    NOH for Health 1999-2004 | Health Sector Reform Agenda

    Highlights of Secretary FTD's Accomplishment from 2005-2007

    HIGHLIGHTS OF ACCOMPLISHMENTS

    Secretary Duque is a dedicated health leader who has injected a lotof dynamism and innovation in how things are done and

    accomplished in the health sector since he began his term in 2005 asthe appointed Secretary of Health.

    A man of action and reform, Secretary Duque charted a clear roadmap for the ehalth sector, which he called the Fourmula One forHealth (F1). The F1 strategy specified clear targets and identifiedpriority projects and activities (PPAs) of the DOH for the medium-and long term emphasizing the needs to focus attention toward theattainment of the Millennium Development Goals and the NationalObjectives for Health for 2005-2010.

    Through F1, Secretary Duque prioritized the four pillars of FourmulaOne:

    1) Health Financing - sustaining and increasing investments forhealth with Philhealth as the center of health financing initiatives.

    2) Health Regulations - flexing the regulatory muslces of DOH toensure safe, affordable and quality health goods and services (i.e.strenthening of BFAD and other regulatory offices of DOH; making

    the Low Cost Medicines Program the key focus during his term)

    3) Health Service Delivery- efficient and effective delivery of healthservices with doable and cost-effective interventions and

    4) Good Governance - improving public health service deliverythrough integrity development and elimination of wastages andcorruption in the DOH.

    On his third year as the leader of the health sector, his reforms haveenables DOH to improve health and reach the poor more effectively.

    He has maintained and sustained PhilHealth's coverage now at73%. With a dramatic increase in Philhealth's budget in 2008,universal health coverage can be reached and 5 million indigentfamilies are expected to be covered with an adequate healthsafety net.

    He continues to increase PhilHeatlh's social relevance throughenhanced benefits such as the expanded Maternity package,the Newborn Benefits Package, the Avian Flue package, and theoutpatient packages for HIV/AIDS, TB and Malaria, all inkeeping with the country's commitment to race toward the

    Milllenium Development Goals by 2015. The poor has greater access to low-cost, high quality medicines

    as shown by the SWS 2006 survey that more than 50% ofFilipinos are now buying and using generics. Close to 11,000Botika ng Barangays have been established nationwide plusmore than 1,200 Botika ng Bayan outlets operating incoordination with PITC since the Low Cost Medicines Programwas launched in 2001. With the integration of the goals andfunctions of DOH and PITC under his leadership, essentialmedicines and drugs continue to be more affordable andaccessible to the public generating overall consumer savings upto 80%.

    Secretary Duque has also earned oversight of the government'smulti-agency drive to mitigate hunger and malnutrition in thecountry. In 2006, he was named by the President as the anti-hunger czar of the Philippines and the National NutritionCOuncil was transferred to the DOH as one of its attachedagencies. The Accelerated Hunger Mitigation Program (AHMP)is currently the lead program of the national government in

    combating the probelm of malnutrtion particularly among yungchildren from poor families. The AHMP consists or targetedinterventions designed to address the supply and demand sidefactors contributing to the dynamics of hunger and poverty inthe Philippines.

    Secretary Duque has also placed the health mothers andchildren at the center of health sector reform consistent withour pledge to cut maternal and child deaths by 2015 (MDG 4and 5).

    o

    He is tenacious in his campaign for Safe Motherhood andis consistent with his advocacy that all women have theright to safe and quality emergency obstetric services(EmOC) to prevent maternal and newborn deaths. By2010, he wants to reverse the current unacceptable realitywhere 60% of women deliver at home and are attended byunskilled health professionals. Target is 100% of allwomen giving birth in health facilities attended by doctors,nurses and midwives.

    o He is resolute in completing the massive push towards

    Universal Child Immunization. In 2005, he did notwaste time to avert a potential pilio resurgence in theMindanao region by conducting mass polio immunizationafter polio reemerged in neighboring Indonesia. In 2006,he signed into implementation the 100% Hepa Bimmunization policy for all infants to counter the highprevalence of Hepatitis B in the country --- the leading

    cause of chrrhosis and liver cancer in the Philippines. Andvery recently, he led the DOH into another victory with a95% coverage during the 2007 Knockout Tigdas campaign.

    o His advocacy for Breastfeeding is recognized hearglobally. His term as Secretary of Health will be

    remembered with his signing of the Revised IRR of theNational Milk Code, the revitalization of the Mother BabyFriendly Hospital Initiative of the DOH, breastfeedingstations in SM supermalls, and the Supreme Courtupholding all but three provisions of the IRR scoringanother big win for the Department.

