Certificate of category (OBC/SC/ST) issued by the competent authority in the prescribed format available in CCB-2012 information brochure. VII. Certificate for Persons with Disabilities (PD), if applicable VIII. Passport, if the candidate has passed the qualifying examination from outside India. IX. Proof of place of examination Center of the final examination from the National Open School for candidates having senior secondary certificate. X. Transfer Certificate (not mandatory while reporting at RC if qualifying examination pass certificate/mark sheet shows place of education. Mandatory at the time of admission) 4. Two sets of self- attested copies of all the documents listed above. The original will be returned after verification and self- attested copies will be retained by the Reporting Center. 5. Two passport size photographs identical to the one pasted on the AIEEE 2012 application Form. II. Score Card of AIEEE 2012 (Original or Printed copy from AIEEE site)

IV. Pass Certificate and Marks sheet of qualifying examination.

V. If compartmental candidate, (10+2) mark sheet and compartmental admit

card.

VI. Certificate of category (OBC/SC/ST) issued by the competent authority in

the prescribed format available in CCB-2012 information brochure.

VII. Certificate for Persons with Disabilities (PD), if applicable

VIII. Passport, if the candidate has passed the qualifying examination from

outside India.

IX. Proof of place of examination Center of the final examination from the

National Open School for candidates having senior secondary certificate.

Payment of Initial Fee

Initial fee is 35,000/- for OP, OPPH, OBC and OBCPH and Rs. 25,000/- for SC, SCPH, ST and STPH for Round I, II & III.

Initial fee is 45,000/- for OP, OPPH, OBC and OBCPH and Rs. 40,000/- for SC, SCPH, ST and STPH for Spot Round.

Mode of payment: Credit/Debit card cards (Visa/ Maestro/Master) or through e-challan or through NEFT or through Demand Draft. For spot round payment should be made through Demand Draft only.

The Demand Draft is to be drawn in favour of “Chairman, CCB - 2012”, payable at Rourkela.

In the fourth round candidates who have already taken the provisional admission and have paid the initial fee at the Reporting Center shall pay the remaining amount of the tuition fee (as applicable to OP/OBC/SC/ST candidates as the case may be) at the Allotted Institute. Candidates, who have been allotted a seat for the first time in the fourth round, shall pay the full admission fee at the Allotted Institute directly. Admission fee should be paid by cheque/cash/DD as per the rules of the Allotted Institute. The cheques or DDs in these cases should NOT be drawn in favour of Chairman CCB-2012.

Payment of Initial Fee through Debit/Credit card

On clicking the ‘Online Payment Using Debit/Credit Card’, following message will be displayed as per candidate category.

Candidate can make the online payment using Credit/Debit card through Axis Bank Payment Gateway services

Keep the details of Credit Card (VISA/Master/Maestro Card) ready.

Login to the CCB website, fill the requisite information like Roll number, Date of Birth and Stream.

There will be link to “Make Payment Online”. On clicking the link, Payment Gateway screen will be displayed. Follow the instructions to make payment.

Make the payment through Credit Card and note down the payment receipt number, after successful payment of fee and take a printout of Payment receipt.

Take a printout of the receipt that is generated after successful payment transaction and produce it at the time of reporting at RC and submit a copy of it to RC.

Payment of Initial Fee through e-challan

On clicking the ‘e-Challan of Axis Bank’, a filled e-challan will be generated.

Candidate should take the printout of the generated e-challan and deposit the requisite amount in any branch of Axis Bank.

Bank will retain its own copy and return back the remaining two copies to the candidate.

Candidate should submit the CCB copy to RC official at the time of Reporting.

Payment of Initial Fee through NEFT (National Electronic Fund Transfer) On clicking NEFT, a filled in NEFT form will be generated.

Candidates should take a print out of the generated NEFT application and deposit the same in any bank other than Axis Bank.

After executing the transaction the bank will provide the UTR (Unique Transaction Reference) number.

The Candidates should submit the UTR No/ Reference number in case of online net transfer of fund at the time of Reporting.

Candidates having net banking access and fund transfer facility of any bank including Axis Bank can transfer the fund online as follows:

a. Keep the auto generated online NEFT form ready. b. The auto generated NEFT application contains all details like account number, IFS

code, name of the account, branch name, amount etc. c. Log in to the net banking site of the bank. d. Choose Fund Transfer option. e. Add new beneficiary details as mentioned in the generated NEFT form. f. Wait for the approval of the beneficiary. g. Once the beneficiary (CCB-2012) is added or registered in the net banking then

proceed for fund transfer. h. Successful fund transfer will generate a reference number. i. Produce this reference number to CCB-2012 at the time of reporting.
