APPLY FOR PURPLE HEART LICENSE PLATES If you are a current member of the Military Order of the Purple Heart you may apply for Connecticut Purple Heart License Plates by taking the following actions:

1. The cost to convert your plate to one with the PH background is $10 while a special vanity plate is $79.

2. Complete the Special Interest Plate Application form below. 3. Do not complete the portion of the form that is related to the Name of the Organization,

Org. ID Number, Name of Organization Liaison and Signature of the Organizational Liaison. These sections will be completed by the MOPH Representative.

4. Enclose a personal or bank check for the appropriate amount made out to DMV (Department of Motor Vehicles).

5. Print the Application Form. 6. Send the check and the application to the MOPH Representative at the following address:

If you are not a member of the Military Order of the Purple Heart you must also apply for membership. You may do this by submitting both the License Plate and Membership Applications together. Be advised that the License Plate Application will be held until approval of membership is made by the National Headquarters of the Military Order of the Purple Heart.. Along with the membership application a check for $50, made out to MOPH is required. Submit both applications and checks to the address above. Also remember to provide the required proof of receipt of the Purple Heart. Acceptable documents are identified on the membership Application Form.
