today researchers are not able to find any antidote for tetrodotoxin which is a potent poison found in puffer fish. Thus treatment is entirely supportive and symptomatic. The sooner the patient takes treatment lesser is the mortality risk. If puffer fish is consumed within few hours of developing symptoms, doctors may induce vomiting to flush out content from the stomach and prevent absorption of left over toxin.

    Patient is given gastric lavage with activated charcoal which can bind with the toxin that is still not absorbed. Other vital parameters such as respiratory rate, pulse rate, blood pressure are constantly monitored and treated accordingly in ICU.

    The first 24 hours after poisoning from puffer fish are critical. Usually if patient survives first 24 hours the prognosis is much better. Patient may survive without any residual deficits.

    ▪ Remove the exposed person from the water to prevent drowning.

    ▪ Immerse the wound for 30 to 90 minutes in water as hot as the poisoned person can tolerate (up to 60 C) because the poisons are heat-sensitive. Repeat as necessary to control pain.

    ▪ Local or regional anesthetics (blocks) may be useful

    ▪ Use tweezers to remove any spines in the wound using caution to not squeeze venom glands that may have broken off in the wound with the spine. Scrub the wound with soap and water. Then flush the affected area with fresh water.

    ▪ Do not apply tape to close the wound as this may increase the risk of infection.

    ▪ Patients may need a tetanus booster; this is usually recommended for all patients with this type of poisoning.

    General Rules MOB Emergency – Steps to Rescue

    Supporting Technology

    «MOB» – Man Over Board Procedure

    ▪ Always try to keep an eye on the victim▪ Don’t risk the boat and the crew by performing

    dangerous manoeuvre (especially under sail)▪ Use the MOB function on the Plotter or VHF (Plotter is

    recommended)▪ Throwables: Rescue ring at the back of the boat, any

    fender, life wests in the ceiling of the rear cockpit ▪ Avoid that another person is jumping into the water to

    help the victim

    ▪ Shout “Man Over Board”

    ▪ Keep an eye on the victim, or ask someone to do so

    ▪ Throw something floatable and visible into the water to mark position, and to have a reference for the current

    ▪ Activate «MOB» on Plotter or VHF (see pages #13 and #25)

    ▪ Stop the boat by sailing into the wind (AWA 0 degrees), or put the engine speed to idle

    ▪ If under sail, bring the boom to «middle position»

    ▪ Take all sails down, or at least roll in the Genoa sail

    ▪ In case the victim carries a MOB-1 device, watch for the Alert Signal on the Plotter (may takes up to 1 minute)

    ▪ Navigate the boat towards the MOB position (under engine), indicated on the plotter (either MOB-1 or the position set manually)

    ▪ If you can see the victim, throw something floatable towards the victim

    ▪ Navigate to the victim from “Luv”, and recover him from the back of the boat

    ▪ In case you cannot find the person within 5 minutes, call Mayday using radio (see page #24)
