Inquiry button, populate CMO ID associated with the user.2 When a date is entered in the From and To Date field and the Search button is clicked,

the data from Care Select Notification table will populate from the search date(s) in the Reported Information section.

4 When the Export List button is clicked, export data to .CSV file.Note:Type of Service field will allow the user to filter the service types.

When the Reset button is clicked, the From and To Date field will be cleared from the Care Select Notification Inquiry page.
