INDEPENDENT SERVICE COORDINATION

• Notice of Funding Opportunity – Fall 2018• Merit-based scoring process

• Results announced early 2019• Total of 12 geographic areas • Total of 8 different agencies

• Transition activities underway for all areas with changes except for Madison and St Clair counties (currently served by DDSME)

• Transitions complete and changes effective July 1, 2019• DHS has entered into settlement negotiations with DDSME in

regards to suits filed in St Clair County Circuit Court• DDSME remains the ISC until settlement agreement reached

• Prairieland assumed responsibility for ISC service in the Rock Island/Mercer counties (CSO/RIM area) effective May 1, 2019.

• ISC manual revisions underway and will be posted to website prior to July 1, 2019.

• ISCs will remain responsible for all case management activities for persons receiving Medicaid Home and Community Based Waiver services.