    In the fight against proverty-related diseases (MDG 6), thefollowing were his accomplishments:

    o TB - hitting and sustaining TB control targets adn theformulation of new policies and guidelines for nationwideprevention and control of TB among children, TB co-infection with HIV/AIDS and management of multiple andextensively drug-resistant TB (MDR and XDR-TB)

    o Malaria - Declaration of more malaria-free provincesbringing the total number to 22

    o HIV/AIDS - kept the HIV/AIDS prevalence at 0.03/100,000population. He has also pushed for the approval of theoutpatient HIV/AIDS package of Philhealth to coverantiretroviral medications.

    o Secretary Duque also launched the Disease-Free ZonesInitiative of the DOH for easily eradicable diseases suchas rabies, shcistosomiasis, filariasis and vaccine-preventable diseases.

    Under his leadership, the National Avian Flu PandemicPreparedness Plan was also crafted. Currently, the DOHmaintains a stockpile of Oseltamivir good for 10,000 treatmentcourses. The country's capability and preparedness in rapidcontainment should avian flu arise was recently tested in the

    PANSTOP II simulation exercise conducted by the World HealthOrganization. While he has undeniably set a record-high of wrestling with 23

    major health disasters and emergencies in 2006 alone, he ismore proud of beating corruption and inefficiency at the DOH.

    Through his leadership example and with a bold directive toexercise good governance in the health sector, DOH embarkedon the implementation of reforms for integrity development,transparency in government transactions and corrupt-freepractices since 2005.

    In 2007, the DOH received the most number of recognition fromPAGC, NEDA, the Office of the Ombudsman, local andinternational development partners as well as different surveyoutfits highlighting the seriousness of the current DOHadministration to bosst performance, increase transparency andaccountability and institute good governance in the healthsector.

    o #1 IN ALIGNING DONOR INITIATIVES WITH HEALTH

    SECTOR PROGRAM (National Economic DevelopmentAuthority, 2007)

    The reforms of DOH through F1 boosted partnerships andstakeholder participation at the local and iternational levels.Effective donor coordination pooled some $530 million worth ofoffical development assistance (ODA) targeting the key priority

    programs, projects and activities of the DOH in the 37 F1 provincesof DOH where reforms are being implemented together with thelocal governments. International partners include the World Bank,

    the European Commission, the Asian Development Bank, the WHO,UNICEF, UNFPA, JICA, USAID, AusAID, GTZ, among others.

    #1 IN INTEGRITY DEVELOPMENT REVIEW AMONG 11GOVERNMENT AGENCIES (Office of the Ombudsman,Development Academy of the Philippines and AsianDevelopment Bank, 2007)

    A joint study of the ADB, the DAP and the Office of the Ombudsmanrecently rated DOH as the "best performer" among 11 public sectoragencies in terms of corruption prevention and integritydevelopment (2nd cycle).

    The DOH got the highest rating of 3 in five out ten parameters:leadership, human resource management, performancemanagement, procurement management and managing interfacewith the external environment. A score of 2 was also given to theHealth department for financial management and corruption reskmanagement.

    #2 MOST SINCERE GOVERNMENT LINE AGENCY INFIGHTING CORRUPTION AMONG 80 GOVERNMENTAGENCIES(Presidential Anti-Graft Commission, 2005 and2006)

    For 2005 and 2006, DOH earned the #1 ranking given by thePresidential Anti-Graft Commission in terms of its compliance inimplementing its Integrity Development Action Plan. For 2007, DOHlanded #2 in tis anti-graft measures next to DSWD among 80

    government agencies.

    The Integrity Development Action Plan involves projects andactivities focusing on systems improvement, values formation andmoral recovery aimed at eliminating graft in all branches andagencies

    GOOD PERFORMANCE IN FIGHTING CORRUPTION(SWSBusiness Survey on Corruption, 2007)

    For two straight years, the DOH earned the approval of the MakatiBusiness Club in terms of its seriousness to fight and eliminate graftand corruption within the bureacracy. In 2006, the DOH got a netrating of +22 (MODERATE performance) and placed #1 among allgovenrment line agencies next only to Local Church Leaders, theSupreme Court and SSS.

    In 2007, the DOH improved ints net rating to +26 (GOODperformance).

    "VERY GOOD PRACTICE" IN IMPLEMENTING THE PARISDECLARATIONS ON AID EFFECTIVENESS AND THE

    SECTOR DEVELOPMENT APPROACH IN HEALTH (SDAH)AMONG COUNTRIES REVIEWED IN SOUTHEAST ASIA(European Commission Study in Southeast Asia, 2006)

    The Philippines sector Development Approach in health (SDAH) wascharacterized as "VERY GOOD PRACTICE" among SEA countries intermes of its commitment to the 2005 Paris Declaration onalignment and coordination of policies and harmonization of donorimputs to the health sector reform program.

    The SDAH approach of DOH was found to be fully compliantto"Best Practice" efforts in 4 out 9 essential characteristics of SDAHand partially compliantin 5 out 9 parameters.

    4. Increased coordination and collaboration between governmentand donors

    Department of Health Profile

    The recent change in political leadership provides the Philippineswith the chance to revitalize the health care system. This is in linewith the administrations thrust to prioritize delivery of services tothe masses and improve the quality of life of all Filipinos, especiallythe poor. The differences in health status among various groups andregions in the country have widened through the years. Thesedisparities indicate deficient economic and social policies, showingthe need to reprioritize interventions to promote equity, fairness andimmediate action. Unnecessary and unfair gaps in the health caredelivery system that deprive the poor of access to basic servicesmust be reduced. The system must work efficiently to reach the

    highest possible health standards that can be shared by all Filipinos,given the limited resources available for health. Althoughsocioeconomic differences significantly influence health status, theequitable distribution of quality health services is an importantmeasure of fairness in the country. Revitalizing the health caresystem must be seen within the broader context of several forcesaffecting the delivery of basic health services in the past two

Among these factors are the devolution of health servicesto local governments, passage of national legislation for universalcoverage for health through social insurance, the epidemiologic shiftand current double burden of disease brought about by the rise indegenerative diseases and the reemergence of previously controlled

    infectious diseases, demographic trends pointing to longer life span,greater number of adolescents and youth, rapid urbanization,industrialization, environmental degradation and climate change.Under these realities, the health sector must work to attain acommon vision of health for all Filipinos. Its mission is to ensureaccessibility and quality of health care to improve the quality of lifeof all Filipinos, especially the poor.

    The DOH Offices

    The DOH is composed of about 17 central offices, 16 Centers forHealth Development located in various regions, 70 hospitals and 4attached agencies.

    Central Office

    The central office is composed of the Office of the Secretary and fivemajor function clusters:

    Office of the Secretary

    The following comprise the staff support services to the Secretary ofHealth: Health Emergency Management Staff; Internal Audit Staff,the Media Relations Group and the Public Assistance Group including3 major Zonal Offices of the DOH located in Luzon, Visayas andMindanao. These Zonal Offices are headed by an Undersecretary andsupported by an Assistant Secretary. These offices are mandated tocoordinate and monitor the implementation the Health SectorReform Agenda, the National Health Objectives and the Local

    Government Code with the various Centers for Health Development.

    It is composed of Health Human Resource Development Bureau andthe Health Policy Development and Planning Bureau.>

    Center for Health Development

    Responsible for field operations of the Department in itsadministrative region and for providing catchment area with efficientand effective medical services. It is tasked to implement laws,regulation, policies and programs. It is also tasked to coordinate withregional offices of the other Departments, offices and agencies aswell as with the local governments.

    DOH Hospitals

    Provides hospital-based care; specialised or general services, some

    conduct research on clinical priorities and training hospitals formedical specialisation.

    Attached Agencies

    The Philippine Health Insurance Corporation is implementing thenational health insurance law, administers the medicare program forboth public and private sectors. The Dangerous Drugs Board on theother hand, coordinates and manages the dangerous drugs controlprogram. The other two agencies are Philippine Institute of

    Traditional and Alternative Health Care and the Philippine NationalAIDS Council.

    The Presidential Anti-Graft Commission recognized and awarded theDOH as the number one government agency in fighting corruption.DOH also topped in the Pulse Asia 3rd Quarter Survey as the numberone government agency in terms of overall performance.

    2005FOURmula ONE for Health (F1) was launched as the health sectorsblue print for the implementation of reforms to bring about betterhealth outcomes, more responsive health system and more equitablehealthcare financing. Province-wide Investment Plans for Healthwere developed in 16 provinces as the basis of F1 implementation inthese sites.

    2003

    The One-Script Systems Improvement Program was established (AO50. S. 2003) to orchestrate unity, synchronicity and focusedtargeting of priority public health programs that would provide thebiggest impact to attaining equity, efficiency, access and qualityhealth care in the country. A major breakthrough was achieved inproviding fiscal autonomy to 68 DOH retained hospitals with theapproval of a special provision of FY 2003 GAA which authorized100% retention and the use of hospital income for upgrading ofhealth facilities and services.

    2001

    In July 13, 2001, Administrative Order 37 which contained theguidelines on the operationalization of the HSRA implementationplan was signed by Sec. Manuel Dayrit. It is also during this year thatthe 13 convergence sites or the advance implementation areas havebeen established.

    2000The year 2000 marked the institutionalization of the Health SectorReform Agenda (HSRA). The HSRA was endorsed for approval andsupport by the National Government Agencies, national and localstakeholders in health, and partners in the international community.

    The HSRA has become the major framework for policies andinvestments for the health sector .

    1999

    The functions and operations of the DOH was directed to becomeconsistent with the provisions ofAdministrative Code 1987 andRA 7160 through Executive Order 102. The Health SectorReform Agenda of the Philippines, 1999-2004 was launched.

    The reforms are: provide fiscal autonomy to government hospitals;secure funding for priority health programs; promote the

    Full implementation ofRepublic Act No. 7160 or LocalGovernment Code. The DOH changed its role from one of

    implementation to one of governance. Significant change: branchingout of the Office of the Public Health Services to form the Office forSpecial Concerns. Two big offices merged to become the Office ofHospital Facilities, Standards and Regulation. Special projects werehighlighted like the NID. National Micronutrient Campaign, DisasterManagement, Urban Health and Nutrition Project, TraditionalMedicine, Doctors to the Barrios Program, "Lets DOH It"! became anational battlecry.

    1987

    Another re-organization under Executive Order No. 119, whichplaced under the Secretary of Health five offices headed by anundersecretary and an assistant secretary. These offices are theChief of Staff, Public Health Services, Hospital and Facilities Services,Standard and Regulations, and Management Service.

    1986

    The Ministry of Health became Department of Health again.

    1982

    Under Executive Order No. 851, the Health Education and ManpowerDevelopment Service was created, and the Bureau of Food andDrugs assumed the functions of the Food and Drug Administration.

    1972

    Through Letter ofImplementation No. 8, pursuant to PresidentialDecree No.1, Sept.24, 1972, the DOH was renamed Ministry ofHealth. The National Cancer Center and Radiation Health Servicewere created. The Ministry was divided into 12 regions coveringseveral provinces and cities under a regional health director.Attached offices were the Philippine Medical Care Commission, theDangerous Drugs Board, National Nutrition Council, Population

    Under Executive Order No. 392, s. 1950, the Department ofHealth gained additional functions brought about by the transfer ofthe Institute of Nutrition, together with the Division of BiologicalResearch and the Division of Food Technology from the Institute ofScience, and the Public Schools Medical and Dental Services from theOffice of the President of the Philippines and the Bureau of PublicSchool respectively to the DOH. Within the Department of Health,certain changes were also effected thru the transfer of the Divisionof Health Education and Information from the Bureau of Health to theDepartment proper, in exchange for which the drug InspectionDivision was transferred to the former office from the latter; theconversion of the Section of Tuberculosis into a Division of

    Tuberculosis, directly under the Office of the Secretary, and theconversion of the Division of Laboratories into an Office of PublicHealth Research Laboratory.

    1947

    Reorganization of government offices under Executive Order No. 94,series of 1947 with the transfer of the Bureau of Public Welfare tothe Office of the President and the Department was renamedDepartment of Health (DOH). Under this set-up were the following:

    01 January 1941

    Creation of the Department of Health and Public Welfare asprovided for in Executive Order No. 317, series of 1941. TheDepartment was under the Secretary of Health and Public Welfare

    and also included the Bureau of Quarantine; the healthdepartment of chartered cities; the provincial, city and municipalhospitals; dispensaries and clinics, the public markets and slaughterhouses; the health resorts; and all charitable and relief agencies.However, the Philippine General Hospital was detached from theDepartment and transferred to the Office of the President of thePhilippines.

    Act. No. 157 of the Philippine Commission - creation of the Board of

    Health for the Philippine Islands; it also acted as the Board of Healthfor the city of Manila

    Acts Nos. 307, 308 and 309-establishment of the Provincial andMunicipal Boards of Health, completing the health organisation inaccordance with the territorial division of the Islands.

    Environmental Health

    Environmental Health is concerned with preventing illness throughmanaging the environment and by changing people's behavior toreduce exposure to biological and non-biological agents of diseaseand injury. It is concerned primarily with effects of the environmentto the health of the people.

    Program strategies and activities are focused on environmental

    sanitation, environmental health impact assessment andoccupational health through inter-agency collaboration. An Inter-Agency COmmittee on Environmental Health was created by viruteof E.O. 489 to facilitate and improve coordination among concernedagencies. It provides the venue for technical collaboration, effectivemonitoring and communication, resource mobilization, policy reviewand development. The Committee has five sectoral task forces onwater, solid waste, air, toxic and chemical substances andoccupational health.

    Expanded Program on Immunization

    Children need not die young if they receive complete and timelyimmunization. Children who are not fully immunized are moresusceptible to common childhood diseases. The ExpandedProgram on Immunization is one of the DOH Programs that hasalready been institutionalized and adopted by all LGUs in the region.Its objective is to reduce infant mortality and morbidity throughdecreasing the prevalence of six (6) immunizable diseases (TB,diphtheria, pertussis, tetanus, polio and measles)

    Special campaigns have been undertaken to improve furtherprogram implementation, notably the National Immunization Days(NID), Knock Out Polio (KOP) and Garantisadong Pambata (GP) since1993 to 2000. This is being supported by increasing/sustaining theroutine immunization and improved surveillance system.

    Update No. 17 - Influenza A(H1N1) Monitoring

    May 21, 2009

    As of May 21, the Department of Healthhas identified seven new cases under

    observation, five of whom were from NCR, one from Region 6 andone from Region 8. All cases had history of travel from Influenza A(H1N1) affected countries. Four of the new cases laboratory testresults for Influenza A (H1N1), together with three cases reportedpreviously, are still pending. All other previous cases tested negativefor Influenza A (H1N1). While the intensive laboratory testing

    procedures for individuals possibly infected with Influenza A (H1N1)is on-going, the DOH reiterates that influenza-like illness monitoringat the community level is also enhanced. This is to ensure thatoutbreaks of influenza are detected early and control measures areimplemented immediately.

    required in order to reach certain basic health objectives. In order toachieve these health objectives provisions for the basic requirementof preventive, promotive and curative health care services must betop priority.

    An essential component of a health policy includes measures topromote the rational use of drugs and ensure the availability ofmedicines of adequate quality at a reasonable price. To date the costof medicines in the Philippines remains hign and is even higher by40% to 70% compared to other ASEAN countries. Moreover, ourbudget for drugs and medicines is too small to procure sufficientquantities of low-cost, essential drugs for the population in need.

    In 23 July 2001, H.E. President Gloria Macapagal-Arroyo made acommitment to lower the prices of drugs and medicines frequently

    bought by the poor by 50%.

    How does Pharma 50 intend to attain its objective?

    The strategies that Pharma 50 will employ are:

    A. Short-term (2001 SONA - 2002 SONA)1. Continue the importation of high quality, safe, effective andaffordable drugs and medicines for as long as the prices in thecountry remain high.

    2. Expand, as appropriate the list of drugs and medicines forimportation. The list will contain essential drugs needed to treat theprevailing causes of morbidity and mortality.

    3. Increase the number of outlets. Initially, the outlets will be limitedto public health facilities i.e., DoH and LGU retained hospitals, Botikang Barangay, and NFA rolling stores. However, initial meetings withprivate retail outlets are being conducted to draw up the guidelinesfor their involvement.

    B. Medium and Long-term (SONA 2002 and beyond)

    1. Promote the use of Generic drugs and medicines. Basically,generic counterparts of branded drugs are far lower in prices.However, generic product in the country has very low acceptability

    to the medical practitioners and general public, as reflected in thesales and use. There is a need to ensure the quality, safety andefficacy of these products through regulations and other measures.

    2. Ensure continuous supply of high quality, safe, effective andaffordable drugs and medicines, whether imported or locallymanufactured. (i.e.Toll Manufacturing for selected drugs andmedicines, the PITC to procure for government health facilities,encourage local production, etc).

    3. Develop reimbursement scheme for medicines with PhilHealth(Reference price for 100 most commonly needed/claimed drugs andmedicines, PHIC to reimburse Generic drugs, etc).

    4. Work with the pharmaceutical industry/organization/companies forthem to reduce significantly the prices of drugs and medicines.

    5. Work with the local retail industry for them to pass on to theconsumers the low price of drugs and medicines, and

    6. Wage war versus substandard drugs.
